This report is composed annually by the Data and Systems Group (DSG) of the Centers
for Medicare & Medicaid Services (CMS). The Enrollment Report profiles Medicaid
managed care programs on a plan-specific basis as of July 1, 2011. County level data
are not collected. The information was collected from State Medicaid agencies and
CMS Regional Offices. Data are presented for all States, the District of Columbia,
Puerto Rico, and the Virgin Islands. Data are not presented for the following territories:
Guam, Northern Mariana Islands, and American Somoa. Special Thanks to Loan
