Introduction
Regardless of age, gender, race, ethnicity, sexual orientation, gender identity or socio‐economic
circumstance, HIV infection is a serious health issue that directly or indirectly affects all members of our communities. The purpose of this manual is to provide information on the benefits and services available for people living with HIV/AIDS, their loved ones, and the professionals who work in the medical and social services in Santa Barbara County.
People with HIV can take control of their health and life by learning about and taking advantage of the many resources that are available. The Santa Barbara County Public Health Department and
collaborating agencies offer a full spectrum of services such as medical care, individual and group support, benefits counseling, advocacy, health education, HIV testing, housing assistance, food programs, dental care, and mental health and substance abuse services.
HIV/AIDS Information
HIV (Human Immunodeficiency Virus) infection develops in three stages: Asymptomatic, symptomatic and progression from HIV to AIDS (Acquired Immune Deficiency Syndrome). The majority of people infected with HIV take between 2 and 12 years to go through the stages. However, everyone responds differently to HIV infection.
Stage 1: Asymptomatic
The first stage of HIV is called, “asymptomatic” (which means without signs or symptoms of illness). People in this stage show no signs of illness and tend to feel fine. Educating yourself about what can be done to protect and maintain your health during this stage is especially important.
Stage 2: Symptomatic
The second stage of HIV is characterized by the presence of symptoms. During this stage, a person infected with HIV begins to develop various symptoms. This stage can last for a short time, or extend over a period of years.
The most common symptoms are:
Persistent fatigue without explanation Unexplained fever, night sweats, and chills Loss of weight (i.e., 10% of normal body weight)
Swollen glands (lymph nodes in the neck or under the arms) White spots or sores in the mouth
Chronic diarrhea Persistent cough
Vaginal infections and pelvic inflammation Skin rashes
Stage 3: AIDS
The third stage of HIV is called AIDS. During this stage the person experiences failure of the immune system that can contribute to opportunistic infections or illnesses that may be life threatening. However, with proper care, many people diagnosed with AIDS can recover their health, and live long productive lives.
Health Insurance
This section provides basic information about special health insurance that may be available to people with HIV/AIDS depending on specific eligibility requirements.
COBRA Extended Health Coverage
The Consolidated Omnibus Budget Reconciliation Act (COBRA) allows for the temporary extension of employer‐provided group health insurance to employees and their families under certain circumstances such as voluntary or involuntary job loss, a reduction in hours worked, or other life events such as death or divorce. Qualified individuals are required to pay the entire premium for coverage plus a 2 percent administrative fee.
Information on how to file for COBRA extended health coverage is provided by the former employer in the employee’s separation paperwork. By law, these rules must clearly explain how to file for COBRA benefits and procedures for processing claims. The employer’s plan administrator is the point of contact for any questions or concerns regarding COBRA extended health coverage.

