Getting on the path to better health just became a lot easier. ActiveHealth Management, your health management provider, maps the way, providing tools, people and programs to help you better manage your health.

Chief among these is your secure, online Personal Health Record (PHR)—with an

intelligent system at its heart. This powerful new capability can make a real difference in the care you receive and ultimately, your health. That’s why the Medical Plan provides these resources at no cost to you.

Get started on your

Personal Health Record

Do you remember the last time you had a tetanus shot? The date of your most recent

mammogram? How about your children’s immunization records—can you put your hands on them quickly?

Now you can. When you launch your Personal Health Record (PHR), your recent health history will be close at hand. It will be clear, consistent and sequentially organized. No more file folders of loose papers with annoying gaps. You can share your PHR with your spouse or doctor, if you choose, but no one can gain access without authorization from you. And every enrolled member of your family can have his or her own PHR.*

With a PHR, your health history will be:

• Clear • Consistent • Organized • In one place • Easy to access

* Dependent children age 17 and younger do not have direct access to their own records, but parents/guardians can maintain a PHR for each.

Your PHR makes it easy to keep track of doctor visits, immunizations, medications, and other care.

Once you launch it, your PHR does the record-keeping for you. It collects information from Highmark, Express Scripts and Quest Diagnostics. This way, your PHR updates automatically every time you receive services through the Medical Plan.

Second—your Health Assessment

Before you can effectively take charge of your health, you need to take a clear-eyed look at your health status. The ActiveHealth Health Assessment gives you an objective evaluation of your health strengths and any self-reported issues or potential causes for concern.

Your answers to the Health Assessment give you a head start on your PHR, as it fills with information you provide during the assessment, among other sources.

Next—automatic updates to your

health history

When you set up a PHR for you or your child, it automatically fills with health history details for the past three years, pulled from your Highmark and Express Scripts claims. You can enter older health history details yourself—for instance, immunization information from many years past—and you’ll have a complete health history that will update automatically going forward.

Ready, set, go—use your PHR!

Now that it’s set up, your PHR simplifies the task of managing your health and supports your communications with doctors in these ways:

• It lets you create email reminders about preventive care screenings or other exams and tests

• You can easily add items that only you would know, like any over-the-counter medications you take

• You can print out a health summary before traveling or visiting a new doctor

• And more

Remember to check back regularly and add any health information only you would know. Get in the PHR habit starting today!

Be alert to opportunities for

improved care

Do your doctors have the benefit of a complete and accurate health history for you to inform their treatment decisions? Do they have all the answers, every time you see them?

Most doctors would admit that’s impossible, no matter how skilled and attentive they are. Sometimes patients forget to mention key

information. That’s why the ActiveHealth system compares your health information to the latest evidence-based medical knowledge and offers Care Considerations, or health alerts—suggested ways to improve your care.

The ActiveHealth system regularly reviews your medications, doctor visits and laboratory tests and considers any information you provide. If it finds a gap in your care or some other matter to consider, it sends a health alert to you and your doctor suggesting a test or screening, or even a change in your treatment, that might help you. Together, you and your doctor can decide whether your treatment should be changed in any way.

Through ActiveHealth, you continue to have access to a Nurse Line 24 hours a day, 7 days a week. Use it whenever you want to speak with a health professional for information and guidance.

Call the 24-Hour Nurse Line at 866-794-3127 and select option 5.

Get advance approval when it’s

required

Under the Medical Plan, you must get approval before having certain tests or procedures performed—that is, you must get them pre-certified or you may be responsible for their cost. You must also pre-certify non-urgent hospital admissions.

• To pre-certify a test, procedure or non-urgent hospital admission, call ActiveHealth at 866-794-3127.

• To pre-certify mental health and substance abuse treatment, call CIGNA Behavioral Health at 866-640-2772.

Allow at least 7 to 10 days for pre-certification of a non-emergency service. Refer to “Your Healthcare Benefits in Brief” booklet, visit Pensions.org or check the back of your medical ID card for the services that require pre-certification.

Get extra help managing your condition

Living with an ongoing health condition can be difficult. ActiveHealth makes it easier for you through its Informed Care Management program. If you have an ongoing condition—diabetes, for instance—you can work over the phone with a registered nurse who serves as your personal health coach.

Working with a health coach can help you take control of your health. It can show you how to get the most out of your doctor visits. And you can learn what steps you need to take to avoid future complications.

More than 30 conditions are supported, including: • Asthma

• Diabetes

• High blood pressure • Heart disease

• Kidney conditions • Cancer

8:30 a.m.–11 p.m., or Saturdays, 9 a.m.–2 p.m. Eastern Time, and choose “Informed Care Management.” ActiveHealth may also contact you.

With parents still

going strong in their

80s, the Reverend

Douglas Smith, at age 56,

thought he had “plenty of

time” before he had to get

serious about his health. Then

he received an email telling

him to check his private and

secure MyActiveHealth account

for a health alert. There he

found a reminder to make an

appointment for a preventive

checkup, something he had

been putting off doing.

“I finally scheduled it, and that

visit may have saved my life,”

he said. Tests detected prostate

cancer which was still treatable.

Receive extra support

for serious health issues

When a serious illness or health crisis occurs, you need the caring support and assistance of health professionals. ActiveHealth provides a key element of this support through its

Case Management program.

A Case Management nurse is available to help you if you are in the hospital for a long time, are hospitalized frequently or need home healthcare or other specialized services for a long period. Your nurse will work with you, your doctor and the Medical Plan to:

• Coordinate and help arrange medical services for you

• Facilitate approval for all medically necessary services

• Provide education and support for you and your family

In most cases, ActiveHealth will contact you if you qualify for Case Management.
