THIS PROPOSAL MUST BE IN THE FISCAL SERVICES DEPARTMENT NOT LATER THAN April 11, 2012 1:00 p.m.

Proposals will not be accepted after the above date and time.

A PROPOSAL BOND WILL NOT BE REQUIRED WITH THIS PROPOSAL . A PROPOSAL BOND OF 5% IS REQUIRED WITH THIS PROPOSAL X . Certified check or proposal bond, if required, must accompany the proposal, at which time the proposals will be opened. No proposal will be considered unless a certified check or proposal bond is furnished, if required.

Every person, firm, or corporation submitting any proposal for construction work, equipment, materials or services of any kind shall be required to furnish with such proposal a certified check or proposal bond in an amount indicated above which shall be forfeited to the Board of Education in case of failure of the proposer to sign a contract after acceptance of the proposal by the Board of Education.

Quotations on equipment, materials or supplies must be F.O.B. point of delivery specified, including packing and crating charges, and fuel surcharges.

MATERIAL INCLUDED IN THIS PROPOSAL CAN BE DELIVERED WITHIN DAYS, FOLLOWING RECEIPT OF ORDER FOR SAME.

Terms allowing discount “A Time of Payment” (not less than 10 business days) will be given preference. Terms of Payment and Delivery will be considered in making awards.

The Board reserves the right to reject any or all proposals or parts of proposals, or to split award by items or to accept the proposal which will best serve the interest of the Board of Education.

Proposers must make their proposals strictly in accordance with requirements and specifications, otherwise they may not be considered.

Proposers are granted the privilege of withdrawing their proposals, if they desire, between the time they are submitted and opened.

Proposers are invited to consult with the Fiscal Services Department concerning conditions required for submitting proposals, use of materials, or previous quotations.

No proposals shall be accepted from, or contract awarded to, any person who is in arrears to the Board of Education of Warren Consolidated Schools upon debt or contract, or who is defaulter as security or otherwise upon an obligation to the Board of Education of Warren Consolidated Schools.

PROPOSAL DESCRIPTION

Gym Floor Refinishing

The undersigned proposes to furnish in accordance with attached specifications and for the prices quoted on the attached Sheets.

Terms to remain firm for the contract period.

References to be provided upon request.

A proposal bond WILL be required.

Prices shall be quoted F.O.B. Delivered.

*Proposals shall be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the owner or any employee of the proposer and any member of the Board or the Superintendent.

* Addenda will be issued no later than three days prior to the date for receipt of proposals except an addendum withdrawing the request for proposals or one which includes postponement of the date for receipt of proposals.

It is the responsibility of any person/persons downloading proposals from the WCS website to revisit the website three days prior to the proposal opening to address any addenda issued. Acknowledgement for the receipt of such addenda must be included in the response.

* Warren Consolidated Schools and the Proposer agrees that they will not discriminate against any employee or applicant for employment on the basis of race, color, religion, gender, age national origin, disability, height, weight or marital status. The supplier shall take affirmative action to ensure that applicants are employed, and that employees are treated without regard to race, color, religion, gender, age, national origin, disability, height, weight or marital status.

In the event that this proposal is accepted to furnish whatever is included herein, the successful proposer agrees to furnish compensation insurance (if labor is required in the fulfillment of this contract order), which will amply protect the interest of the Board of Education.

GENERAL CONDITIONS

1.0 GENERAL CONDITIONS

1.01 Familial - Proposals shall be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the owner or any employee of the proposer and any member of the Board or the Superintendent.

1.02 Conditions - The Warren Consolidated Schools reserves the right to reject and/or accept all proposals or parts of proposals, or to split award by items or to accept the proposal which best serves the interest of the school district.

1.03 Smoking and use of Tobacco Products – Smoking and/or the use of any tobacco product is strictly prohibited within any facility or any grounds at all times.

1.04 Deviations - All deviations from specifications must be listed separately in writing and included in this proposal.

1.05 Proposals - Proposals will be accepted on the forms provided by the owner. No oral, fax or telephone proposals or modifications will be accepted.

SPECIFICATIONS FOR GYMNASIUM AND STAGE FLOOR REFINISHING

1.0 GENERAL CONDITIONS

1.01 Local, State & Federal Laws and Regulations - All work shall be done in conformity with all local, state, and federal laws and regulations and all necessary permits shall be applied and paid for by the contractor.

1.02 Insurance

1. Workers' Compensation - The contractor shall carry during the life of this contract, workers' compensation insurance and shall require all subcontractors to similarly provide such insurance.

2. Public Liability & Property Damage - The Contractor shall carry and maintain during the life of this contract:

Bodily Injury

Each Accident Occurrence $500,000.00

Aggregate 500,000.00

Property Damage

Each Accident Occurrence 500,000.00

Aggregate 500,000.00

Or Combined Single Limit $1,000,000.00 with Broad Form Comprehensive

General Liability Endorsements

3. All other insurance as required by the laws of the State of Michigan.

4. The successful proposer shall provide evidence of his insurance coverage within ten days notice of award.

1.03 5% Proposal Bond - The contractor shall be required to submit a 5% proposal bond with their proposal.

1.04 Performance, Labor, and Material Bond - The contractor shall be required to furnish performance, labor and material bonds in full amount of the contract insuring faithful performance of all provisions of the contract.

1.05 Conditions - The Warren Consolidated Schools reserves the right to reject and/or all proposals or parts of proposals, or to split award by items or to accept the proposal which best serves the interest of the school district.

