Thank you for your interest in the nursing programs at Faulkner State

Community College. Read this entire packet carefully as an application

that does not meet all of the requirements is rendered incomplete and

will not be considered for admittance. Admittance is based on a point

system; therefore, it is imperative to pay particular attention to the point

system. There is no waiting list.

To assist you in gaining successful entry into Faulkner State’s nursing

programs; the nursing department offers nursing information sessions.

MOBILITY APPLICANTS ARE RANK ORDERED USING THE SAME POINT SYSTEM AS

FOR ADN APPLICANTS (See Page 6)

Mobility (LPN to ADN) applicants must provide a copy of a valid, unencumbered Alabama LPN

license.

LPN’s will be required to successfully complete NUR 200 (LPN Role Transition to Associate Degree Nurse) prior to integration into the nursing program in order to validate the theoretical knowledge and clinical skill level consistent with students progressing to the third semester of the ADN curriculum. Students who successfully complete this course are awarded 15 non-traditional hours at the completion of the LPN mobility curriculum.

NUR 200 will be waived for LPNs who are admitted to the Mobility program within two years of graduation from the Alabama Community College System Standardized Practical Nursing Curriculum implemented in Fall 2005.

Upon successful completion of the Mobility requirements, the student will follow the regular ADN curriculum outline beginning in the third semester of the program.

NURSING ENTRANCE EXAM

(TEAS)

TEST OF ESSENTIAL ACADEMIC SKILLS

The Test of Essential Academic Skills (TEAS) measures basic essential skills in the academic

content areas of reading, mathematics, science, and English and language usage.

Reading is 58 minutes with 48 questions

Mathematics is 51 minutes with 34 questions

Science is 66 minutes with 54 questions

English and Language usage is 34 minutes with 34 questions

Total time limit for testing is 209 minutes. The TEAS is composed of a total of 150

multiple-choice questions, each with four options. Questions not answered count against the student.

Creating an ATI Account

All individuals preparing to take the TEAS must create an ATI account to register for the test.

Create an account by going to ATI’s website at

www.atitesting.com

and click on “Create a New

Account” then, follow the screen prompts. Click the tab “online store” to order the TEAS exam.

The cost for the TEAS is $56. Payment is made on-line at the time of registration for the test.

All tests are given by the Student Development Office on the Bay Minette Campus in the

Memorial Hall Building (same location as Admissions and Financial Aid) or on the Fairhope

Campus in the administration building. Bring ATI ID and a pencil to test. No calculators are

allowed.

Rescheduling

A notice of 48 hours is required to reschedule a testing date. Failure to notify ATI within 48

hours constitutes forfeit of the registration testing fee.

Retesting

The TEAS test may be repeated once during any semester. The student’s highest score may be

submitted with the nursing application, provided it is within the three-year time frame. The

TEAS score is good for three years.

The Nursing Department requires students to participate in a standardized testing program to assist in evaluating achievement of the content of the courses throughout the curriculum. Included in this testing are examinations to evaluate areas that are essential for academic success:

1. Diagnostic and Achievement Tests help to identify strengths and areas which need remediation in comprehending the content of the courses.

2. Pre-RN/PN mock licensure examinations provide reliable predictors of success for the National Council Licensure Exam (NCLEX ).

4. Specific examinations may be required as a part of progression in select nursing courses. NOTES REGARDING OTHER ENROLLMENT REQUIREMENTS

After acceptance to the Nursing Program, students will be required to provide the following:

• History & Physical Evaluation (Form provided by nursing department)

• Current two-man professional CPR Certification – (Classes offered through nursing department) • Documentation of immunizations – (Form provided by nursing department)

• Proof of having medical insurance – (Student plan available for purchase)

• Drug screen and background check

• Ability to meet all Essential Functions with or without reasonable accommodations • Computer with internet access

HIV or HEPATITIS B REPORTING POLICY

Public law #102-141, Section 633, and “The Alabama Infected Health Care Worker Management Act” require HIV or Hepatitis B infected healthcare workers to notify the State Health Officer within 30 days of the time the diagnosis is confirmed. Physicians caring for HIV or Hepatitis B infected healthcare workers are mandated to notify state health officials within 7 days of the diagnosis. All students are required to comply with this law.

Must meet minimum admission standards for the nursing program.

Must possess a grade of C or better in all nursing program required courses taken at another institution and possess a minimum of a 2.0 cumulative GPA at time of transfer.

3.

Dean/Director of previous nursing program must provide a letter of eligibility for progression in previous nursing program.

4.

Must comply with all program policy requirements at accepting institution.

5.

Complete at least 25% of the nursing program required courses for degree /certificate at the accepting institution.

6.

Must meet acceptability criteria for placement at clinical agencies for clinical experience.

7.

Acceptance of transfer students into nursing programs is limited by the number of faculty and clinical facilities available. Meeting minimal standards does not guarantee acceptance.

8.

Student selection for transfer is based on GPA in nursing program required courses.

NOTE: Students must submit the transfer packet. Transfer requests are due prior to the last 3 weeks of any given semester. Transfer packets may be downloaded from the FSCC website or may be obtained from the FSCC nursing department.

THE ALABAMA COLLEGE SYSTEM NURSING PROGRAMS ESSENTIAL FUNCTIONS

The Alabama College System endorses the Americans' with Disabilities Act. In accordance with College policy, when requested, reasonable accommodations may be provided for individuals with disabilities.

Physical, cognitive, psychomotor, affective and social abilities are required in unique combinations to provide safe and effective nursing care. The applicant/student must be able to meet the essential functions with or without reasonable accommodations throughout the program of learning. Upon admission, an individual who discloses a disability can request reasonable accommodations. Individuals will be asked to provide documentation of the disability in order to assist with the provision of appropriate reasonable accommodations. FSCC will provide reasonable accommodations, but is not required to substantially alter the requirements or nature of the program or provide accommodations that inflict an undue burden on the College. In order to be admitted one must be able to perform all of the essential functions with or without reasonable accommodations. If an individual's health changes during the program of learning, so that the essential functions cannot be met with or without reasonable accommodations, the student will be withdrawn from the nursing program. The nursing faculty reserves the right at any time to require an additional medical examination at the student's expense in order to assist with the evaluation of the student's ability to perform the essential functions.

The Alabama Board of Nursing will require applicants to answer the following questions when applying to take the licensure examination at the conclusion of a nursing program of study. Eligibility to take the examination is the decision of the ABN; graduation from an accredited school such as FSCC does not ensure eligibility.

1. Have you ever been arrested for, been charged with, been convicted of, entered a plea of guilty to, entered a plea of nolo contendre or no contest for, received deferred prosecution or adjudication for, had judgment withheld for, received pretrial diversion for, or pleaded not guilty by reason of insanity or mental defect to any crime other than a minor traffic violation in any state, territory, or country? A crime related to driving while impaired or while under the influence of any substance is not a “minor traffic violation.”

2. In the past five years, have you abused alcohol, drugs (whether legal or illegal, prescribed or unauthorized), and/or other chemical substances or received treatment or been recommended for treatment for dependency to alcohol, drugs (whether legal or illegal, prescribed or unauthorized), and/or other chemical substances?

4. In the past five years, have you had, or do you now have, a physical or mental health problem that

may impair your ability to provide safe nursing care?

5. 6. Is the Board of Nursing or other licensing authority of any state, territory, or country, including but not limited to the Alabama Board of Nursing, currently investigating you?

8. Have you ever been placed on a state and/or federal abuse registry?

A “yes” answer will not necessarily prevent you from eligibility but will require submission of an explanation accompanied by certified documents.
