Certification Admission/ Planning Form (CAP)

Rio Salado College in Partnership with Northcentral University

Undergraduate Teacher Preparation Program

Elementary Education 1-8

This form is required and must be submitted with the Teacher Education Program Application. Students will

benefit from mapping out an education plan prior to enrolling in the Undergraduate teacher education courses.

Rio Salado College advisors or EDU staff members are available to assist as needed. Failure to submit a

completed Certification Admission Plan to the Rio Salado Program Admissions office may impact students’

ability to receive financial aid awards and/or plan for student teaching. Upon submission of this form, students

will be notified within two weeks of ADMISSION into the Rio Salado Teacher Preparation Program. FORMAL

ADMISSION into Level II occurs upon completion of all Level I courses with an overall GPA of 3.0. EDU

Courses taken with a final grade of “D” or ”F” must be retaken regardless of GPA. In addition, students need a

passing score on the AEPA/NES subject knowledge exam, as well as satisfying all other requirements and

documentation outlined in the formal admission application.

ITEMS TO CONSIDER PRIOR TO ENROLLING IN COURSEWORK

Rio Salado College's academic calendar is divided into 16 blocks each semester (fall, spring and

summer). Each block is 16 weeks long. When registering for classes at Rio Salado, you select a start

date for your classes. Your start date will determine your block for that semester. You will take all of

your classes that semester within your block's start and end date. For example, in the Fall Semester,

Block 1 begins in August and ends in December. All of your classes must be completed within that

timeframe. If you complete courses early, you must still wait until your block end date before beginning

the next semester’s classes. Taking your classes block by block provides you with a roadmap to

follow toward the completion of your educational goals.

For flexibility, some courses are offered in an 8-week format. Field experience courses, however, are

scheduled for 14-weeks to allow time to arrange and complete the required field experience

placements. Please note that field experience courses are

not

available during some of the blocks

due to school district availability. In other blocks, field experience courses have limited availability,

meaning field experience hours must be scheduled either at the very beginning or end of your course

due to school district scheduling. Refer to the calendar on page 7 of this document for assistance

building your block schedule around field experience coursework availability.

If you are requesting financial aid, it is important to understand that 12 credits per semester is

considered full-time. Please be sure to speak with a financial aid representative to determine your

funding expectations if you are enrolling in less than 12 credits.

By using this form, you are planning your academic success in the Teacher Education Program. Keep

the original planning form and send a copy to Teacher Education with your Teacher Education

Program Application. Complete pages 2 through 6 and mail, fax or email with your application to Rio

Educator Preparation Program

**Limited Field Experience Availability (LFEA) indicates that in-class field experiences are possible but may be difficult to complete due to individual district/school availability such as testing, spring, and summer breaks. During Limited Field Experience Availability the approval process for field experiences can take longer which, in turn, lessens the total number of weeks available to complete a field experience, i.e. if a field experience confirmation is obtained five weeks into a course a student must complete the field experience hours in the remaining nine weeks of the course. Please keep this in mind before scheduling a class during LFEA as exceptions will not be made.
