Welcome to Royal Children’s Hospital’s (RCH)
Graduate Nurse Program
Introduction
The RCH Graduate Nursing Program (GNP) is designed specifically for new graduate nurses to develop specific knowledge and skills required to work in a children’s speciality hospital. This is a structured program with integrated study days and debriefing sessions to assist the graduate nurse to practice as safe, confident and accountable professionals.The Program is designed to promote:
• Child and family centred care
• Competence in paediatric nursing practice
• Engagement in critical questioning and reflective practice • Integration into effective multidisciplinary teams
RCH philosophy of care is also developed focused on these four key principles:
Child and family-centred care includes the principles of informed choice, collaborative decision-making and the inclusion of families and carers as partners in the provision of care, as well as the provision of a child and family-centred environment and facilities;
Respect for children’s needs and issues focuses on issues such as developmental needs, pain management and assisting children to become active participants in their treatment and care;
Care co-ordination becomes part of the core business of the hospital – programs of care developed, particularly for patients with chronic conditions requiring multidisciplinary care; and Partnerships and linkages becomes part of the core business of the hospital – stronger relationships developed with metropolitan and regional hospitals, community paediatricians, GPs, community agencies and schools to facilitate integrated care co-ordination.
Program Outline
Program Aim
The Graduate Nurse Program aims to facilitate the transition from a new graduate nurse to a competent registered nurse by supporting learning and development needs in a safe environment.
Program Outcomes
At the completion of this program the participants will:
• be able to demonstrate their practice as a competent registered nurse • demonstrate confidence in patient and family-centred care
• be an advocate for Children and children’s health
• contribute as an effective member of the specialty multidisciplinary team through
collaborative and co-operative teamwork.
• be a talented, respected and valued staff member in the Unit/department • be able to demonstrate the ability to practice in a culturally responsive manner
Structure of the Program:
The program offers the graduate nurse one single 12 month placement in a clinical setting in RCH. A supportive learning environment is provided during this clinical placement. Each graduate will be allocated a preceptor and will have access to Clinical Nurse Facilitators, Clinical Nurse Educators and Transition Program Educators to support their transition process. The graduate nurse will have supernumerary time with their preceptors for up to six to ten weeks of the program depending on each departmental orientation requirement. There is the flexibility to extend or reduce these timeframes in specific instances, subject to agreement with the New graduate, Preceptor, Nurse Unit Manager, Clinical Nurse Educator and Transition program Educator. The key determinant should be that the new graduate is supported adequately to meet the learning requirements during this time.
Content
A planned learning development course combining the clinical experience provides the new graduate nurse the time to explore the knowledge and concepts required for the development of sound clinical reasoning. The content of the program comprises of a mix of clinical and theoretical components presented through the study days focused on management of paediatric patients and families.
Clinical Component:
The clinical time will comprise of normal rostered shifts in an allocated clinical area under the supervision of a Preceptor and the guidance of a Clinical Nurse Educator and Clinical Nurse Facilitator. This is further supported by providing debriefing sessions to support and learn from others. There is ongoing clinical training that occurs in the clinical units with the support of a hospital wide preceptorship model and a competency framework with clear goals, objectives and timelines. Furthermore, collaboration between the Nurse Unit Manager, Nurse Educator, Preceptor & Transition Programme Educator will ensure that a strong support network is available to the new graduate throughout the programme with a clear delineation of roles & responsibilities
Theoretical component:
The GNP consists of a total of 12 study days including GNP orientation day, RCH nursing orientation day and Post graduate studies. During the first half of the program year six study days are facilitated to assist the graduate nurses to develop the knowledge and skills to work in a paediatric speciality environment. During the second half of the program, the nurses are further assisted in building up their advanced clinical assessment skills by participating in Child health assessment program delivered in partnership with the University of Melbourne.
The Child health assessment program is designed as a theoretical (48 hrs) and clinical subject (min 300 hrs) that will enable the staff to receive 25 credits towards post graduate studies. This program facilitates registered nurses to conduct a comprehensive and systematic nursing assessment of children and to deliver effective patient care based on sound theoretical reasoning and critical thinking. The theoretical time may be through planned study blocks, study days/ half days or structured tutorials. Details of the Child Health Assessment program will be provided soon in the Paper Outline.
The equivalent of twelve Study Days, (96 hours), over the period of the programme comprises of: (Appendix 3.1)
• Orientation and paediatric specific study days to RCH, (6 days)
These study days will be facilitated and delivered by experienced clinical nurses/educators under the guidance of the transition programme co-ordinator. These facilitators and teachers will be chosen for their clinical expertise and will be assisted by the Transition Program Educator to utilise problem-solving methods that develop clinical thinking and explore the application and use of knowledge in different clinical settings. These facilitators and teachers will then be available to support the nurses and their preceptors in the practice experience with developing knowledge.
Assessment Process
Feedback is critical to the development of practice and in the progress of learning. Practice assessments will be based on clinical observation and emphasise the new graduate’s ability to manage nursing care, make sound clinical decisions and work effectively in a multidisciplinary team.

