• Agent National Producer Number (NPN). Click here for help finding your NPN. • Certificate of Insurance for Errors & Omissions (carrier, policy and expiration date) • Federally Facilitated Marketplace (FFM) Certification and obtain Certificate of
Completion. Click here for help finding your Certificate.
• AZ License Number. Click here for help finding your License Number.
• If you are assigning commissions to an agency you will need the Agency Tax
Identification Number (TIN). If you are an independent Broker, you can enter your TIN or your Social Security Number

Submit Request to Become a Broker
Go to the Meritus website,
www.meritusaz.com
and select Brokers > Sign
Up to be a Broker
Submit Request to Become a Broker
The link will take you to an online enrollment form. Items with a red
asterix are required fields. Fill in all required fields and select “Create
Account”
Enter your personal NPN number. If you do not know it, click on the link below to use the search functions
Enter your AZ Insurance License number, if you do not know it, click on the link below to use the search functions
To avoid delays to your registration please upload all requested
documents

Meritus Training

Meritus will hold training sessions. Information for date and time will be
forthcoming
Receive Email with Links to Setup Access
3-5 business days after you have attended and completed the Meritus Broker
training, you will receive an email with several links to sign up for our the
Meritus Broker tools.
Link 1: Setup your password for access to our Customer Relationship Management system (CRM).
Link 2: Setup your access to our Online Sales Portal.

Link 1 provided in the email you receive is for the CRM tool. CRM is used to
manage leads and contact information.

Link 2 - Setup Access to Online Sales Portal
From the Link 2 provided in the email, you will be taken a log in screen for the Online Sales Portal. Select Register as new Agent or Assistant below the login button.

Once You Have Completed Training to Become a Broker…
Approximately 3-5 business days following your completion of the Meritus Broker Training, you will receive an email with a variety of documents requiring your online signature. From that email select View Documents.
Receive Contracting Email
Check the “I agree to do business electronically with Meritus” and select
Review Documents

Complete Contracting
Complete all of the documents. Make sure to sign all documents requiring your signature. Review each page and input the requested data in any boxes highlighted in red. All red boxes and requested documents and must be completed in order to finalize this process.
Each page can be viewed by clicking the next button on the left hand side or selecting each page on the right hand side.
Complete Contracting
Once all fields have been successfully completed a note will appear on the left side of screen. Click the yellow button “Confirm Signing”
A pop up box will appear where you will have the opportunity to download or print the completed application. Once you are finished click the X button on the top right and you will be signed out.

Complete Contracting
The signed documents are sent back to Meritus for Meritus signatures and approvals. Once this is complete you will receive an email with signed copies of all the documents.