1.06 State Sales Tax - The contractor is required to pay all Michigan Sales and Use taxes on

1.08 Protection of Work and Property - For the duration of work performance, contractor shall take all necessary precautions to insure the safety of employees and the public. Contractor shall use reasonable care of owner's building, site, and equipment. Contractor shall be responsible for damages caused to facilities at no cost to the owner.

1.09 Deviations - All deviations from specifications must be listed separately in writing and included in proposal package.

1.10 Guarantee - Each contractor shall furnish a written one year guarantee covering all materials and workmanship.

1.11 Proposals - Proposals will be accepted on the forms provided by the owner. No oral, fax, or telephone proposals or modifications will be accepted.

1.12 Contractor's Qualifications - Before the award of a contract, the successful proposer will be required to furnish information as listed below to satisfy the Warren Consolidated Schools Board of Education that the proposer is adequately capable to fulfill the contract.

A. Address the description of proposers' plant and equipment.

B. List of past projects of similar scope as this and names and addresses of references.

1.13 Square Footage - Contractor will be responsible for actual measurements and quantities.

1.14 Time of Completion - Work to be completed between 6/11/12 and 08/13/12. Work will be coordinated between contractor and school representatives to be worked around building use due to building renovations.

1.15 Work Schedules - Schedule to be provided to owner for approval prior to beginning work.

Building to be available for contractor to perform work from 6:30 a.m. - 5:00 p.m. Monday - Friday.

SECTION A - SCREEN & REFINISH

2.0 SCOPE OF WORK/LOCATIONS

2.01 Provide all necessary labor, material and equipment to surface-screen existing gymnasium and stage floors and the re-application of two coats of new floor finish, including touch-up of game lines, keys and center circle at the following buildings: Including auxiliary gyms at Cousino, Sterling Heights, and Warren Mott.

Gym Floors:

Beer Middle School – 3200 Martin, Warren

Butcher Community Center - 27500 Cosgrove, Warren Carleton Middle School - 8900 15 Mile Rd, Sterling Heights Carter Middle School - 12000 Masonic, Warren

Flynn Middle School – 2899 Fox Hill, Sterling Heights Grissom Middle School – 35701 Ryan Rd, Sterling Heights

Warren Mott High School – 3131 Twelve Mile Rd, Warren – Main and 2 Auxiliary gyms Cousino High School – 30333 Hoover, Warren – Main and 2 Auxiliary gyms

Sterling Heights High School – 12901 Fifteen Mile Rd, Sterling Heights – Main and 2 Auxiliary gyms 3.0 METHODS/SCREENING AND FINISHING

3.01 Adequately screen existing finish to insure proper adhesion of new finish using one 20” 100 grit 3M screen per 500 sq. ft. of floor. Remove any tape and gum prior to abrading. Abrade between coats and all paint lines using #2 steel wool or Hillyard Maroon pads per 500 sq. ft. of floor. Touch up all game lines, as needed.

Repaint all center court logos and keys.

3.02 All work to be in accordance with manufacturers recommended methods for wood floor re-finishing.

4.0 SEALING AND FINISH

4.01 Apply 2 coats of new finish at a rate of 500 sq. ft. per gallon per coat. Include cost of finish in quote.

4.02 Floor finish to be Hillyard 450 or owner approved equal.

5.0 GAME LINE PAINTING

5.01 All game lines to be in conformance with National Federation of State High School Associations Court Diagram Guide.

5.02 Repaint all keys and center court logos at all schools. All other game lines to be touched up as needed to match existing colors. Paint is to be top quality alkyd semi-gloss latex enamel, compatible with gym floor finish.

SECTION B –SCREEN, PAINT, & REFINISH

2.0 SCOPE OF WORK/LOCATIONS

2.01 Provide all necessary labor, material and equipment to surface-screen existing hardwood floors. Apply two coats of new floor finish on hardwood floors and paint existing painted areas.

2.02 Locations:

Screen and Paint Sterling Heights High School Mini Theater Stage per layout 3.02 below Screen and finish side entryways to stage part of Sterling Heights High School PAC stage

Screen and paint front and back part of Sterling Heights High School Arts stage per layout 3.02 below Paint Cousino High School Café stage per layout 3.02 below

Screen and paint Warren Mott High School P.A.C. stage per layout 3.02 below Screen only, no paint or finish at Sterling Heights High School T.A.P.P. room

Sterling Heights High School – 12901 fifteen Mile Rd, Sterling Heights (Performing Arts Center) 3.0 METHODS/SCREENING AND REFINISHING

3.01 Adequately screen existing finish to insure proper adhesion of new finish. Abrade between coats and all paint lines using #2 steel wool or Hillyard Maroon pads per 500 sq. ft. of floor Repaint al lines where needed using a different color.

3.02 Stage floors to be painted per specifications as follows: Use Rosco Black Tough Prime satin finish #06055.

3.03 All work to be in accordance with manufacturers.

4.0 SEALING AND FINISH

4.01 Apply 2 coats of new finish at a rate of 500 sq. ft. per gallon per coat. Include cost of finish in quote.

4.02 Floor finish to be Hillyard 450 or owner approved equal.

5.0 GAME LINE PAINTING

5.01 All game lines to be in conformance with National Federation of State High School Associations Court Diagram Guide.

Extra Line Painting. Include on the proposal form a quote, per hour for additional line painting.
