DOCUMENTATION FOR QUALIFICATION SYSTEM

1.

Introduction

1.1. The document specifies basic qualification requirements that candidates shall fulfil to become qualified SE suppliers for contracts for “Pressure equipment – maintenance, repair”. The qualification (inclusion into the qualification system) will be successful in case that the candidate fulfilled all requirements defined in the General qualification terms (GQT), as well as requirements defined in this documentation (Documentation). GQT form a part of the Documentation in a form of separate document. In case of differences in the conditions defined in GQT and Documentation, the conditions of the Documentation shall prevail.

1.2. The qualification system for pressure equipment repair and maintenance is kept for 2 categories: Category 1 – Nuclear power plants

Category 2 – Conventional power plants

1.3. Each of these categories are further divided by subject of matter of the activities in subcategories: - Pressure vessels and heat exchangers

- Piping systems

1.4. The candidate will be assigned to the respective category and subcategory depending on the subject of the performance

- By assessment of the submitted authorization and certificates

2.

Description of Subject from Material Group

2.1. Subjects of contracts for execution of which suppliers will qualify:

Maintenance and metalwork (services) at pressure vessels and heat exchangers in nuclear and heat power plants:

o Primary circuit

steam generators, hydro accumulators, pressurizers, spray system coolers, MCP oil coolers, residual heat removal system pump oil coolers, fuel storage pit coolers, pump intermediate circuit coolers, storage tanks, blow-down coolers, steam expanders, ion exchangers, heat exchangers, evaporators

o Secondary circuit

high-pressure heaters, low-pressure heaters, deaerators, steam expanders, motor-driven pump oil coolers, separators – re-heaters, TG oil coolers, intermediate circuit coolers, technological condensers, compressed air accumulators, heat exchangers for DGS, ion exchangers, water heaters, heat exchangers, storage tanks, feed-water tanks

The contract subject execution will cover the following:

a) Preparation for pressure vessel and low-pressure and high-pressure heat exchanger inspections

opening vessel hatches, disassembly of heat exchanger lids cleaning jointing material, flange and lid contact surfaces manufacture of seals for closing vessels and heat exchangers sealing vessels and heat exchangers

b) Repair of pressure vessels and heat exchangers

cleaning pressure vessel internal surfaces, including their accessories repair of contact surfaces by mechanical machining

repair of welds and base material by grinding and welding

c) Removal of defects of welds in pressure vessels and heat exchangers using the following welding technologies used in vertical (PC) and horizontal (PF) positions:

carbon steel of grades 11416.1 and 12 021.1 shielding metal arc welding (111)

shielded argon arc welding using a TIG method (141) stainless austenitic steels of grade 17 248.4

shielded argon arc welding using a TIG method (141)

Pressure vessels and heat exchangers have been defined by Regulation No. 508/2009 Coll. as technical equipment pressure – group A letters “b” as well as in group B letters “a” and “b”.

Maintenance and welding performances and services (production of new welds at replacement or installation of new pipe parts and supports and remedy of defects of existing welds) at the following pipe systems:

o thin-walled pipe systems with diameters DN 10 to DN 1800 from steels of grade 1.1 according to CR ISO 15608

o steam generator feed-water pipe system with diameters DN 25 to DN 250 with the maximum operating pressure 7 MPa from steels of grade 1.1 according to CR ISO 15608

o live steam pipe system with diameters DN 10 to DN 500 with the maximum pressure 4.6 MPa from steels of grade 1.1 according to CR ISO 15608

o thin-walled auxiliary pipe systems of the primary circuit with diameters DN 10 to DN 600 from steels of grade 8 according to CR ISO 15608

o thick-walled pipe systems of the primary circuit with diameters DN 15 to 500 with the maximum operating pressure 12.5 MPa from steels of grade 8 according to CR ISO 15608

o components with heterogeneous welds from steels of grades 1.1+8, 4+8, 5+8, and 6+8 according to CR ISO 15608

o contact surfaces and technological equipment surfaces from steels of grades 1.1, 4, 5, 6, 7, and 8 according to CR ISO 15608

o feed-water pipe system with diameters above DN 100 with the maximum operating pressure 25 MPa from steels of grade 1.1 according to CR ISO 15608

o live steam pipe system with diameters DN 10 to DN 273 with the maximum pressure 13.6 MPa from steels of grade 6 according to CR ISO 15608

o heated steam pipe system with diameters DN 10 to DN 377 with the maximum pressure 3.2 MPa from steels of grade 6 according to CR ISO 15608

