To avail any of the benefits enumerated, declare that you are a ValuCare member to the accredited provider and/or present your ValuCare Card together with another valid ID with photo.

I. OUT-PATIENT SERVICES
Consultations with ValuCare Coordinators

STEP 1: Present your ValuCare card with another valid ID with photo. STEP 2: Plan Coordinator will refer you to a specialist
STEP 3: If accredited specialist requests for any diagnostic or therapeutic procedure, return to the ValuCare Coordinator to get approval. (Procedures costing more than P1,000 need approval from the Medical Services at the ValuCare head office).
Note: ValuCare Coordinators are NOT available 24/7 at hospitals/clinics. Check their schedules from the hospital/clinic or from the ValuCare Customer Service Hotlines.
II. IN-PATIENT SERVICES

IN-PATIENT Availment Procedures:
STEP 1: Secure admitting orders from ValuCare.
STEP 2: Proceed to the Admitting Section of the ValuCare accredited hospital and present the admitting order and ValuCare card together with another valid ID with photo.
STEP 3: ValuCare head office will be notified of your admission. A ValuCare Liaison Officer will issue the Letter of Authorization (LOA) after proper verifica-tion of your eligibility and coverage.
ValuCare Member may also call & notify ValuCare of Admission STEP 4: UPON DISCHARGE
File PhilHealth together with all other requirements.

Excess in Room and Board and Incremental Rate Charges“Excess in Room and Board” means the actual difference in room rates for transfer to a Room and Board accommodation higher than the planholder's room category.
“Incremental Rate Charges” means the adjustment in charges for hospital ancillary services, professional fees, medicines, operating room and supplies resulting from the transfer by a planholder to a Room and Board Accommodation not included in his enrolled program and higher than his allowable benefit. It shall be computed as follows:
Thirty Percent (30%) shall be charged on top of the Original Charges to a Room and Board accommodation higher than the planholder’s allowable benefit

STANDARD DENTAL BENEFITS
Any number of consultations with an accredited dentist;
Treatment of dental related pain excluding cost of prescribed medicines;
Simple Oral Prophylaxis once a year;
Simple tooth extractions, except surgery for impactions;
Gum treatment excluding the cost of prescribed medicines;
Recementation of jacket crown, inlays and onlays;
Treatment of lesions, wounds and burns;
Temporary fillings;
Annual dental examination;
Adjustment of dentures;
Relief and/or prescription for acute dental pain;
Emergency desensitization of hypersensitive teeth;
Orthodontic consultation; and
Aesthetic dental consultation.Note: Above dental services can be availed with prior appointment with the accredited dentist. Medicines prescribed by the dentist are not covered and for the account of the Planholder.
VI. NON-NETWORK EMERGENCY
Emergency cases in areas without accredited providers and official business trip outside Philippine territory may be reimbursed depending on the amount stipulated in the Healthcare Agreement.

VIII. PRE-EXISTING CONDITION (P.E.C.)
It is an illness or injury or any adverse medical condition that is considered to have been in existence prior to the effective date of the planholder’s coverage in any of the following cases:
When any professional advice or treatment has been obtained for such illness or injury prior to the said effective date of coverage;
Such illness or injury is evident to the planholder before the effective date of his/her membership;
The natural history of such illness or injury can be clinically determined to have started prior to the effective date of coverage.

(f) Eye disorders like cataracts
and glaucoma;
(g) Fatty liver;
(h) Gallbladder diseases like
cholecys titis, cholelithiasis;
(i) Gastric & duodenal ulcers;
(j) Gynecological conditions
(endometriosis, myoma,
ovarian cysts);
(k) Hemorrhoids and anal
fistula;
(l) Hypertension,
atherosclero-sis, dyslipidemia;
(m) Neurological disorders like
Paralysis;
(n) Osteoarthritis, gout,
hyperuricemia;
(o) Peptic Ulcer Disease;
(p) Primary Koch’s Infection/
Tuberculosis;
(q) Prostate diseases like
benign prostatic hypertrophy;
(r) Thyroid disease (Nodular
Nontoxic Goiter,
Hypothy-roidism, Hyperthyroidism);
(s) Varicose veins;
(t) Any dreaded disease, if
present upon enrolment.

Tract Disease requiring bowel
resection and/or anastomosis;
(h) Diabetes Mellitus and its
complications;
(j) Malignancies and Blood
dyscrasias ( Cancer,
Leu-kemias, Idiopathic
Thrombo-cytopenic Purpura);
(k) Complications of an
apparent ordinary illness
including MODS and SIRS (e.g.
sepsis due to pneumonia,
typhoid ileitis, cerebral
malar-ia, etc.);
(l) Single or multiple organ
dysfunction and failure
(MODS and MOF);
(m) Conditions that may
require dialysis;
(n) Chronic pain syndrome
(greater than six weeks);
(o) Any illness other than the
above which would require
Intensive Care Unit
confinement.

