UnitedHealthcare Community Plan (formerly Arizona

Physicians IPA) has been your CRS health plan since

2008. There will not be any changes in how you get care

for your CRS condition or your current CRS services.

We have been serving AHCCCS members for more than

30 years. We are happy to have you as our member.

Exciting changes to the CRS program

You should have received a letter in the mail from AHCCCS. It is

about great new changes to the CRS program that begin Oct. 1,

2013. Many parts of the program are changing. But a lot is staying

the same. Look inside to learn your new CRS coverage type.

AHCCCS is adding primary care and behavioral health services to

the CRS program. This will make it easier to have UnitedHealthcare

Community Plan coordinate your benefits. It will also help doctors

treat you, especially if you go to your clinic for most or all of your

services. Doctors at the CRS Multispecialty Interdisciplinary Clinic

(MSIC) are part of a care team.

What your coverage type means to you.

Find your coverage type by looking at the letter you got from AHCCCS or your new CRS

ID card. Your coverage type will tell you how you can get CRS, primary care and behavioral

health services.

2. 

Read your coverage type description to find out how you will seek care.

3. 

If you have any questions about the changes, call Member Services at 1-800-348-4058.

CRS FuLLY inTEGRATED 010115

Most CRS members are in this coverage type. Your

letter from AHCCCS says “CRS#1-010115” in the

lower right corner. Your CRS ID card says “CRS Fully

Integrated” on the right side of the card.

Use this ID card when you get these services:

CRS specialty services (no change)

Primary care services

(new)

Behavioral health services

(new)

You can continue to see your primary care

physi-cian or your CRS clinic (MSIC) may provide these

services.

For behavioral health services, you will no longer

go through the RBHAs, but you can continue with

your behavioral health provider. Also, your MSIC

may provide these services.

If you have an AHCCCS ID card from another

plan, throw it away when you get your new CRS ID

card. Use your new CRS ID card for all your doctor

appointments and at the pharmacy. Your PCP and

your MSIC are listed on the front of your ID card.

This coverage type is an option for American

Indian members. Behavioral health services will

you can also receive primary care and behavioral health services

at the Metro Phoenix and Southern Region MSICs. Primary

care includes preventive care like flu shots and visits for

everyday illnesses like sore throats. Behavioral health services

include care for substance abuse or mental illness. You will find

your MSIC listed on the front of your ID card.

Getting to the pharmacy can be difficult and time consuming.

CRS members and members of UnitedHealthcare Community

Plan’s DDD program can get many prescription drugs through

the mail.

UnitedHealthcare Community Plan works with optumRx as

your plan’s mail service pharmacy. They can send your

prescrip-tion drugs right to your mailbox. Plus, mail service can save you

time — no more trips to the pharmacy or waiting in line. optumRx

also makes reordering easy.

With mail service, you can get a three-month supply of

medica-tions you take regularly. Just get a written prescription from your

doctor for a 90-day supply of each medication. once optumRx

receives your complete order and the prescription(s), your

medi-cations should arrive within ten business days. Completed refill

orders should arrive in about seven business days. There is no

shipping charge for standard U.S. mail. Mail order is not available

for specialty drugs.

Beginning oct. 1, 2013, CRS members will be

given a one-time option to stay in the CRS

pro-gram when they turn 21. This is for members who

are enrolled in CRS before age 21.

AHCCCS will send an Advance Notification

letter 60 days prior to the enrolled member’s 21st

birthday month. This letter provides instructions for

members to contact AHCCCS to choose to opt in

to CRS.

If you are 20 years old, make sure you look for

this letter from AHCCCS approximately 60 days

before you turn 21. You must wait until you receive

the letter before contacting AHCCCS. If you

choose not to contact AHCCCS, then you will

age out of the CRS program. AHCCCS will

auto-enroll you into an acute care health plan, and will

give you 30 days to change that plan if you wish.

There are lots of simple things you can do to keep your home

as safe as it can be. Explore safety with your child during your

fun! Helmets that fit well help keep children safe on bikes,

scooters or roller skates.

Make a family fire plan. Map and try out two exits from

each room, and go to a safe meeting place outside.

Soap up! Wash your hands before cooking and eating to

keep germs away. Sing “Row, Row, Row Your Boat” twice

to help you remember how long to wash.

Keep a list of important phone numbers near each

tele-phone. Talk about each number so everyone knows how

to get help in an emergency.

Make safety cool by keeping hot objects, liquids or foods

away from the edge of the stove, table or counter.
