application to the Rochester-Bern Executive MBA pogram. The complete application includes the following documents:
The application form (pp. 3-6)
The letter of motivation (an essay describing your motivation for pursuing an EMBA; details can be found on p. 7)
A sponsorship form to be signed by an authorized representative of your organization (p. 8)
Two recommendation forms (pp. 9-12) Your curriculum vitae
An organization chart showing your position in the organization
Transcripts of academic records from all undergraduate, graduate, and professional schools

Application procedure
You can either print out the forms and submit them handwrit-ten, or you can save the file and then use the PDF typewriter function (tools/typewriter).
Please make sure that you sign the documents where a signature is needed.

We recognize that the Rochester-Bern Executive MBA is an intensive eighteen-month business education that meets every other week on Friday and Saturday, January 2015 through June 2015, and again from August 2015 through June 2016. During the summer of 2015, there is a six-week summer residency in Rochester, New York. In the spring of 2016, there is a study week in Asia. We grant the applicant with work-release time on Fridays to attend classes for the duration of the program, as well as work-release time over the summer to attend classes in Rochester, New York, and in the spring of the second year to attend the seminar in China.
