1. The Zoning Data should provide proof of compliance with all of the area and bulk regulations

for each of the proposed lots (Zoning 255-26).

2. The Borough should verify that the parking lot on proposed Lot #2 meets the paved area

setback requirement of 10 feet (Zoning 255-212.C).

3. It appears that 2 residential units are proposed. The plan should address the Pennsylvania DEP

Sewage Planning requirements (SLDO 226-20.B).

4. The Erosion Control Plan should be reviewed and approved by the Cumberland County

Conservation District (SLDO 226-20.K).

5. The plan should indicate a dedication of recreation land or a fee in lieu of such dedication

(SLDO 226-32.B.1.D).

6. Subdivision Notes #8 indicates that there is currently no water or sewer services associated

with these lots. This note should reflect water and sewer service for the proposed residential

units.

7. The numbering in the Subdivision Notes and the General Notes should be verified.

8. Prior to recording, all plans must be delivered to the Cumberland County Planning Department

for signature. At this time, the applicant / engineer should provide a .dwg AutoCAD file that

shows the parcel boundary, lot lines – all lots on the plan, building footprints, road

rights-of-way and the edge of pavement.

LOCATION PLAN

1. The purpose of this plan is to subdivide portions of DABD Limited Partnership property at 120 G Street and 101 G Street add subdivide them into 5 total parcels. See lot calculations on Sheet 3.

2. Frederick, Seibert and Associates has not performed any subsurface investigation. All Utilities included on plan have been plotted according to field survey, site inspection of visible elements and Carlisle Borough records.

3. Topography shown hereon is based on PASDA GIS data.

4. According to FEMA FIRM Panel 42041C0229E, this property is not located in a 100 year floodplain.

5. There are no watercourses, wetlands, rock outcrops or marshes located on this property according to visual site inspection. 6. Survey in PA SPC South NAD 83 performed by Frederick, Seibert and Associates in February 2021.

7. No proposed state road access. Before access to any state road is to be constructed, a Highway Occupancy Permit is required from PennDOT. 8. There is currently no water or sewer services associated with these lots.

7. According to the National Wetland Inventory and on site inspection, there are no wetlands located in the site area.

1. Any damage to adjoining public roads, utilities, etc. during construction will be repaired in kind by the contractor to Carlisle Borough's specifications. 2. No subsurface investigation has been performed by Frederick, Seibert and Associates, Inc. to determine ground water, rock, sinkholes or any other

natural or man-made existing features.

3. FSA, Inc. assumes no liability for the location of any above ground and below ground utilities. Existing utilities are shown from the best available information. Contractor to field verify location and depth of all above and below ground utilities prior to construction. Contractor to contact PA One Call System Inc. at (800) 242-1776

4. The contractor shall locate existing utilities in advance of construction operations in the vicinity of proposed utilities.

5. Utility easements are offered for dedication to their respective Authority upon approval and acceptance of said respective authority.

6. No trees, shrubs, fences, buildings, or improvements are permitted within the 100' clear sight triangle at any street intersection, existing or proposed. 7. No trees, shrubs, fences, buildings, or improvements are permitted within any easements, existing or proposed, as shown on this plan

8. The contractor shall take all necessary precautions to protect the existing utilities and to maintain uninterrupted service. Any damage incurred due to the contractor's operation shall be repaired immediately at the contractor's expense. Contractor to use caution in areas where low hanging wires exist. 9. All utilities shall be cleared by a minimum of 1'-0". All utility poles shall be cleared by a minimum of 2'-0" or tunneled if required.

10. The Contractor shall notify the following utilities or agencies at least five (5) days before starting work shown on these drawings:

11. The contractor shall be responsible for coordination of his construction with the construction of other contractors.

12. The contractor shall notify the Architect/Engineer, before construction, of any conflicts between the plans and actual field conditions.

13. The contractor shall protect all utilities and culvert pipes during construction by insuring proper cover, increasing cover, or constructing roadway and parking through base course before loading site with heavy vehicles.

14. Job site safety is the sole responsibility of the contractor. The Contractor shall perform all excavation in accordance with O.S.H.A. Regulations for trench safety.

15. The contractor shall be aware that in the event of discrepancy between scaled and figured dimensions shown on the plan, the figured dimensions shall govern.

16. It shall be distinctly understood that failure to mention specifically any work which would naturally be required to complete the project shall not relieve the contractor of his responsibility to complete such work.

17. Load bearing fills (Class 1) proposed for support of buildings, walls, and other structures whose function thereof would be especially impaired by the settlement shall be compacted at optimum moisture content to a 95% density.

18. Load bearing fills (Class 2) proposed for support of roadways, pavement, and structures which would not be especially impaired by moderate settlement shall be compacted at optimum moisture content to a 90% density.

19. All fills shall be placed in approximately horizontal layers each layer having a loose thickness of not more than eight (8) inches for class 1 and twelve (12) inches for class 2.

20. A stormwater operation and management agreement must be submitted to and approved by the Borough prior to obtaining a building permit.

22. Improvement security must be posted with the Borough and a developer's agreement entered into with the Borough prior to obtaining a building permit 23. There may be areas on the lot containing rock that may impact utility placement, proposed stormwater facilities to be constructed on the lot and building

locations

24. and there have been no subsurface investigations done as a condition of approval of the plan that created the lot.

STORMWATER MANAGEMENT & UTILITY NOTES:

1. The stormwater plan and details shown are for reference only. The proposed disturbance for these lots is shown as under 1 acre for each lot.

2. The owner of each lot shall sign, execute and record a Stormwater Operations & Maintenance Agreement with the Borough prior to the issuance of a building permit for the lot.

3. Facilities, areas or structures used a stormwater management BMPs shall be enumerated as permanent real estate appurtenances and recorded as a deed restrictions or conservation easements that run with the land.

4. Lot owner shall convey to the Borough stormwater conservation easements to assure access for periodic inspections by the Borough. 5. Roof drains and sump pumps shall discharge to infiltration drywells.

6. Each drywell is designed to be a 4ft. depth.

7. Drywells are designed to manage runoff for each dwelling for the required 2 year storm event.

8. No person shall modify, remove, fill, landscape or alter any SWM BMPs, facilities, areas or structures, without the written approval of the Borough. 9. A blanket stormwater management easement over the lands of DABD Limited Partnership was recorded under instrument number:

10. The sewer and water lateral locations shown on the plan are approximate only. A final utility design shall be performed and approved by the Borough prior to construction of each lot.

11. The future owner of each lot shall be responsible for the installation, operation and maintenance of a gravity sewer system or pressure sewer system, whichever is required, and its connection to the Borough's sewer main in N. Pitt Street.

12. The proposed water laterals are to be copper tubing conforming to requirement of ASTM B-88, Type K. No splices permitted in lengths of less than 100 ft. Water service lateral piping shall be type K copper between the curb stop and the water meter. The property owner is permitted to use a plastic service line on the house side of the water meter

13. The future owner of each lot shall access their property for steep slopes over 15% and, wherever possible, avoid development for the dwellings and driveways on these areas.

14. Frederick, Seibert and Associates has not performed any subsurface investigation. All Utilities included on plan have been plotted according to field survey, site inspection of visible elements and Carlisle Borough records.

15. The proposed 10 ft. sewer easement shown on the plan over the existing sewer system is for the benefit of the dwelling at 120 G Street over the new subidvided lands and the lands of Husler.