COVERAGE OF PRE-EXISTING CONDITIONS
For Individual/Family Account 1st Year—No coverage.After 12 months and upon renewal, said pre-existing illness will be covered based on the following schedules:
2nd Year – 50% of ABL (Aggregate Limit) 3rd Year – 100% of MCB (per illness/year)
However, for kidney & gallstones cases, coverage shall be up to 75% of MCB (Aggregate Limit); unless the case falls under the provisions on exclusions and limitations clause. No coverage for dreaded diseases and conditions except for cases acquired during the effectivity of membership.
4th and succeeding years – 100% of MCB (per illness/year) for all medical cases.
Corporate AccountPEC coverage may vary depending on the Healthcare Agreement.
IX. MAXIMUM COVERAGE BENEFIT (MCB)
ValuCare planholders are protected by a Maximum Coverage Benefit (MCB). The MCB is automatically replenished the following year upon renewal giving the planholder FULL BENEFIT COVERAGE every year.
Coverage is always subject to Pre-existing Conditions provisions, exclusions and limitations, antecedent expenses.
X. GENERAL EXCLUSIONS AND LIMITATIONS
ValuCare does not cover the following as part of the Standard Health Service Agreement. However, any item/s included in a member’s program supersedes the item in question.
Non-Transferability of Benefits
The healthcare benefits under this Agreement are personal to the planholder. Under no circumstance whatsoever shall they be transferred or assigned to another person.

Philhealth Benefits
Philhealth MembersBenefits under Philippine Health Insurance Corporation (PHIC) shall first be deducted before applying any of the benefits due a planholder under this Agreement. Hence, planholders who are members of Philippine Health Insurance Corporation (PHIC) and/or ECC shall, before discharge, submit to the admitting hospital all documents necessary to avail themselves of the benefits under the said medical insurance program. Failure to do so shall make the planholder liable for any unclaimed Philippine Health Insurance Corporation (PHIC) and/or ECC benefit. ValuCare is therefore under no obligation to pay or advance the cost of the benefit under Philhealth.
Non-Philhealth MembersValuCare planholders who are not members or beneficiaries of Phil-health shall be liable to pay the applicable PhilPhil-health portion of the hospital expens-es and profexpens-essional feexpens-es..
Deed of Subrogation
A planholder shall be entitled to the Emergency Care services de-scribed in Annex “B” for injuries sustained in a motor vehicle accident caused by the willful act or negligence of a third party provided that the planholder, or his legal guardian (if the Plan holder is a minor), executes a Deed of Subrogation assigning to ValuCare whatever rights or causes of action that he or his ward may have against the third party or his insurer by reason of the said accident, up to the extent of the value of the services ValuCare has so rendered.
No Liability
ValuCare shall not be liable for any defaults, failures to assist, or delays in the delivery of services herein provided which are due to causes beyond its con-trol, including, but not limited to, acts or any order of government, fires, floods, epidemics, complete or partial destruction of hospital or medical facilities, civil disturbances, strikes and other labor disputes, unduly severe weather, or inci-dents of war. ValuCare shall, however, continue to exert its best efforts to provide healthcare services to the Member insofar as circumstances will allow.

Refusal to Comply with Treatment
ValuCare shall have no obligation to continue providing healthcare coverage to planholders who refuse to comply with any treatment or procedure recommended by a ValuCare accredited physician.
ValuCare is also under no obligation to continue providing healthcare coverage to planholders who refuse to be discharged from confinement after his/her attending ValuCare physician has recommended the discharge.
Hospital Policy
Hospital care services shall be subject to the rules and regulations of the Accredited Hospital.
XI. STANDARD EXCLUSIONS AND LIMITATIONS
No healthcare benefits shall be paid for the following:
a) Service from Non-ValuCare accredited physicians and / or non-ValuCare accredited Hospitals and other health / medical providers, including adverse medical conditions arising from the same except those stipulated under the provisions on emergency care services;
b) Convalescent Care or Rehabilitation Care - A care to help restore a person’s ability to function as normal as possible after a disabling illness or injury which do not require confinement to a hospital/institution;
Custodial or Intermediary Care - Non-medical care furnished primarily to provide room and board but skilled nursing care is not;
Domicilliary Care - A level of care for persons requiring minimal medical attention that is provided in a planhololder’s residence;
Confinements which are for purely diagnostic purposes except as specified in this Agreement;