Previously set learning objectives are reviewed during the Performance Assessment in terms of progress with outcomes. Your assessor (Preceptor or CNE/F) and NUM will tick to indicate this has occurred. Once the Performance Assessment meeting has occurred and all comments are documented you must sign and date to indicate that you agree with the comments
provided. Once the Performance Assessment meeting has occurred and all comments are documented a member of the Transition Team (or nominated representative) must view this form and sign to indicate that they have read the content.
Your assessor (Preceptor or CNE/F) and NUM are required to provide written feedback after discussion at the Performance Assessment meeting. You are required to capture a self reflection with regards to your performance to date prior to the meeting. You must then sign and date your entry.
Performance Assessments
Performance Assessments provide you with an opportunity to reflect on your own performance as well as meet with your Nurse Unit Manager (NUM) and assessor (Preceptor or CNE/F) to discuss your performance and receive constructive feedback. A Performance Assessment occurs at four time points throughout the GNP: 10 weeks, 6 months, 9 months and 12 months. Your reflections and the comments from your NUM and assessor regarding performance are documented on the corresponding forms found in this appendix as evidence of your professional development throughout the GNP.
Academic Assessments
Child Health Assessment Program assessments include:

Details of these assessment points will be provided in the Paper Outline.
For successful completion of the programme, participants will be required to have their clinical practice assessed as constantly meeting the required performance standards set by you unit manager and hospital policies. This includes Preceptor Feedback, Self-Assessment and Performance Appraisal.
Participants who successfully complete all clinical assessments will receive GNP completion certificate and those who complete academic assessments will receive 25 credit points towards post graduate certificate in nursing.

Support Process
A matrix (Appendix 3.2) which outlines the support network available to all graduates has been developed to assist graduates in accessing appropriate support and resources. Graduate nurses are provided with external resources for support should they be required. The Employee Assistance Program is a free confidential counselling service that is available to all RCH employees. Graduate nurses at RCH are also provided with the contact details of the Graduate and Final Year Student Support Officer from the Australian Nursing Federation.
Facilitated Debriefing
Graduate nurses are also provided with ten opportunities to attend facilitated debriefing sessions throughout the Graduate Nurse Program (GNP). It is an expectation of the GNP that graduate nurses participate in a minimum of five sessions by sharing or actively listening. Should a graduate nurse experience difficulty in attending it will be their responsibility to contact their lead facilitator to negotiate participation in one of the other groups for the month
The purpose of Facilitated Debriefing is to provide support in a safe environment that promotes reflective practice and critical thinking to improve nursing practise.
Participants are provided with an opportunity to:
• Share experiences with others in similar situations • Support and learn from others
• Be challenged to explore current issues in their practise • Consider how learning might be translated into practise
The Transition Stream works closely with the Nursing Development team (NDT) to facilitate and evaluate facilitated debriefing for Graduate Nurse Program (GNP)
Facilitated debriefing sessions are scheduled monthly. Graduate nurses are allocated to one group. These groups meet on one of four days: Tuesday, Wednesday, Thursday or Friday. There are lead facilitators for each of the four groups. Please refer to the table below for a list of lead facilitators and group members.
If there is a change in facilitator for any reason graduate nurses will be notified via email a minimum of 24 hours prior to the scheduled session.
Each group will establish ways of working together during the first scheduled session. Information shared in all sessions remains confidential. Any information shared outside of the group must be negotiated between the facilitator(s) and individual(s).

Learning Objectives
Learning objectives are “statements of desired, observable, teachable, learnable behaviours that are evidence of learning” (Ferguson, 1998). Formulating learning objectives helps to provide focus in terms of what you need and want to learn throughout your GNP. You are required to generate learning objectives that are then agreed upon with your assessor (Preceptor or CNE/F). Learning objectives must initially be formulated by week four and must then be reviewed at each of your Performance Assessments, which are detailed on the following page. After each Performance Assessment you will have a week to agree on any additional learning objectives with your assessor (Preceptor or CNE/F). Please note there is a page for additional learning objectives to capture those that do not fit on one page. This can be photocopied and used as needed.
