Students must complete an online application and submit the application with all
supporting information by 5:30 p.m. on October 2, 2015.
Thank you for your interest in becoming a REACH Memphis student. Our participants experience exciting journeys that ignite bright futures, develop friendships and create successful leaders.
A completed application contains the following:
• Online application and essay
• Online Parental Acknowledgement Form • A current photograph (jpeg format)
• Letter of recommendation from one of your teachers (current or last year) • Counselor recommendation form
• Current high school transcript • Parents’ 2014 W-2 and 1040 forms
To apply to REACH Memphis, follow the steps below:
STUDENTS – You are solely responsible for making sure that your teacher, guidance counselor and parents have submitted their necessary information.
Teacher
• Deliver a Teacher Recommendation Form to your teacher.
• Check with your teacher to make sure the recommendation letter is submitted before 5:30 p.m.
on October 2, 2015. Guidance Counselor
• Deliver the Counselor Recommendation Form to your guidance counselor.
• Confirm that the completed Counselor Recommendation Form, along with a copy of your
transcript, has been uploaded to the REACH Memphis website before 5:30 p.m. on October 2, 2015.
Parent(s)
• Ask your parents to complete the online Parental Acknowledgement Form. • Instruct your parents to submit copies of their 2014 W-2 and 1040 forms.
• Confirm that your parents have provided the information to the REACH Memphis staff by October 2, 2015.
Parents may submit their forms by:

Use this checklist to navigate your way through the REACH Memphis college application process. Place a
checkmark by completed application components to ensure you’ve finished each section. Don’t forget to check in with your parents, teachers and guidance counselors about their tasks. Remember, the deadline for
applying to REACH Memphis is 5:30 p.m. on October 2, 2015. ONLINE APPLICATION AND ESSAY.
Visit reachmem.com, and click on the Student Application tab to complete the online portion of your application packet.
ONLINE PARENTAL ACKNOWLEDGEMENT FORM
Visit reachmem.com, and click on the Student Application tab, and ask one of your parents/guardians to complete the parental acknowledgement form.
A CURRENT HEAD SHOT (PHOTO)
Visit reachmem.com, and click on the Student Application tab. Find the “Student Photo” button, and follow the upload instructions. Remember, we only accept .jpg files.
LETTER OF RECOMMENDATION FROM ONE OF YOUR TEACHERS
Ask one of your teachers to submit a recommendation letter on your behalf by giving them the Teacher Recommendation Request Form on Page 9 of this packet.
COUNSELOR RECOMMENDATION FORM
Ask your guidance counselor to complete the online recommendation section by giving them the Guidance Counselor Recommendation Request Form on Page 8 of this packet.
CURRENT HIGH SCHOOL TRANSCRIPT
Ask your guidance counselor to upload a current copy of your high school transcript by giving them the Guidance Counselor Transcript Request Form on Page 8 of this packet.
PARENTS’ 2014 W-2 AND 1040 FORMS
Instruct your parents to submit copies of their 2014 W-2 and 1040 forms to REACH Memphis by Oct. 2. They may submit forms by: 1) sending an email to [email protected], 2) faxing the forms to the REACH Memphis office at 901.682.0866 or 3) delivering the forms to the REACH Memphis office at 4646 Poplar Ave, Suite 327.

Once you begin the online application, you cannot save and return to it. You must complete the online application in one sitting. Use the TAB key or the mouse to navigate between sections. Do not press the ENTER key whennavigating from field-to-field because it will end the application process.
STUDENT PROCESS
1. Launch reachmem.com and click the Student Application tab in the top menu. 2. Click the Student Application button and complete each field.
3. Cut and paste 350 to 500 word essay into the essay template. Select one of the following topics:
• Who is the one person that has been a positive influence on you, and why? • Tell us about a significant event in your life and how it has impacted you. • What subject do you enjoy the most in school and why?
• What do you enjoy doing when you are not in school?
4. Review and ensure that your application and essay are complete. 5. Click the Submit button.
6. Click the Photograph button.
7. Save your image as firstname_lastname.jpg and upload your photo.
8. Enter your email address, first and last name and high school affiliation below the upload section. 9. Click Submit. When the web page refreshes, you will see the following text:

PARENT PROCESS
1. Launch reachmem.com and click the Student Application tab in the top menu. 2. Click on the Parental Acknowledgement button.
3. Complete the online Parental Acknowledgement button. 4. Review and click the Submit button.
TEACHER PROCESS
5. Launch reachmem.com and click the Student Application tab in the top menu. 6. Click on the Recommendation Letters button.
7. Complete personal information and insert letter content. Please type your student’s name in the upper right hand corner of the letter.
8. Review and click the Submit button.
GUIDANCE COUNSELOR PROCESS
1. Launch the REACH Memphis website: reachmem.com and click the Student Application tab. 2. Click on the Counselor Recommendation and Student Transcript button.
3. Complete the Counselor Recommendation form.
4. Upload the student’s transcript into the transcript template. 5. Review and click the Submit button.