c) Diagnostic work-ups, treatment and operations, to treat congenital deformities and abnormalities (e.g., herniorrhaphy to treat indirect inguinal hernia); d.) Diagnostic examinations and treatments for cosmetic purposes, and / or treatment
of illnesses resulting from cosmetic procedures, except reconstruc-tive surgery necessary to treat a functional defect resulting from an accident injury;
e) Weight reduction programs, surgical operation or procedure for treatment of obesity, including, but not limited to, gastric stapling;
f) Maternity care and other conditions resulting from and related to pregnancy; g) Physical Examinations and Psychological testing necessary for employment, school,
insurance or licensing purposes;
h) Experimental medical procedures or alternative medicines, such as but not limited to acupuncture, acupressure reflexology and chiropractic's;
i) Services to diagnose, work up, treat and / or reverse infertility or fertility, virility / potency (erectile dysfunction);
j) Organ transplantation and related laboratory examination, and its complications; k) Purchase or lease of medical equipment, oxygen and oxygen dispensing equipment
except during covered in-patient care;
l) Corrective appliances, artificial aids, surgically implanted internal and external prosthetic devices;
m) Speech Therapy for congenital and developmental disorders;
n) Psychiatric and / or psychological illnesses and conditions including neurotic and psychotic behavior disorders; Anxiety disorders; Hyperventilation Syndrome o) Prescribed out-patient or take home medicines;

p) Treatment of injuries or illness resulting from war, or any combat related activities while in Military service; injuries resulting from riots, strikes, and other civil disturbances; injuries or illnesses resulting from attempted suicide or self-destruction, regardless of whether planholder was sane or insane; diagnostic examinations & treatment of infectious disease declared as epidemic by the Department of Health of the Philippines;
q) Treatment of illnesses and injuries attributable to the Planholder's own miscon-duct, domestic violence, assault, gross negligence, alcoholic intoxication, illegal drug use, intemperate or chronic use of drugs or alcoholic liquor (e.g., hepatic cirrhosis secondary to alcoholism), sexually transmitted diseases like AIDS, gonorrhea and syphilis, condylomata, herpes and their attendant complications, participation in the commission of a crime whether consummated or not, violation of a law or ordinance, and unnecessary exposure to any imminent danger or hazard to health;
r) Dental cases and illnesses, including its complications; oral surgery for purposes of beautification;
s) Treatment of injuries or illnesses caused by directly or indirectly by participation in any hazardous sport or activity which includes, but is not limited to, BASE jumping, bunjee jumping, gliding, hang gliding, high wire, sky jumping, skydiving, sky surfing, ballooning and sky flying; land sports including martial arts (judo, karate, taekwondo), gymnastics, boxing, wrestling, horseback riding, polo, paintball, mountain or indoor climbing, rock climbing, motor sports (car racing, motorbike racing), adventure racing, aggressive inline skating, BMX, extreme motocross, extreme skiing, freestyle skiing, land and ice yachting, mountain biking, mountain boarding, outdoor climbing, caving or spelunking, sand boarding, skateboarding, speed biking, speed skiing, scootering and street luge; water sports including barefoot water skiing, cliff diving, free-diving, jet skiing, open water swimming, powerboat racing, round the world yacht racing, scuba diving, snorkeling, speed sailing, surfing, wakeboarding, whitewater kayaking, windsurfing; among others.

t) Treatment of injuries sustained in a motor vehicle accident where planholder is not at fault, if the planholder or his guardian fails or refuses to execute the Deed of Subrogation specified in Article 6.3;
u) Corrective eye surgery for error of refraction such as myopia, astigmatism and hyperopia;
v) Allergy testing, desensitization treatment and related materials w) Professional fees of medico-legal officers;
x) Congenital and genetic disorders, developmental disorders, sleep and eating disorders; and metabolic conditions like but not limited to Crigler-Naijar syn-drome, cystinosis, diabetes insipidus, Fabry, fatty acid metabolism disorders, galactosemia, Gaucher glucose 6-phosphate dehydrogenase (G6PD), Hurler-Scheie, Hunter hypophosphatemia, lactic acidosis, mucopolysaccharidosis, Nie-mann-Pick, organic acidemias, purine phenylketonuria (PKU), Pompe, porphyria, Pseudo-Hurler, pyruvate dehydrogenase deficiency.
y) job-related illnesses and injuries. of workers obtained in any of the following hazardous workplaces:
(i) where the nature of work exposes the workers to dangerous environ-mental elements, contaminants or work conditions including ionizing radiation, chemicals, fire, flammable substances, noxious components and the like;
(ii) where the workers are engaged in construction work, logging, fire-fighting, mining, quarrying, blasting, stevedoring, dock work, deep-sea fishing and mechanized farming;
(ii) where the workers are engaged in the manufacture or handling of explosives and other pyrotechnic products;
(iv) where the works use are exposed to power driven or explosive powder actuated tools; and
(v) where the workers are exposed to biologic agents such as bacteria, fungi, viruses, protozoas, nematodes and other parasites.

cc) Gamma globulins, immunoglobulin (e.g. rabies immunoglobulin) interfere on and hormonal therapy;
dd) Screening tests;
ee) Vaccines (except anti-tetanus, anti-rabies, and anti-venom, if with coverage); ff) Hyperalimentation (parenteral nutrition);
gg) Molecular genetic/chromosomal diagnostic examinations.
