Key Features:
– Requires applicant to submit application materials in two components: 1. WEB-BASED COMPONENT … includes online submission of application and
application fee.
2. PAPER-PACKET COMPONENT… includes submission of all paper supporting materials, such as official transcripts, copies of certifications and licenses, proposed plan of study [MSN – CNL applicants only], etc. in a single packet.
– Requires references to submit Recommendation Forms online. – Allows applicants to track their application materials’ status online.

PART 1. WEB-BASED COMPONENT
– Application must be made online.
– Includes $50 application fee (non-refundable) that must be submitted online.
– Recommendation Forms mustbe submitted online by applicant’s references.
Notes: (1) The applicant must provide names & email addresses of three references as part of the online application. Applications without this information cannot be submitted.
(2) References (aka “recommenders”) listed by applicant will be sent an email with the Recommendation Form attached – and asked to complete and submit a completed version of the Recommendation Form online. (3) The online application must be submitted on or before the application deadline.
PART 2. PAPER-PACKET COMPONENT
– Applicantis responsible for collecting all paper application materials, such as official transcripts, copies of licenses, copies of certifications, etc.
– Applicantis responsible for submitting all paper application materials directly to the UTHSC Office of Enrollment Servicesin a single packet, unless the supporting document is unavailable (e.g., final transcript not yet available).
Notes: (1) No paper application materials should be sent to the Office of Enrollment Services by anyone other than the applicant (i.e., all paper materials should be sent to Enrollment Services by the applicant).

4. Select Program of Study and Option you are
interested in applying for (e.g., MSN-CNL, MSN– CRNA, DNP–FNP, DNP-Critical & Acute Care, etc.)
Note: This begins the process of obtaining an application account.
The application account will allow you to apply and track your
application materials as they are received by Enrollment Services (e.g., licenses, transcripts,

5. Once you have obtained your application username and password, you will be able to begin completing the online component of the application. It has a number of parts.
Please be sure you go through each one. For example, please provide a complete description of your relevant work experience. You must SAVE your entries in each section before moving on. You can return to prior sections to make changes.

6. By the way … once you have an “online application account,” you may enter – and exit – the online
application multiple times … before actually submitting it. Note: After you have submitted the application, you will not be able to make further modifications to the application. However, you will be able to view your application in its entirety.

7. Your ESSAY must be entered online as a part of the online application process.
Be sure your essay is at least 3 pages.
Note: You may wish to use a word-processing program (e.g., Microsoft WORD) to write your essay, then cut and paste it into the space provided.

8. The online submission of
Recommendation Forms by references is a requirement in the application process.
As an applicant, your task is to be sure the name, phone
number, and email address of each of reference is correct. Once you submit your
application, emails will be sent
immediately to each reference listed asking them to complete a Recommendation Form and return it electronically.

9. Once your application is complete and you are ready to submit your
application fee, click on the PAY button.
You will be shown
instructions on how to pay your application fee online, either by credit card or by electronic check (“ACH”). Note: Online payment of the
Application Fee is considered part of the application process.

10.This part is interesting …
You will be required to enter your credit card (or account) information to ensure
payment. After you accept the payment terms and submit the online payment, you will immediately be logged off! NOTE: YOUR APPLICATION HAS NOT BEEN SUBMITTED! To sign and submit the online portion of your application, you must login again … www.utmem.edu/nursing ... and click the SIGN button.

11. To submit your application – which can only be done after you have paid the application fee online –
you are required to electronically sign the application.
By signing the application, you are certifying the
truthfulness of your responses. “Sign” by typing in your full name, enter the date, and then … click “Submit Application”!

12. After signing and submitting your online application*, the web-based
component of your application will be forwarded to the UTHSC. You have now completed Part 1 of the application process.

* Note: Emails will be sent automatically to each person you listed as a Reference as soon as you press “Submit Application”.
Note: You may follow the receipt progress of your application materials online by logging on to the Online Application for Enrollment using the same Username and Password you used to apply.
PART 2. PAPER-PACKET COMPONENT
1. Transcript Requests –
Request official transcripts – from all colleges and universities you have
attended – be sent directly to you.
Note: The transcripts will arrive in sealed envelopes. Do not open the envelopes!!!
2. License and Certifications – Make photocopies of all your nursing licenses and healthcare-related certifications.
Example: CPR certification, nursing license, etc.

3. Other Paper Supporting Documents – Complete or collect other supporting application materials, as applicable.
Example: Complete “Proposed Plan of Study”, if applying to MSN-CNL program.
4. Checklist – Complete the supporting application materials Checklist.
Be sure every item marked is included in the packet. Also, be sure to explain whyif some materials cannotbe included in packet.

5. Prepare Packet – Collect additional paper-based supporting materials, including Checklist.
Enclose all documents in a large envelope addressed to UTHSC Office of
Enrollment Services.
6. Mail Packet – Ensure
packet has proper amount of postage … and place in U.S. Mail!
