About this resource
This resource includes both a flyer and a limited directory of mental health services in New
Hampshire to support pediatric primary care providers in offering successful referral options for
their patients who may need mental health services.
The directory includes information such as practice name, address, phone number, website,
towns served, Medicaid acceptance, and programs or types of services. The directory does not
list the many individual private mental health providers practicing in New Hampshire due to the
changing nature of practices, providers, and insurance. However, additional providers may be
included in future versions.
Note about printing
The cover page and next four pages should be printed on standard 8.5x11 size paper, however, the rest of the Directory should be printed on legal size paper for the best results.

Family Resource Centers
Family Resource Centers provide comprehensive services to parents and their children, from
pregnancy through age 18. In most cases, services include parenting classes, parent-child
groups, early leaning centers, play groups, assistance with tax preparation, information and
referral, after school assistance, and other programs to meet specific community needs. Family
resource centers are open to the public and can be especially supportive of families struggling
with challenging issues, limited financial resources, and/or troubled family dynamics.

Student Assistance Program
SAP counselors served more than 10,000 NH kids in 2016, helping youth with various social
issues, such as bullying, dating violence, and peer pressure. The program has grown from five
schools in 2013 to more than 40 NH middle/high schools today.

Improve communication between physical and mental health providers to increase access for patients.
Care coordination is enhanced when everyone connects on a regular basis.
Join your patient in treatment planning; their ability to incorporate the plan into their lives will increase the chance of positive
outcomes.
Your patient's goals may not align with the goals of the medical provider, but creating shared goals will help them experience
success and continue with the treatment process.
Assess the needs of your patient population, then find local mental health professionals who can meet these needs.
If resources are scarce, use remote or telehealth options.
Evidence-based screening tools identify patients who need treatment.
A deeper assessment identifies what needs the patient/families may have to ensure that you are making appropriate referrals.

To determine whether your referral was a success, develop a process flow with clear steps to establish a referral to a specialist
in a timely manner. Include supporting information for the specialist so their response will address the reason for the referral.
Ensure that the PCP, specialist, and patient/family are satisfied, and communicate with all parties in a timely manner.
Maximize the process with EHR support.
When possible, develop a mutual Release of Information form at both sides of the referral. Lack of consent to share PHI inhibits
the ability to provide quality care.
Active communication among the team of providers maximizes patient outcomes.
