Core components included at no cost

Needs Assessment • Members of HEW staff will conduct a needs assessment to evaluate the target population needs • HEW will provide a link for electronic survey to be sent by employer • Aggregate results report provided for management Health Communications • Marketing materials, such as flyers, provided to either email and/or display on bulletin boards to communicate information about HEW and MyHEWOnline • Standard health education materials on popular health education topics (diabetes, heart health, weight management, smoking cessation) provided for employers to distribute (each topic template will include an article, health tips, e-mail blast and recipe)

Customized Website Portal - MyHEWOnline

• Personalized website developed for employees with online learning modules and webisodes

• Learning modules on the following topics: diabetes, heart health, pregnancy, preventive healthcare, stop smoking, and weight management

• Utilization tracking

Health Risk Assessment (HRA)

• The HRA is an online tool used to assess health risks. Results from the HRA flag health risk factors that require additional attention by the employee • Custom login and password will be supplied to your employees to create an employer group user account • Employees receive a personalized profile report • Aggregate result report provided for management • HEW staff can meet to present recommendations to management team

Employee Health Challenges and Health Fairs • Onsite behavioral modification programs • Printed collateral materials to facilitate program • Health educator trains designated employee to become a facilitator to conduct onsite program • Health fair option available for employers with 500 or more employees (30 day advance notice required)

Virtually Connected Online Incentive-Based Program

Participant completes the following: • Online Health Risk Assessment (HRA) • Follow up with 30-minute telephonic consult • Complete one of six online learning modules • Telephonic-based health coaching • Up to five 30-minute telephonic consults with action plan • Unlimited inbound calls from member Employer receives the following: • Customized aggregate reporting • Utilization tracking Worksite Wellness Packages

In addition to the core components, this package includes risk stratification and medical services utilization of your population to establish goals and baseline data for tracking. Class to include: • Review the HRA, explaining the profile report • Fitness assessments are conducted • Exercise: benefits of both static and active stretching - 30 to 45 minute workout; learn the basics of health and fitness • Nutrition: learn the relationship between exercise and nutrition

• Attend stress management class with health educator

• Discuss stress management techniques and problem solving

• Discuss how lifestyle choices impact stress

• Review the HRA explaining the profile report

• Exercise and nutrition: benefits of physical exercise and nutrition to help control stress

• Stress: learn to change your thinking patterns, understand the nature of stress, physiology and disease

Tobacco Cessation Bundle

Tobacco Cessation Program

• Online Health Risk Assessment (HRA) • Orientation – one-hour orientation to explain the program; health educator would be on site at the orientation to schedule interested employees into the program • Attend tobacco cessation group sessions • Learn how to make it through the quitting process and stay smoke free by identifying triggers and developing coping strategies

Personal Health Coaching

Class to include: • Review the HRA, explaining the profile report • One-on-one consultations – all employees that sign up for the program will then meet with the tobacco cessation counselor for a ½ hour, one-on-one consultation to complete any necessary paperwork and to discuss smoking cessation goals • Group sessions – one group session each week for 12 weeks (each session would be one hour long – depending on what works best for your population)

Weight Management and Support Group Bundle

Weight Management Program

• Online Health Risk Assessment (HRA)

• Attend one-on-one consultation with health educator

• On-site biometric screenings (weight, blood pressure, waist circumference, fasting glucose, cholesterol, LDL, HDL, Chol/HDL ratio, and triglycerides)

• Anthropometric measurements (weight, waist circumference and body fat) on weeks 1, 6, 12.

Personal Health Coaching

Group to include:

• Initial consultation with the on site health educator to develop an action plan towards making and adhering to lifestyle goals

• Non-smokers choose from one of the following:

• Participate in a weight loss or weight management program • Participate in a stress management program • Participate in a nutrition class Year 2: Engagement • Complete Online Health Risk Assessment (HRA) and attend one-on-one consultation with health educator • Annual physical exam or biometric screenings - communicate move to outcomes next year • Participate in tobacco cessation class if participant is a tobacco user (or remain smoke free if a non-tobacco user) – 10 of 12 sessions

• Non-smokers choose from one of the following:

• Participate in a weight loss or weight management program

• Participate in a stress management program

• Participate in a nutrition class

Year 3: Engagement and Outcomes

• Complete Online Health Risk Assessment (HRA) and attend one-on-one consultation with health educator • Complete biometric screening: • BMI ≤ 27.5 or waist circumference < 40 for men < 35 for women • Blood Pressure ≤ 130/80 • Total Cholesterol ≤ 200 mg/dL • Nicotine (health assessment) • Annual physical exam/age-related cancer screenings • Participate in tobacco cessation class if participant is a tobacco user (or remain smoke free if a non-tobacco user) – 10 of 12 sessions

• Non-smokers choose from one of the following:

• Participate in a weight loss or weight management program • Participate in a stress management program • Participate in a nutrition class Outcome data feed into MyHEWOnline and participants can track their progress on MyProfile Multi-Year Program

Associate Director Wendy Ronovech, BS M.Ed.

Wendy is the Associate Director of UnitedHealthcare’s Health Education and Wellness (HEW) Division. She develops wellness programs, plans, organizes, and directs the day-to-day operations of the HEW department. Wendy strives to provide clients with improved services and quality for living a healthy lifestyle. Supervisor If interested in worksite wellness programs, please contact Alexis at (702)240-8972. Alexis Jorge, BA Alexis supervises the health educators located in the Southwest Medical health care centers, other health care centers, and worksite locations. She markets Worksite Wellness services to employer groups and brokers, and assists in the development of wellness programs with the HEW team by organizing, setting up the day-to-day operations, and implementing programs for clinics and worksites.

Registered Dietitian Kristen Christiansen, MS, RD

Kristen specializes in culinary nutrition, nutrition analysis, and healthy dining room rotation menus. She will provide guidance, create healthy menu plans, and set goals related to nutrition and wellness.
