Film Insurance Guidelines

Filmmakers seeking permits to film on-location in any of the areas served by FilmL.A. must provide proof of insurance meeting both our requirements and those of each jurisdiction involved. The

insurance step is very important to the permit coordination process; failure to submit proof of insurance can delay permit application processing.

Please note that mandatory coverage limits are activity-based. We do not advise you to purchase insurance without calling us at 213.977.8600 and discussing your plans with a FilmL.A. Insurance Specialist.

Requirements by Jurisdiction

FilmL.A., Inc.

• One Commercial General Liability or Comprehensive Form naming "FilmL.A., Inc.. and its officers, agents and employees” as "Additional Insureds".

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Minimum coverage limits are as follows:

$ 500,000 for still photography only

$ 1,000,000 for motion capture (filming)

$ 5,000,000 for aircraft use

• Submit certificates to FilmL.A. by email at insurance@filmlainc.com or by fax at (213) 977-8601. Please attach a cover sheet including any additional information you wish to share with our insurance specialists. If applicable, your policy will be kept on file for your future use.

County of Los Angeles

• One ACORD Certificate of Insurance naming both the certificate holder and the "County of Los Angeles" as "Additional Insured." (This may be on same certificate as FilmL.A.).

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Minimum coverage limits are as follows:

$ 500,000 for still photography only $ 1,000,000 for motion capture (filming)

• Submit certificates to FilmL.A. by email at insurance@filmlainc.com or by fax at (213) 977-8601. Please attach a cover sheet including any additional information you wish to share with our insurance specialists. If applicable, your policy will be kept on file for your future use.

City of Diamond Bar

• One Commercial General Liability or Comprehensive Form naming "The City of Diamond Bar” as an "Additional Insured".

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Minimum coverage limits are as follows:

$ 500,000 for still photography only $ 1,000,000 for motion capture (filming)

City of Industry

• One Commercial General Liability or Comprehensive Form naming "The City of Industry” as an "Additional Insured".

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Minimum coverage limits are as follows:

$ 500,000 for still photography only

$ 1,000,000 for motion capture (filming)

City of Lancaster

• One ACORD 25 Certificate of Liability Insurance. Certificate holder = "City of Lancaster, its officers, servants, agents, employees, and volunteers."

• "Additional Insured" endorsement = "City of Lancaster, its officers, servants, agents, employees and volunteers"

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Liability coverage: premises/operations, products/completed operations (when applicable), contractual, personal injury and broad form property damage, with limits of:

$ 1,000,000 for motion capture (filming) $ 5,000,000 for aircraft use

• Copy of Workers Compensation Policy if not evidenced on insurance certificate. Must meet liability limits as required by the Labor Code of the State of California.

• City of Lancaster Hold Harmless Agreement signed by an authorized company representative. • When using private property, a private property authorization form must be completed and signed

by the property owner and an authorized company representative.

City of Los Angeles

• Filmmakers seeking permits to film on-location in the City of Los Angeles must comply with the requirements of the City's Risk Management Office, which can be accessed online at the following website: http://www.lacity.org/cao/risk/

• Completed insurance forms meeting the City's criteria should be submitted directly to the City of Los Angeles Risk Management Division for approval. Documents can be sent via email to

cao.insurance.bonds@lacity.org or by fax to (213) 978-7615.

• Direct all questions, information and approvals regarding insurance for filming in the L.A. Harbor to The Port of Los Angeles Risk Management Division, attention Marie Gutierrez, either via email mgutierrez@portla.org, phone at (310) 732-3857 or fax (310) 833-8230.

• Minimum coverage limits are as follows:

$ 500,000 for still photography only $ 1,000,000 for motion capture (filming)

$ 2,000,000 with Department of Water and Power involvement $ 3,000,000 for water areas under Harbor Department jurisdiction

City of Palmdale

• One insurance certificate stating coverage for broad form property damage, personal injury, automobile, employers' and comprehensive liability insurance, naming the following as “Additional Insureds”:

"The City of Palmdale, the Palmdale Community Redevelopment Agency, the Palmdale Civic Authority, the Palmdale Housing Authority, and the Palmdale Industrial Development Authority, their employees, officers, agents, and elective and appointed boards."

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Insurance policy must stipulate that such production company insurance will operate as primary insurance and that no other insurance maintained by the City of Palmdale will be called upon to cover any loss.

• Total coverage must include a minimum limit of one million dollar ($1,000,000) on a per-occurrence basis, increasing to five million dollars ($5,000,000) if aircraft are used.

• Copy of Workers Compensation Policy if not evidenced on insurance certificate. Must meet liability limits as required by the Labor Code of the State of California.

• When using private property, a private property authorization form must be completed and signed by the property owner and an authorized company representative.

City of South Gate

• One Commercial General Liability or Comprehensive Form naming "The City of South Gate” as an "Additional Insured".

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Minimum coverage limits are as follows:

$ 500,000 for still photography only

$ 1,000,000 for motion capture (filming)

City of West Hollywood

• The City of West Hollywood operates its own film permitting office. Nonetheless, a special

arrangement between this office and FilmL.A. allows West Hollywood locations to appear alongside others on a FilmL.A.-issued permit. If FilmL.A. is coordinating the permit, the following insurance needs must be met.

• One ACORD Certificate of General Liability or Comprehensive Insurance naming “the City of West Hollywood, its officers, agents and employees” as "Additional Insured."

• Certificate must include a thirty-day notice of cancellation provision (10 days for non-payment of the certificate premium). It must be obtained from an insurer admitted to do business in California or written through a California-licensed broker.

• Minimum coverage limits are as follows:

$ 500,000 for still photography only $ 1,000,000 for motion capture (filming)