At execution of works, one of the following technologies should be used:

shielding metal arc welding (111 according to STN EN 24063)

inert-gas-shielded tungsten arc welding (141 according to STN EN 24063) shielded metal arc welding (131, 135, 136, and 137 according to STN EN 24063)

for steam ducts (141 according to STN EN 24063), filling and cover sheets, shielding metal arc welding (111 according to STN EN 24063)

Piping systems have been defined by Regulation No. 508/2009 Coll. as technical equipment pressure – group A letters “e” and group B letters “e, 1,2”.

2.2. The supplier will perform works by one of the following methods:

2.2.1. In work teams of the client managed by a client’s technical supervisor based on SE technological procedures and technical documentation.

2.2.2. Independently completely based on design documentation submitted to SE and own technological procedures of working and welding procedures.

2.3. In the event that contract is performed in the controlled area of the nuclear power plant, the contract with the qualified successful tenderer will be concluded provided that the following certificates for all qualified workers are submitted:

2.4.1 Certificate of health, mental competence and impeccability in accordance with Act No. 541/2004 Coll. on peaceful use of nuclear energy (Atomic Act) § 26 Sect. 5, .

2.4.2 Document of impeccability – a valid abstract from the criminal record no older than three months for the workers, who will perform work in the controlled area

2.4.3 Document of health competence – a certificate issued by a doctor authorised to issue such confirmations

2.4.4 Document of mental competence – a confirmation of successfully passed psychological tests, issued by the psychological workplace authorised to issue such confirmations . 2.4.5 Certificate of competence in accordance with Act No. 355/2007 Coll. as amended on the

protection, support and development of public health pursuant to §15 Sect.1 Letter c) d) e) in compliance with §45 of the respective act, required..

In the event that the people, who will perform the subject of the qualification system, have valid entry permits issued by SE, for which they proved necessary trainings for work in the NPP issued by VÚJE, impeccability, health and mental competence, it will not be necessary to submit such documents again.

3.

Qualification Requirements

3.1. Basic requirements the candidate must fulfil in order to fulfil the qualification conditions are as follows:

personal status conditions;

economic and financial standing conditions;

technical ability and professional suitability conditions, quality assurance and environmental management conditions

.

Personal status conditions, economic and financial standing conditions, as well as the way of submitting and proving of qualification requirements by the candidates for this qualification system is defined in the GQT.

3.2. Technical ability and professional suitability, quality assurance, environmental management requirements

3.2.1. The candidate fulfils technical ability and professional suitability, quality assurance, environmental management requirements for the contract fulfilment if:

- it has provable experience with implementation of similar projects;

- it has manufacturing facilities, technology, qualified personnel and organisational structure for fulfilment of similar subjects at disposal;

- it has a qualify management system related to the given subject documented and functional, and the quality management system has been validly verified by a successful audit made by SE pursuant to Annex 1 to Regulation of NRA SR No. 56/2006 Coll. The required verification is required only for category Nuclear Power plants;

- it has an occupational health and safety (OH&S) system documented and functional;

- it has an environmental management system documented and functional according

to ISO 14001:2004 or EMAS.

