This directory is current as of 09/25/2021.

This directory provides a list of your plan's current network providers.

We update our directory each month, based on information we get from providers in your

network. This information can change at any time. So, before you make an appointment or get services, check with the provider to be sure they are in your network.

Also, a provider may not be in our network, even if they are at an in-network hospital. So, it is important to make sure the provider is in our network before you get outpatient services.

This directory is for STATEN ISLAND NEW YORK.

Aetna Medicare is a HMO, PPO plan with a Medicare contract. Our SNPs also have contracts with State Medicaid programs. Enrollment in our plans depends on contract renewal.

This directory provides a list of your plan's network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC).

Your plan covers primary, specialty, and urgent care telehealth visits, including with behavioral health providers. Please check with your provider directly to see if a telehealth visit is available and an option for your care.

If you are enrolled in an Aetna Medicare Plan (PPO), you have the flexibility to choose either network providers to receive covered services or out-of-network providers. Although you are not required to select a Primary Care Provider (PCP), you are encouraged to do so. Covered plan services do not require referrals for specialist care. For some services, your PCP or other provider is required to obtain prior authorization from Aetna Medicare.

Out-of-network providers are under no obligation to treat Aetna Medicare Plan members, except in emergencies. With the exception of emergencies or out-of-area renal dialysis, it may cost more to get care from out-of-network providers. For a decision about whether we will cover an out- of-network service, we encourage you or your provider to ask us for a pre-service organization determination before you receive the service. Please call our Member Services department at the number on your member ID card or 1-833-570-6670 (TTY: 711). You may also refer to your Evidence of Coverage (EOC) for more information, including the cost sharing that applies to out- of-network services.

The network providers listed in this directory have agreed to provide you with your health care, vision, hearing and dental services. You may go to any of our network providers listed in this directory.

In cases where out-of-network providers submit a bill directly to you, you should not pay the bill.

Submit it to Aetna Medicare for processing and determination of your cost-sharing amount, if any.

Our claims/billing address is on the back of your ID card.

Certain network health care services, such as hospitalization or outpatient surgery, require prior authorization from Aetna Medicare. This means the service must be approved by Aetna Medicare before it will be covered under the plan. To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC). When you need services that require prior authorization from a network provider, the provider is responsible for obtaining

Notify your PCP as soon as possible after receiving treatment. Or if a delay would not be harmful to your health, call your PCP.

• If you are admitted to an inpatient facility, you, a family member or friend on your behalf should notify your PCP or Aetna Medicare as soon as possible.

If you are traveling outside of the Aetna Medicare Plan service area, you are covered for

emergency and urgently needed care. Emergency care can always be obtained in or out of the service area from the nearest available provider.

Follow-up care after emergencies

All follow-up care should be coordinated by your PCP. Whether you were treated inside or outside your Aetna Medicare service area, if your plan requires referrals, you must obtain a referral before any follow-up care can be covered. If your plan does not require referrals you should contact our Member Services department at 1-833-570-6670 (TTY: 711), before care is received at out-of-network facilities. Suture removal, cast removal, X-rays and clinic and emergency room revisits are some examples of follow-up care.

For PPO plans, network and out-of-network follow-up care after emergencies will be covered under the terms and conditions of your plan.
