Ponderosa was provided health coverage at work and was covered for every month of 2015 by her 

employer’s plan and received Form 1095‐B and Form 1095‐C at the end of the year.  Her son, Spruce, 

was covered for the entire year by a Marketplace plan purchased by his father, Juniper.  Juniper 

provided a copy of Form 1095‐A (attached) to Ponderosa which reported the health coverage for their 

son, Spruce.  Juniper and Ponderosa agree to allocate the premium percentage, SLCSP percentage, and 

the Advance PTC of the Marketplace policy at 50% each.

Facts

 

Ponderosa and her son are both U.S. citizens and she cannot be the qualifying child of anyone. 

She provided proof of maintenance of the household, including utility bills and rent receipts (attached).  

There is evidence that Ponderosa paid more than 50% of the cost of keeping up the home in which she
