University of Louisiana at Lafayette And

Louisiana School for Math, Science, and the Arts Articulation Agreement

This document, when signed by all parties, serves as a formal agreement between the University of Louisiana at Lafayette (hereinafter UL Lafayette) and the Louisiana School for Math, Science, and the Arts (hereinafter LSMSA). All conditions of the agreement must be met before students receive credit at UL Lafayette.

General Conditions

1. The continuing agreement is effective on the 22nd day of March 2010. 2. Amendments to this agreement require approval by both parties.

3. The courses of study subject to this agreement may be expanded from time to time by addendum mutually agreeable to both parties.

4. This agreement may be cancelled with thirty (30) days written notice by either party.

5. Faculty employed by LSMSA must meet stated professional credential requirements set forth by the Southern Association of Colleges and Schools which governs the acceptability of course work taught and accepted for transfer credit by colleges and universities.

6. LSMSA must submit a course portfolio, to include, but not limited to examinations and other course documents, for review by UL Lafayette annually or upon request.

7. LSMSA will provide an opportunity for UL Lafayette faculty to observe course instruction. 8. Students must submit a formal University of Louisiana at Lafayette Application for Admission

and an official transcript from the Louisiana School for Math, Science, and the Arts. LSMSA students must meet required academic standards in effect for beginning freshmen (or transfer students, if applicable) at the time of application in order to be admitted to UL Lafayette and receive articulated credit as outlined in this agreement. Application materials are available

online

Enrollment Services at (337) 482-6553 or toll free (800) 752-6553.

9. Students will be granted credit based on the course equivalencies and related requirements listed in this agreement. Course credit will be earned only and indicated with a grade of “S” on the UL Lafayette transcript.

10. Upon enrollment at UL Lafayette, students must have their LSMSA transcript evaluated by the Office of Admissions. This should take place before the student registers to eliminate any

(3)

problems with course credit. Admission applications will be processed and credit posted at that time.

By signature below, the University of Louisiana at Lafayette and the Louisiana School for Math, Science, and the Arts affirm that course equivalencies in Appendix I may continue to be articulated as transfer credit as under the original July 15, 2009, agreement, provided that all conditions of this agreement are met.
