HUMBOLDT COMMUNITY SCHOLARSHIP PROGRAM
Eligibility: High School seniors who plan to attend some type of school during the school
year immediately following their high school graduation.
Scholarship recipients are selected without regard to gender, race, national origin, creed,
age, marital status, or disability.
The procedure for applying for the scholarships listed on the following page is as follows:
1.
Complete the scholarship program application form which is located in the “public
folder” in the Teacher Assignments. The application is located in the Guidance
folder. Type the information requested on the form.

Attach a billfold size photo of yourself.

In fifty to one hundred words tell why you should be considered for a scholarship.
Include information about your financial need in as much detail as you deem
appropriate. The statement of need should include but not necessarily be limited to:
an explanation of how you plan to finance your education, to what part of your
college expenses you would apply any scholarship money, and whether or not the
receipt of a scholarship would be a factor that might determine your ability to go to
college. Be certain that your name is in the upper right hand corner of the paper.

Also, list school, community service, and church activities. Include any offices, titles,
and/or honors. You may also include work experience, especially location of work,
number of hours per week, and dates of employment.
5.
Print and submit the application form and accompanying material to your high school
counselor by February 1st.
Answer all questions. If an item does not apply, type “NA” or “Not Applicable” in
that space.
