Thursday – Friday November 19-20, 2015

Purpose

The Continuous Service Readiness (CSR) Program for the Carolinas is a joint effort between the Joint

Commission Resources (JCR) and South Carolina Hospital Association (SCHA) to structure and

implement a program for hospitals to receive education, consultation, and feedback on a continuous basis

concerning Patient Safety and Joint Commission compliance requirements. Through CSR, hospitals have

an avenue to help share a continuous service readiness process that incorporates performance

measurement and patient safety into the quality processes.

Objectives

Upon completion of this workshop, participants will:

• Identify one revision to the Joint Commission Standards for 2015.

• Identify two of the most challenging Joint Commission Standards.

• List what surveyors are looking for during a Joint Commission Survey through discussion with a panel of individuals from hospitals that have been recently surveyed.

 Explain the requirements and discuss strategies for complying with medication management standards.

• Identify two to three challenging medical staff standards.

• Explain the logistics of the survey process.

• Identify at least one best practice for the medication reconciliation process.

REGISTRATION AND FEES

The early bird registration fee is $230. Registrations received after November 11 will be considered late and the fee will be $260. All walk up registrations are required to pay the late registration fee and payment must be collected onsite.

SCHA will now offer online registration. To register for this meeting online, visit the SCHA web calendar,

www.scha.org/calendar, select CSR, and click “Online Registration” to sign up today. A registration and payment confirmation will be emailed directly to you once you complete the online registration process. To complete the online registration, you will need to use a username/password that you can create if you do not already have one. If you experience problems, please contact SCHA at 803-796-3080 for assistance.

REFUND/CANCELLATION POLICY

The deadline for registration is November 11, 2015. Any registrant who cannot attend a meeting may send a

substitute from the same organization. If possible, please notify SCHA of the substitution in writing (by letter or FAX).The registration fee (less a $25 processing fee) is refundable if SCHA is notified of the cancellation no later

than November 11, 2015 (5 business days prior to the program.) Refunds will not be made for no-shows and late cancellations.

Block rate for the CSR for the Carolinas is $189.00 a night single or double. The cut-off date for this room block is

November 6, 2015. To receive this special rate, please call 1-888-627-8048 and ask for the SCHA CSR Room block.

WORK TO BE FIT

SCHA is committed to providing a safe and healthy work environment for its employees and guests, compliant with our “Work to be Fit” wellness program. Therefore, policies and standards for tobacco, nutrition, and physical activity have been implemented to support such a culture.

Tobacco – SCHA encourages tobacco free people and places so tobacco use is prohibited at SCHA events and on SCHA grounds.

Nutrition – SCHA provides a healthy food environment where the healthy choice is the easy choice. Healthy menu items that comply with the healthy food nutrition criteria will be available at each meal service. Please note that sugar sweetened beverages do not comply with the nutrition criteria and will not be readily available.

Physical Activity- SCHA encourages the use of scheduled breaks for physical activity (stretching, walking,

etc) to help maintain optimal health. A paved community walking trail is available adjacent to SCHA

grounds.
