4.Unsafe condition on the lifting gear (chain sling) the hook latches were missing.

Unsafe lifting operation. Foreman in charge assigned incompetent banks man to sling loads.

Monthly colour code inspection of the sling has not implemented.

Instructed to remove the chain sling from service.

Instructed to have a thorough inspection carries out in all the lifting gear prior to use.

8. Wearing of loose cloths and jewellery’s while using portable tools such as grinder.

Risk of entanglement at drawing in to rotating nip point which may cause cutting of fingers.

Giving training and awareness on how to handle portable tools. Adequate supervision is necessary. Routine inspection to all (PAT) portable power tools and put colour coding sticker as indication of inspection.

Using of adequate PPE such as respiratory (dust), gloves (cotton), hearing defender (ear plug),

goggles (clear).

9. Open trenches alongside the road of workshop.

Risk of falling of person which may result fracture of bone after being fall and risk of vehicular accident.

14. PPE such as hard hat, hand gloves, eye protection are not stored

appropriately in the safe storage room found laying in the ground elsewhere in the site office

Risk of falling from height due to defective fall arrest system issued to worker.

Responsible person must Review records of PPE. Replacement of defective PPE should be done as per company policy.

Recorded equipment daily inspection report by operator prior to operate equipment and workers should report any damages or tampered PPE’s issued by the company or using of their co-worker.

Construct adequate containment wall underneath the generator to avoid soil contamination in the ground.

All workers must wearing their personal protective equipment such as hearing defender (muffs or plug), respirator for dust, fitted jacket or cover all, PFAS or personal fall arrest system, hard hat, gloves, goggles.
