This directory is for the South Country Health Alliance service area: Brown,

Dodge, Freeborn, Goodhue, Kanabec, Morrison, Sibley, Steele, Todd,

Wabasha, Wadena and Waseca.

SeniorCare Complete and AbilityCare are health plans that contract with

both Medicare and the Minnesota Medical Assistance (Medicaid) program

to provide benefits of both programs to enrollees. Enrollment in either plan

depends on contract renewal.

you can see a particular provider on this list.

If you want to make sure, you should call that

provider to ask whether he or she is still part of

this health plan. You should also ask if he or she

is accepting new patients. This health plan may

not cover all your health care costs. Read your

contract or Evidence of Coverage carefully to find

out what is covered.

American Indians can continue or begin to use Tribal

and Indian Health Services (IHS) clinics. We will not

require prior approval or impose any conditions for

you to get services at these clinics. For enrollees

age 65 years and older, this includes Elderly Waiver

(EW) services accessed through the tribe. If a doctor

or other provider in a Tribal or IHS clinic refers you to

a provider in our network, we will not require you to

see your health plan primary care provider prior to the

referral.

All providers in this directory accept both

Medicaid and Medicare, unless otherwise noted.

This directory provides a list of South Country Health Alliance’s (SCHA) eye care providers available to you as

a South Country member. The eye care providers listed in this directory have agreed to provide you with your

health care coverage. You may go to any of our network providers listed in this directory without referral. If you

have been going to one network provider, you are not required to continue going to that same provider.

The information printed in this directory was correct as of September 2015. A downloadable version of this

document will also be posted on our website at www.mnscha.org under “Programs” and the program you are

enrolled in. Any updates to this directory will be published on the website. You can also call Member Services

at the number inside the front cover to help find a provider. If you are looking for or have questions about a

specific provider, call Member Services. You can request a copy of this directory from Member Services in print

or electronically.

Members get a health plan Member Identification (ID)

Card. This card must be shown whenever you get

health care.
