UnitedHealthcare provides a variety of health plans that help you achieve your goals while educating and engaging employees in making wise health care choices to help control costs. Our UnitedHealthcare portfolio includes a variety of products with a wide range of affordable and flexible health plan solutions. Choose from traditional open access plans, account-based, tiered benefit, limited network and higher cost share plans. We also integrate pharmacy benefits with our medical plans and offer a variety of specialty benefit options so you have the ability to offer your employees a comprehensive benefit package.

In accordance with the Affordable Care Act (ACA), all of our small group plans can also be identified by “metallic” benefit coverage levels: Bronze, Silver, Gold and Platinum. These designations indicate the relative value of the covered benefits, from Bronze (lowest) to Platinum (highest). All plans must meet standard requirements for affordability, essential health benefits and consumer protections. Each metallic level includes choices of benefits, plan types and price.

Level of Member Involvement - LOW

UnitedHealthcare

Choice Plus Choice Plus gives members the freedom to see any doctor and visit any facility in our large nationwide network, including specialists, without a referral and without choosing a primary care doctor. If services are received outside of the network, costs will be higher. Choice Plus is a traditional health plan with copayments, coinsurance, deductibles and out-of-pocket maximums.

UnitedHealthcare Choice

Choice allows members to see any network doctor, including specialists, or visit any network hospital, without needing a referral, however if services are delivered outside of our large national network, the plan will not cover the cost except in an emergency. While a primary care doctor is not mandatory, selecting one is recommended to help manage care. The plan also has copayments, coinsurance, deductibles and out-of-pocket maximums.

Level of Member Involvement - MODERATE

UnitedHealthcare Choice HMO

Choice HMO is a non-gated plan with a strong local network. The plan includes many member tools and resources and is available at a slightly lower monthly member premium. Choice HMO does not require referrals to see specialist. Preventive care is covered 100% in our network – there are no out-of-pocket costs (co-payment, co-insurance or deductible) for preventive care as long as a network doctor is used. Women’s preventive health services are also covered, including well-woman visits, gestational diabetes screenings and more.

Optimum Choice, Inc. (OCI) HMO

OCI is a referral-based plan in which members work closely with a primary care physician for appropriate, cost-effective care. With a strong, local network that includes 33,000 physicians, 200 hospitals, and 3,500 retail pharmacies, OCI offers lower premiums than traditional health plan options. If services are delivered outside of our local network, the plan will not cover the cost except in an emergency. OCI includes 100% network wellness coverage and a wellness program with fitness reimbursement, at-home biometric screenings and telephonic health coaching.

UnitedHealthcare Choice Plus HSA UnitedHealthcare Choice HSA UnitedHealthcare Choice HMO HSA OCI HMO HSA

Available with Choice Plus, Choice, Choice HMO, and OCI HMO, this consumer-driven health plan is made up of two parts: a high deductible health plan and a Health Savings Account (HSA). With these plans, you can open and deposit money, before taxes, into an HSA, which is your own personal savings account. Use the money to pay for eligible medical and pharmacy expenses, including your deductible, or save it for future expenses.

Core Essential Core Essential is an open-access health care plan that features a smaller, tailored network of health care professionals. Members have the freedom to choose a network physician or specialist without visiting a primary care physician (PCP) for a referral. Core Essential has similar plan designs and features as Choice plans, with a network designed to be more affordable. Members must receive care from network providers for benefits to be covered. There is out-of-network coverage for emergency services only.

Navigate Navigate provides well-rounded coverage for many services including: preventive care, primary care physician and specialty office visits; lab, X-ray and diagnostic services; urgent care, emergency services, inpatient hospital services, rehabilitation and skilled nursing facility services; and mental health services. An array of health and wellness resources, such as the Online Health Assessment, Personal Health Record and Online Health Coaching modules, is also included. Navigate offers tailored network coverage for members who obtain a referral from their primary care physician prior to seeing another physician or specialist in the network. No coverage is provided without a referral. Non-network coverage is not available, except in the case of emergency care.

We also offer condition management programs to help members with chronic health issues, such as chronic kidney disease and heart failure enhance self-care, identify warning signs and access resources for assistance, reducing the need for urgent or emergency services. By developing trusted relationships we can reinforce and support physician treatment plans, and help members get the most out of their care encounters. And, all members (not just those in crisis) are able to tap into our

Employee Assistance Program (EAP), 24 hours a day, seven days a week, for

Health Discount Program*

Our health discount program helps employees and their families typically save 10 to 25 percent on a wide range of health- and wellness-related programs, products and services. No driving. No crowded waiting rooms. See a doctor when you need a doctor.

When employees are sick they often miss work. If they are unable to see their own doctor, they may visit an urgent care or emergency department, which can be costly and time-consuming. With virtual visits, employees can see and speak to a doctor 24 hours a day/7 days a week using a mobile device or computer, all from the convenience of their home or office. If needed, a prescription can be sent to their local pharmacy. Virtual visits are integrated into their medical benefits. The virtual visit provider groups we contract with deliver care using live audio and video technology based on quality standards aligned with American Medical Association (AMA) and Federation of State Medical Boards (FSMB) guidelines.
