Valley Series plans offer benefits on a multi-tiered basis. This offers members access to robust

local and national networks of hospitals and physicians, so employees can make choices that

minimize out-of-pocket expenses while maximizing their benefits. We know how important

flexibility is, and this is why members can also use a non-network provider of their choosing

if they have a plan with out-of-network benefits. Non-Network providers are those that do not

contract with UnitedHealthcare. Below are easy steps to find Valley Series providers.

Tier 1 Network

When employees access Tier 1 physicians and facilities in the Valley Preferred network, they

receive a higher level of benefits, including full coverage for eligible preventive care and lowest

out-of-pocket expenses.

To view the Valley Series Tier 1 Provider Directory:

1. Go to www.myuhc.com/groups/valleyseries

2. Log in or register if it’s your first time visiting the site

3. Select the Find Physician and Facilities tab

4. Select Directory of Physicians and Facilities

Tier 2 Network

Employees can choose to access UnitedHealthcare’s broader network of 589,000

physicians and 4,920 hospitals in Tier 2.

To view the Tier 2 UnitedHealthcare Provider Directory:

1. Go to myuhc.com

2. Log in or register if it’s your first time visiting the site

3. Select the Physician & Facilities tab or the Find a Doctor button

4. Select Find Physician under Search for Care in Your Network

5. Fill in the form to define your search. You can search by Location,

Specialty, Physician Name, Condition or Procedure

6. Then click on the Continue or Search button

Tier 3 Non-Network

Employees can access out-of-network providers in Tier 3 for

Choice Plus and Consumer-Driven Health plan designs.

Welcome!

We’re glad you have chosen UnitedHealthcare for your health coverage. We want to

help you take control and make the most of your health care benefits. Here are a few

suggestions and reminders to help you get started.

It is important that every individual establish a relationship with a physician

to facilitate easier access to care, should the need arise. Establishing a

relationship has proven to improve access to care when you do need it and

helps promote continuity in your health care.

Remember that your choice of a physician or other health care professional is

your own. We will provide you with information to help you make informed

choices, such as physicians’ and health care professionals’ credentials, range of

services offered, fees they charge and performance on quality measures and

surveys. But, this information is not an endorsement of a particular physician or

health care professional’s suitability for your needs.

Typically, you will have lower out-of-pocket expenses by visiting a physician or

health care professional who participates in our network. These physicians and

health care professionals have contracted with us to provide financial discounts

and participate in programs that may facilitate and enhance your health care

experience.