Cal‐COBRA Extended Health Coverage
Cal‐COBRA is California’s version of federal COBRA and was designed to provide continuation of
coverage for federal COBRA beneficiaries. Once federal COBRA benefits are exhausted, Cal‐COBRA may extend continuation coverage up to a combined total of 36 months of coverage for California enrollees. Like federal COBRA, you will be responsible for the entire premium plus up to an addition 10 percent for administrative costs. Unlike federal COBRA, Cal‐COBRA applies to group health plans sponsored by employers with 2 to 19 employees.
The insurer, not the employer, is responsible for administering the extended Cal‐COBRA coverage. If you are switching from COBRA to Cal‐COBRA, the health plan must send you a notice explaining the change in premiums and what you need to do to keep your coverage. If you don’t get a notice, and think that you should have, contact the health plan.
Office of AIDS/Health Insurance Premium Payment Program (OA/HIPP)
OA/HIPP, formerly known as the Comprehensive AIDS Resources Emergency/Health Insurance Premium Payment (CARE/HIPP) Program, is a subsidy program that pays health insurance premiums for eligible California residents with an HIV/AIDS diagnosis. The program is available to individuals with health insurance that are at risk of losing it as well as to individuals currently without health insurance and who would like to purchase it.
Individuals are eligible for OA‐HIPP if they: Have an HIV/AIDS diagnosis Are a California resident Are 18 years of age or older
An adjusted gross income of no more than $50,000
NOT enrolled in Medicare, Medi‐cal, or the Low Income Health Program
Have (or plan to get) a comprehensive health insurance plan with prescription drug benefits
This program is not available to individuals whose insurance premiums are all or partially paid for by their employer. OA‐HIPP clients can remain on the program as long as the services are needed and they continue to meet all the program requirements. Once approved and enrolled in the program, each OA‐ HIPP client will be required to re‐enroll annually.
California Major Risk Medical Insurance Program
MRMIP is a state funded program of the Managed Risk Medical Insurance Board (MRMIB) that offers health insurance benefits to California residents who are unable to purchase health insurance coverage in the individual insurance market due to a preexisting condition. Although a private health insurer cannot deny health coverage solely because an applicant is HIV positive, if an applicant has been treated for AIDS a health insurer can deny coverage based on a pre existing medical condition.
Californians who qualify for MRMIP benefits participate in the cost of their coverage by paying a monthly premium, an annual‐deductible, and copayments. The monthly premium costs are based on your age, where you live in California, and the number of dependents that are enrolled in MRMIP. There are a wide variety of services covered by MRMIP; including preventive care, hospital care, physician office visits, prescription drugs, laboratory and x‐ray services, and home health/hospice care.
MRMIP does not require you be a U.S. Citizen or U.S. National to apply or qualify for coverage. Presenting your Social Security Number when applying is optional. Because of funding limitations, MRMIP does sometimes have a waiting list. For more information on availability and eligibility

Medi‐Cal
Medic‐Cal, California’s version of the federal Medicaid health care program, is designed to help low income individuals who meet certain eligibility requirements receive the medical services they need. Some services available through Medi‐Cal are:
Office visits to doctors and dentists Hospital and nursing home care Laboratory and x‐ray services Mental Health services
Selected prescriptions and medications Medical supplies ordered by a physician Chiropractic services
If you are enrolled in Cal‐Works/Cal‐Win or receive a Supplemental Security Income/State
Supplementary Payment (SSI/SSP) cash grant, Refugee Cash Assistance, or In‐Home Supportive Services, you are automatically eligible for Medi‐Cal coverage.
If you have limited resources but are not eligible for the above programs, you may still be eligible to receive Medi‐Cal if you meet one of the following requirements:
Age 65 or older or younger than 21 Blind
Disabled Pregnant A refugee
The vast majority of people with an HIV/AIDS diagnosis who apply for Medi‐Cal coverage claim disability. Santa Barbara County Department of Social Services manages most Medi‐Cal eligibility determinations. Applications can be submitted online by visiting www.BenefitsCalWIN.org. Applicants can also drop in, apply via telephone, make an appointment, or mail in their application to any Department of Social Services office in the county. To apply for Medi‐Cal coverage call or go to your local enrollment workers are also available on a regularly scheduled basis at Cottage and Marin Hospitals.
It may take up to 45 days to process your Medi‐Cal application. If you apply for Medi‐Cal based on disability, your application may take up to 90 days to process. You will receive a letter in the mail telling you if your Medi‐Cal application has been approved or denied. If your application is denied, you may contact the SDI office to appeal the decision and set up a hearing before an administrative judge.
Medicare
Medicare, a federal health insurance program, is an important source of health insurance coverage for those with disabilities (such as AIDS) and those with illnesses and chronic conditions associated with aging. To be Medicare eligible you must be 65 or older, under 65 with certain disabilities, or an individual of any age with End‐Stage Renal Disease (ESRD). HIV disease and some of its treatments are associated with renal complications, including ESRD.
Medicare’s broad coverage is organized into four parts: Part A (Hospital Insurance); Part B (Medical Insurance); Part C (also known as Medicare Advantage); Part D (Medicare Prescription Drug Coverage).
Part A helps cover inpatient care in hospitals, skilled nursing facilities, hospice, and home health care.
Part B helps cover medically‐necessary services like doctors' services, outpatient care, durable medical equipment, home health services, and some preventive services.
Part C offers health plan options run by Medicare‐approved private insurance companies. Part D helps cover the cost of prescription drugs.