3.2.2. The candidate shall prove its technical and professional competence by submitting the following documents in myHome, depending on assigning to the qualification categories and subcategories pursuant to clause 1.2. and 1.3.:

a. A list of contracts (supplies), at least one, of the same or similar character, performed for last 5 years in the following equipment listed in The Annex 1 for required category and subcategory:

The candidate shall submit information on performed contracts (references) on a form, template of which will be available after registration to the relevant qualified material group in myHome. Execution of each contract shall be confirmed by the customer, if it is not possible, the fact shall be confirmed by the candidate. SE, a. s. shall be entitled to verify the facts stated in the List of Contracts at the customer.

b. A supplier’s certificate to perform activities pursuant to Act no. 124/2006 Z.z. according to § 15. 1 for the equipment listed in Annex 1 to Decree č.508/2099 Coll Part 1 The distribution of technical equipment of pressure

c. A list of workers with their technical qualifications for performed activities that are a subject of this qualification. The List shall contain the workers' names, surnames, qualifications and their legal relations to the supplier. To verify the workers’ qualifications, copies of certificates must be attached in myHome.

i. Certificate pursuant to Regulation No. 508/2009 Coll. according to Article 18(1) the equipment listed in Annex 1 to Decree č.508/2099 Coll Part 1: Distribution of technical equipment of pressure under the name listed in Annex 1 of the required categories and subcategories.

ii. Certificate pursuant to Regulation No. 508/2009 Coll. according to § 17 paragraph 2 for binding loads listed in Annex 1 of the required categories and subcategories

iii. Certificate to the inspector within the meaning of Regulation no. 508/2009 Coll. § 16 set out in Annex 1 of the required categories and subcategories

iv. Certificate for welding documented by a test certificate according to STN TN EN 287-1 + A2:2006 for a respective welding method, group of materials, weld dimensions, a welding material type, and weld joint position and type (TIG 141, MIG 135, 111) listed in Annex 1 of the required categories and subcategories.

v. Diploma of a European welding technologist listed in Annex 1 of the required categories and subcategories.

vi. Certificate pursuant to NV SR 396/2006 Z.z. the minimum safety requirements for construction sites to coordinate safety, listed in Annex 1 of the required categories and subcategories

d. A list of machinery and hardware the candidate has at disposal for execution of the contract subject. The List must contain the equipment name, its description and relation to the candidate (ownership, operative leasing, rental). SE is entitled to verify facts mentioned in the List by an audit made at the candidate or its supplier.

For fulfilment of the qualification conditions, the supplier has to have the following equipment at disposal:

i. general utility tools, tools for work with austenitic steel (a list of torque wrenches and other tools, including their metrological inspection certificates should be submitted);

ii. bevelling equipment to DN 500, welding apparatus for welding by the TIG method;

iii. a transport vehicle for transporting workers and material;

e. A qualify certificate related to the corresponding subject of qualification issued by an independent institution according to requirements of EN ISO 9001:2008 or other document (e.g. Quality Manual) proving that the candidate has a qualify management system implemented according to EN ISO 9001:2008 requirements.

f. A statement of external audit performed by SE for the category nuclear power plants. If the candidate was not audited or the audit certificate validity expired, the candidate will submit a solemn statement that the candidate and its supplier agree with its execution.

The audit purpose is to verify information obtained about the candidate, and to check a quality management system’s functionality by qualified SE personnel. The supplier the quality management system of which was audited by SE will be included in a List of Audited SE Suppliers at a respective level during the audit validity term.

g. An OH&S management system certificate according to OHSAS 18001 standards, or the following documents based on which SE will assess the OH&S management functionality:

i. Company safety policy by which the company proves an obligation to prevent accidents and to establish a safe work environment; ii. Working procedure for performed activity (Safety Plan) containing a

safe working procedure for provision of service/work, and proving a candidate's ability to follow the safety principals and to prevent occurrence of accidents in a workplace;

iii. A list of workers responsible for the OH&S management with specification of their positions in the company's organisational chart, data and certificates related to their technical qualification in OH&S; iv. A list of registered industrial accidents with a sick leave exceeding

30 days and fatal accidents with specification of adopted measures to prevent them for the last 3 years.

h. An environmental management system certificate issued by an independent authority according to EN ISO 14001:2004 requirements, or other document (e.g. by a registration in EMAS) proving that the supplier has an environmental management
