Employee Discount Card

This discount card is provided by New Benefits, Ltd. It offers employees access to a discount Vision Plan, a Nurses Hotline, Counseling Services and benefits for Hearing Aids. This is not an insurance plan.

The Discount Vision Planthrough Coast to Coast network allows the employee to receive discounts of 20% to 60% on eyeglasses, non-prescription sunglasses, contact lenses (including disposables), and frames from over 10,000 independent retail optical locations nationwide. Providers include independent practitioners, regional chains, department store optical, and the largest chains in the U.S. Some of these providers are Lens Crafters, Pearle Vision, Sears Optical, and JC Penney Optical (among others).*

The Nurses Hotlineallows access to experienced registered nurses 24 hours a day, 7 days a week, 365 days a year. These hotline nurses are an immediate, reliable and caring source of health information, education and support. Services provided by this plan include:

 Information based on physician-approved guidelines

 Answers about medication usage and interaction

 Information on non-medical support groups

 Translation services for non-English speaking callers

 Full-time medical director on staff

The TeledocTM provides telephone access to a board-certified physician from any phone, 24 hours a day, 7 days a week. Physician can diagnose routine medical problems and prescribe short-term medication when appropriate and will contact the member within three hours.

The Counseling Servicebenefit allows the employee to speak with a counselor 24 hours a day, 7 days a week, regarding any personal problems they may be facing.

The Hearing Aidbenefit provides savings of up to 15% off the retail cost on over 70 models of hearing Aids and a free hearing test when utilizing one of the 1,200 participating Beltone locations nationwide. Or, The employees can realize savings of up to 50% off suggested retail price on over 90 models of hearing aids in over 1,000 locations nationwide.*

Why Consider TransConnect Insurance?

You have insurance so you think you are covered. Then you get hit with a deductible. Protect

your pocket with out-of-pocket insurance.

TransConnect is a group supplemental hospital and medical expense indemnity insurance. It pays an indemnity benefit for out of pocket expenses of deductibles, co-insurance and co-payments for charges from your employer’s comprehensive medical plan.

In-Hospital Benefit

This benefit helps pay the out-of-pocket expense for inpatient hospital stays, inpatient surgeries, physician’s in-hospital charges and routine nursery care for dependent children.

Outpatient Hospital Benefit

This benefit helps pay the out-of-pocket expense for surgery in a hospital outpatient facility or a free-standing outpatient surgery center, up to $100 for surgical procedures performed in a doctor’s office; radiological diagnostic testing in a hospital outpatient facility or MRI facility (does not cover lab fees); or treatment in a hospital emergency room or urgent care center for injury due to an accident (emergency room charges for sickness are not covered). The outpatient hospital benefit is 50% of the selected In-Hospital Benefit amount.

Accident Only Ambulance Benefit

This benefit helps pay for the out-of-pocket expenses incurred for ambulance transportation (ground or air) to a hospital or

emergency center for injuries sustained in an accident.

You present a card to your provider at the time of service. The provider files a claim with the insurance company. A benefit is paid based upon covered out-of-pocket expenses from the underlying medical plan. The benefit offsets expenses created by the medical plan’s deductible, co-pays and/or coinsurance share.

Why Consider Critical Illness Insurance?

The number one cause of personal bankruptcy in the United States is expenses and loss of