Financial Assistance
State Disability Insurance (SDI)
The state of California provides short‐term disability insurance, commonly referred to as SDI, administered by the Employment Development Department (EDD). SDI benefits are provided to workers who are unable to work due to an illness, injury, or pregnancy that is not covered by worker compensation law.
The SDI office serving your community will evaluate your claim, and you will receive written notice within two to three weeks after applying. If your application is denied, you may contact the SDI office to appeal the decision and set up a hearing before an administrative judge.

Application Process
Submit a completed claim form to your local SDI office. (Forms are available at all hospitals, your local SDI office and from private physicians, and many employers). Fill out everything on the form except for the physician’s section, which your doctor must fill out to certify your disability.
It is important that you apply as soon as possible after becoming disabled, because you could be penalized or become ineligible for filing a late claim.
Eligibility
You must be employed by an organization that participates in the program (refer to your pay stubs, section SDI) or are self‐employed and have elected to participate in the SDI program. You must be paying into the program at the time of disability.
SDI defines disability as illness or injury, either physical or mental, which prevents an individual from doing their usual or customary work. The SDI definition includes disabilities resulting from elective surgery, pregnancy, childbirth, or a related medical condition.
The SDI application requires certification of the disability by a medical provider. There is no proof of United States immigration status required for application to SDI.
Benefits
Benefits are paid every two weeks (bi‐monthly) for a maximum of 52 weeks. Benefits based on self‐ employment elective coverage are paid for a minimum of 39 weeks. Benefit payments range from $50 to $840 per week depending on the individual’s income during the SDI program’s 12‐month base period prior to the onset of disability. Consult a benefits counselor to determine the amount of your benefits according to the SDI assessment system.
Social Security Administration
The Social Security Administration has two programs for people with disabilities: 1. Social Security Disability Income (SSDI or Title 2)
2. Supplemental Security Income (SSI of Title 16)
A disability is defined as any medical condition (physical or mental) that prevents someone from working for at least 12 months, or is expected to result in death.

Social Security Disability (SSDI)
To qualify for Social Security Administration benefits, you must have a medical condition (be disabled) that meets Social Security’s definition of disability. The definition of disabled under Social Security is based on your inability to work. You are considered disabled “if you cannot do work that you did before” and “cannot adjust to other work because of your medical condition (s).” Your disability must last for one year or result in death. You also must have paid into the Social Security system through your employers during approximately five of the last ten years before becoming disabled. This means you must have worked in a job or jobs where Social Security taxes (FICA) were withheld from your paycheck, or worked as a self‐employed person who paid taxes quarterly.
Benefits
The amount of your benefits depends entirely on your earnings: how much and how long you have paid into the Social Security system. Please refer to the website www.socialsecurity.gov for benefit

Application Process
You should apply for Social Security Disability as soon as you become disabled, because it takes time for your application to be processed. You may begin this process by calling the national toll‐free number 1‐ 800‐772‐1213, or your local Social Security office to make an appointment. If you have AIDS or disabling HIV diagnosis, let them know when you make your appointment so they will process your application more quickly. Supplemental Security Income (SSI)
SSI benefits depend on where you live. The basic SSI check is the same nationwide. Effective January 2005, the SSI payment for an eligible individual is $579 per month and $869 per month for an eligible couple. In most cases, the state of California provides additional income for qualifying individuals and couples. The total benefits are based on specific eligibility requirements, which vary according to factors. Payments are made in a combined federal and state sum on a monthly basis. Veteran’s Benefits
The Veterans’ Services Program with the County of Santa Barbara provides assistance to the veteran community in applying for, obtaining, and maintaining all available benefits and entitlements to which they may be eligible. Those eligible for assistance include veterans, widow of a veteran, children of a deceased or disabled veteran, and parents who lost a son or daughter in military service. Readjustment counseling for returning combat veterans and victims of Military Sexual
Trauma (MST) offered every Thursday at the Veterans Memorial Building located at 112 West Cabrillo Boulevard in Santa Barbara. Sessions last between 45 to 60 minutes with a focus on Post Traumatic Syndrome Disorder (PTSD). To schedule an appointment, contact Vet Center staff. All participants are asked to bring their DD Form 214 (Certificate of Release or
Discharge from Active Duty) to the first session to provide evidence of military service.

Cal Fresh (Food Stamps)
Cal Fresh is a federally funded, county administered program designed to help low income people with their dietary needs. Your monthly allotment of CalFresh depends on your income. If you receive SSI (Supplemental Security Income), you cannot also receive CalFresh, since SSI includes money for food. There are specific eligibility requirements. Contact the Santa Barbara County Department of Social Services for more information.

CalWORKs
CalWORKs is a federal, state, and county program administered by the Department of Social Services. It provides money and services to eligible families with children. Benefits depend on the size of your family and your income. The state decides how much money you should be allowed, and then subtracts your income from that amount. CalWORKs pays the difference. If you receive CalWORKs benefits, you will also receive Medi‐Cal. To apply, contact one of the following Social Service agencies below or apply at https://www.benefitscalwin.org .

General Relief
General Relief is a county funded loan program. Eligible clients may receive between $91 to $300 depending upon their living situation. You must apply separately for CalWORKs. If you are unable to work, you need to submit a letter from your doctor to that effect and participate in the Work Project Program. You will also be required to apply for SSI/SSP and sign a lien. When a client is approved for SSI/SSP, the retroactive payment will be used to repay any General Relief received.

Unemployment Insurance Claims
If you become unemployed through no fault of your own and other work is not available, or your hours have been reduced, and you’re ready, willing, and able to accept immediate work, you may be eligible to receive Unemployment Insurance Benefits. Specific eligibility requirements apply. You may file an Unemployment Insurance claim by using one of the following methods:
Online
File online with eApply4UI. This is the fastest way to file an UI claim. The application is available in English and Spanish, and you can file a new claim, reactivate an existing claim, or file for extended benefits (when these benefits are available) using the eApply4UI application. It is secure, reliable, and available 24 hours a day. Access eApply4UI on the EDD website at https://eapply4ui.edd.ca.gov.

Customer service representatives handle UI claim filing Monday through Friday, 8 a.m. to 5 p.m., Pacific Standard Time. Mondays and Tuesdays are the busiest days. For faster service, you may wish to call Wednesday through Friday. The EDD is closed on state and federal holidays.
Mail or Fax
File a UI application by mail or fax by accessing the application online at
www.edd.ca.gov/Unemployment. The application for UI can be filled out online and printed, or printed and completed by hand. Mail or fax your UI application to the EDD address or fax number listed on the application.
Workforce Services
The EDD’s Workforce Services is a partner in the One‐Stop Career system, which integrates employment and training programs and provides flexible solutions at more than 200 service locations throughout California. There is never a fee for services to job seekers or employers. Resource Rooms offer access to computers, the internet, labor‐market information, job postings, resume assistance, job search
workshops and more. Job seekers can access job listings 24 hours a day, 7 days a week using CalJOBS EDD’s on‐line and resume bank. To take advantage of these services visit your local Workforce Services office or for more information visit EDD’s website at www.edd.ca.gov.

AIDS Drug Assistance Program
The California AIDS Drug Assistance Program (ADAP) was established in 1987 to help ensure that people living with HIV and AIDS who are uninsured or under‐insured have access to medications which have been determined to prolong quality of life and to delay the deterioration of health among individuals infected with HIV. Individuals are eligible for ADAP if they:
Are a resident of the State of California Are at least 18 years of age
Have an HIV diagnosis
Have limited or no prescription drug benefit from another source Have a Federal Adjusted Gross Income of not more than $50,000

There are currently over 180 FDA approved medications commonly used in the treatment of HIV/AIDS infections that are available through the California ADAP Program. Currently, most pharmacies in Santa Barbara County serve persons enrolled in ADAP. Your Pharmacist will be able to tell you if they are an ADAP provider.
You may apply for services with your local ADAP enrollment coordinator at any of the three County Public Health Department locations in Santa Barbara, Santa Maria, or Lompoc.

HIV Care Program & Ryan White Part C
The California Office of AIDS HIV Care Program and the US Department of Health and Human Services Ryan White Part C Program provide funding for medical, case management, psychiatric, dental, and other services for people with HIV who do not have medical insurance or other means of payment. These services are funded through the HIV/AIDS Program of the Santa Barbara County Public Health Department. People with HIV do not apply directly to these programs, but work with their case
manager or the staff of the Public Health Department Health Care Center to determine their eligibility.

Medically Indigent Adult Program
The Medically Indigent Adult Services Program (MIA) is operated by the Santa Barbara County Public Health Department to help uninsured permanent residents of Santa Barbara County secure critically needed medical care. Eligibility for MIA assistance is based on specified medical conditions, financial need, citizenship, age, and the absence of other medical coverage. Applications must be submitted seven days before or seven days after a medical consultation or procedure.

Syringe Exchange Program
Pacific Pride Foundation operates The Right Outfit Syringe Exchange Program through its two locations in Santa Barbara and Santa Maria. Program staff provide risk management and harm reduction
education, substance abuse counseling, HIV and Hepatitis C testing, and needed referrals, in addition to 1‐1 exchange of new syringes for contaminated ones. Program locations include:

Case Management, Housing and Support Services
Case Management Services
Pacific Pride Foundation coordinates the care of over 150 people living with AIDS and also helps HIV positive people seeking assistance through the Early Intervention Program (EIP). The staff is made up of nurses, social workers, therapists, and other professionals who work with clients to link them with the services and resources they need for their continued care and support. Each client is assigned a Case Manager who, along with the client, develops an individualized care plan to coordinate care and connect him or her with community resources and services.

Nutritional supplements and counseling Transportation
HOPWA (Housing Opportunities for People With AIDS) Skilled in‐home nursing
Benefits Counseling: Social work Case Managers provide information about private benefits, such as employer sponsored insurance and public benefits such as State Disability Insurance, Social Security Benefits and Supplemental Security Income (SSI). Counselors can also assist clients in obtaining financial assistance.
Housing Assistance: Case managers coordinate access to emergency rental assistance and long‐ term housing solutions.
Risk Reduction Therapy: A Risk Reduction Specialist is available to work with high‐risk clients on a weekly basis.
Nutritional Counseling: Available to clients in conjunction with the Necessities of Life food pantry.
Legal Assistance: Referrals available to Pacific Pride Foundation staff attorney.
Other Services: Clients are also assisted with transportation services, volunteer support services and food programs.

Volunteer Services
A number of client services are coordinated through Pacific Pride Foundation’s Volunteer Services Program. This program serves as a link between requests for services by clients and the volunteers who provide practical services and support for those living with HIV/AIDS. Services include:
Ride Requests: Rides are available to medical appointments, to and from the Food Pantry, and other important appointments. This service is provided depending on staff and/or volunteer availability only.
AIDS Information Hotline: Staffed by Pacific Pride Foundation employees and specially trained
volunteers, this service provides information and resource referrals to callers 18 hours a day, seven days a week. The Hotline provides both English and Spanish language services everyday from 8:00 a.m. until midnight. Santa Barbara & Ventura: (805) 965 – 2925
Legal Services
Free and low‐cost legal services are available to low‐income families and senior citizens through the Legal Aid Foundation of Santa Barbara County. Legal services are provided in the following areas of law: domestic violence, landlord/tenant law, public benefits, limited family law, civil rights, and consumer protection. Additional assistance is provided through PROJECT OUTREACH in Santa Barbara and Goleta. Volunteer lawyers provide ‘advice only’ outreach at various community centers, offer pro bono
representation in certain types of cases, and assist at Legal Aid’s offices. For more information visit
