/ Physical Hazards Note:
If asbestos or suspected asbestos is found, stop work and
immediately notify the Kodak Representative.
Presume all painted surfaces contain lead unless otherwise specified.
Burning, grinding of lead
containing materials is prohibited Kodak to designate all contractor chemical/material storage
locations.
Kodak Representative to provide asbestos surveys for demolition and renovation work.
Kodak Representative to complete Appendix A, Chemical/Material/ Biological/Physical Hazards Form and provide to contractor. Kodak Representative to complete Pipeline Checklist and provide to contractor, if applicable.
Contractor to describe in how they will mitigate and/or control hazards identified.
Contractor to provide a list of chemicals/materials to be brought on site to the Kodak Representative for approval.

Kodak Representative to identify methods and frequency of communications with contractor for project HSE information or issues:

Confined Space Note:
Kodak will determine whether an area is to be treated as a confined space.
Kodak HSES will approve permit and conduct initial atmospheric monitoring for Red Permits. HSESs can be contacted for assistance in planning any job. Kodak specifies PPE for Confined Space entry.
Contractor cannot use Kodak Breathing Air systems.
Kodak Representative will obtain permit (red or green) from the operating department.
Kodak Representative to call HSES (22121) 24 hrs prior to the start of job for red permit.
Describe Confined Space Identify Hazard
Kodak Representative must debrief contractor on completion of job, as identified on the permit.
Contractor to identify individuals entering confined space and ensure training is current.
Contractor to wear PPE, as identified on the permit, and ensure employees are trained in its use.
Contractor is required to perform atmospheric monitoring during entry, as identified on the permit, and ensure employees are trained in the use of the LEL O2 meter.
Contractor is required to provide retrieval devices, as defined on the permit, and ensure employees are trained in the operation of the equipment.
Contractor is required to return the permit to Kodak
Representative once the job is complete and participate in debrief.

HSE Issues HSE Requirement for Contractors Coring & Cutting Kodak Representative will obtain the permit.
Contractor to contact Kodak Representative prior to cutting or coring through walls or floors.
Electrical Work Note:
Kodak’s Electrical Methods Group before completion of the project must inspect all new, revised, modified, or relocated electrical installations.
Kodak Representative to establish inspection frequency on projects involving > 40 hours of electrical work:

Prior to coming on Kodak Sites skilled trades, construction labor and their supervisors must have the OSHA 10 hr Construction training.
Contractors must attend Contractor HSE Orientation to receive site access:

Note:
Kodak minimum PPE requirement in manufacturing & construction areas include:

Contractor to specify, provide and train their employees in the proper use of PPE.
Plumbing Note:
Kodak Licensed plumber to inspect all new, repaired, modified or dismantled plumbing systems prior to the completion of work.
Kodak Representative to arrange for plumbing inspections. Contractor will obtain the plumbing permit and supply a master plumber if work is within the City of Rochester.
Contractor to notify Kodak Representative 24 hrs prior to completion to arrange for an inspection by a Kodak Licensed Plumber.
Powered Industrial Vehicles Note:
No propane-powered vehicles are permitted on site without approval from the Kodak Fire Department. All diesel or gasoline vehicles to be equipped with scrubbers if used in or near confined spaces or building air intakes.
Contractor to notify Kodak Representative 24 hrs prior to the use of any powered industrial vehicles.
Railroad Note:
For work within 12 ft of the tracks.
Kodak Representative will obtain the permit.
Contractor to notify Kodak Rep 24 hrs prior to when work is to begin.

HSE Issues HSE Requirement for Contractors Roadblock/Traffic/Pedestrian
Control and Barricades Note:
For full or partial roadblocks.
Kodak Representative will obtain the permit.
Contractor to notify Kodak Rep 24 hrs prior to when work is to begin.
Contractor must provide properly trained and equipped personnel to direct pedestrian and vehicular traffic.
Scaffolds Kodak Representative to notify scaffold department at least 5 working days prior to the beginning of a job that requires a scaffold to be erected.
Contractors must identify a competent person(s) responsible for inspecting the scaffold, scaffold components and ropes on suspended scaffolds before each work shift and after any
occurrence which could affect the structural integrity and authorize prompt corrective action.
Contractor to identify employees who will be working on a scaffold and provide the required user training.
Contractors are required to sign the permit prior to using the scaffold.
Waste Management Note:
Kodak Representative is responsible for proper
characterization, containerization, segregation, storage & disposal of all waste(s) generated by
contractors from Kodak owned chemicals/materials.
Contractor is responsible for proper characterization, containerization, segregation, storage & disposal of all waste(s) generated from chemicals / materials brought on to Kodak sites.

Wastewater Management Kodak Representative to obtain authorization for discharge to sewer or floor drain.
Kodak Representative to complete Appendix B, Waste / Wastewater Management Form and provide to contractor. Contractor to document a plan for containing and managing wastewater not directly discharged to sewer or floor drain.
Working on Elevated Surfaces Kodak Representative to identify competent person to review the contractor fall protection plan.
Kodak Representative to identify anchor points, if permissible.
Contractor to provide the name(s) and contact information of their competent person(s).
Contractor to notify Kodak Representative at least 24 hrs prior to the start of a job to request approval to rig from a Kodak structure.
Contractor to provide a documented fall protection plan and demonstrate training is current.

Appendix A
CHEMICAL/MATERIAL/BIOLOGICAL/PHYSICAL HAZARDS FORM
Kodak Representative to identify chemical/material/biological or physical hazards, locations and know medical or health concerns in the work area (adjacent pipes, vessels, lead, asbestos, process chemicals, lasers, heat/cold, animal droppings) and provide MSDS’s or IH reports upon
request.

Appendix B
WASTE / WASTEWATER MANAGEMENT FORM
All waste must be characterized before it is placed into suitable, labeled containers. The placement of waste containers must be coordinated with the Kodak Representative.
