Financial Assistance from the Queensland Government 2014

APPLICATION GUIDELINES

Special Education Program 2014

IMPORTANT INFORMATION

Applications should be submitted when an eligible student is identified / enrolled. Education Adjustment Program (EAP) documentation MUST be provided.

EAP information, forms and a checklist are available on the ISQ website. Log in and go to

OBJECTIVE:

To improve the educational participation and outcomes of young people with disabilities through the provision of assistance targeted at schools.

TARGET GROUP:

Students with verified low incidence disabilities who require significant adjustments to achieve educational outcomes.

ELIGIBILITY:

A student with a disability means a student who is in a non-government school and who has been assessed by a person with relevant qualifications as having intellectual, sensory, physical, social emotional or multiple impairments to a degree that satisfies the criteria for enrolment in special education services or programs provided by the Queensland government. i.e. the student is verified through the Education Adjustment Program.

• A student or child whose only impairment is a specific learning difficulty or for whom remedial education or remedial support is appropriate is not eligible.

USE OF FUNDS:

To meet the objectives of the program funds may be used for:

• salaries of teachers, specialists, professionals, e.g. therapists and support personnel

• consultancy and advisory services

• development of curriculum

• monitoring and evaluation of educational programs

• programs of professional development for staff, parents and community members associated with special education

• purchase of educational equipment and materials

• essential travel and transport costs of students, teachers, specialists, professionals and support personnel

NEW STUDENTS

The application form with EAP verification documentation should be emailed to ISQ on the identification / enrolment of an eligible student.

Full documentation must be received:

1. Before 28 February to access a full year of funding in 2014. 2. Before mid-term to access funding the following term.

The following documentation is required for each new student:

Students with Hearing Impairment (HI), Physical Impairment (PI), Vision Impairment (VI):

• Application form

• EAP2 (Parent / Carer Consent Form)

• Verification form with documentation demonstrating significant adjustments

• Current IEP

• Curriculum Comparison Form (CCF) over 3 curriculum areas

• Support Timetable

• AIMS Printout

Note: Documentation should not be forwarded until the student has been verified as eligible for a profile through Department of Education, Training and Employment (DETE) and an AIMS printout obtained.

Students with Autism Spectrum Disorder (ASD), Intellectual Disability (ID), Social Emotional Disorder (Psychiatric Disorder) (SED), Speech Language Impairment (SLI):

• Application form

• EAP2 (Parent / Carer Consent Form)

• Verification form with documentation demonstrating significant adjustments

• Current IEP

• Curriculum Comparison Form (CCF) over 3 curriculum areas

• Support Timetable

Note: An AIMS printout or verification documentation should be provided for students if they have

been verified in a state or Catholic school.

CONTINUING STUDENTS

To facilitate continuing funding for students supported in 2013:

1. In the last week of January 2014, ISQ will post to schools the ‘Confirmation of Enrolment’ - the listing of students for whom the school received funding support in 2013.

2. Schools make any appropriate changes relating to these continuing students and / or deletions relating to students who are no longer enrolled.

3. Principal confirms accuracy of information.

4. Signed ‘Confirmation of Enrolment’ returned to ISQ by 28 February 2014.

PAYMENT:

Payments are made four times a year, provided that Independent Schools Queensland has received the money from the Queensland Government. A payment advice, with a list of eligible students, is sent to the school prior to the transfer of funds.

The first payment will generally include funding for continuing students only.

Funding for new students confirmed as eligible in Term 1 will be included in Term 2 payment.

EDUCATIONAL ACCOUNTABILITY:

Educational Accountability is met through;

the return of completed student data to ISQ (sent to schools in the first week of October) by Friday 7 November 2014

FINANCIAL ACCOUNTABILITY:

Certification by a qualified accountant is required to show that the financial records of the school have been examined and the opinion is formed that the amounts equal to the amount or sum of the amounts of financial assistance provided and paid for that year has been spent (or committed to be spent) in respect of that year for the purpose or purposes for which the assistance was granted. A qualified accountant is:

A qualified accountant within the meaning of the Corporations Act 2001; or

A person registered (or taken to be registered) as a company auditor under the Corporations Act

2011.

Financial Accountability documents must be lodged with Independent Schools Queensland by:
