How do I sign up for the tour?

The attached application with essay question and waivers must be filled out completely by the student, signed by the student and a parent or guardian and returned as soon as possible. Return completed applications to one of the following places:

Applications are due no later than Tuesday, October 14, 2014 at 3:00pm. Space is limited, so

turn in your applications as soon as possible to ensure your spot on the trip!

How much does the North Dakota College Tour cost?

The cost of the tour is $125.00. Included in the price of the tour is transportation throughout Minnesota and North Dakota on a coach bus, walking tours and information sessions at each school, hotel accommodations for Friday night, meals, and Project SUCCESS staff who will guide you through a series of workshops and chaperone the trip. Once Paul has confirmed you have a spot, the $125 will be due no later than Wednesday, October 22, 2014 and should be mailed to: Project SUCCESS, College Tours, One Groveland Terrace, Suite 300, Minneapolis, MN 55403. Please make your check or money order payable to Project SUCCESS. Please do not send cash in the mail.

The following is a tentative schedule for the Project SUCCESS North Dakota College Tour. We will be visiting three

universities in a short period of time. The schedule is rigorous, but by the end of the tour you will be that much wiser

in making decisions for your future!

Saturday, November 1, 2014

8:00-9:15am Breakfast at hotel, hotel check-out

10:00am Admissions presentation, campus tour, and Project SUCCESS workshop at North Dakota State University 12:30pm Lunch at North Dakota State University

1:30pm Travel to Moorhead, Minnesota

2:00pm Admissions presentation, campus tour, and student panel at Minnesota State University, Moorhead 4:00pm Project SUCCESS closing workshop

5:00pm Drive to Minneapolis, MN (dinner on the way) 9:30pm Bus drops off at the Project SUCCESS Office

**The bus will return to the Project SUCCESS office at One Groveland Terrace in Minneapolis (directly behind the Walker Art Center). Students should make arrangements to get a ride home from the Project SUCCESS office Saturday night. We will

have cell phones on the bus so students can call home as we get closer to Minneapolis.**

7:30am Bus departs from the Project SUCCESS office, drive to Grand Forks, North Dakota Introduction workshop during the bus ride

***Bus will leave promptly at 7:30am and cannot wait for latecomers. Please leave yourself plenty of time to get to our office and call in advance with any questions or concerns.

1:00pm Lunch at the University of North Dakota

2:00pm Admissions presentation and full campus tour of the University of North Dakota 4:00pm Explore Grand Forks, North Dakota

5:00pm Drive to Fargo, North Dakota

6:30pm Arrive in Fargo, North Dakota. Dinner, check in to hotel, explore Fargo, North Dakota

9:00pm Students will participate in a college tour workshop led by Project SUCCESS. This workshop is meant to help acquire the tools necessary to make this tour and others as productive and informative as possible.

GETTING TO THE PROJECT SUCCESS OFFICE

All students should meet the bus at the Project SUCCESS office and arrange transportation home from the

Project SUCCESS office at the end of the trip.

We are located at One Groveland Terrace, Minneapolis, MN 55403. It is a two story tan brick building

directly behind the Walker Art Center at the corner of Hennepin Avenue and Groveland Terrace.

The bus will leave promptly at the time listed for the trip and cannot wait for latecomers. Please

leave yourself plenty of time to get to our office and call in advance with any questions or concerns.

Project SUCCESS is dedicated to making these tours accessible to all of our high school students.

Bus passes are available to get to and from our office. You can find bus route Information at www.metrotransit.org or by calling 612-373-3333.

Please indicate on the application if you will need a bus pass.

Bus passes can be found in the office where you picked up the application at your school.

Make sure to fill this out completely and return it as soon as possible, but NO LATER THAN Tuesday,

October 14, 2014 at 3:00pm. Space is limited so do not wait until the deadline to apply. Applications

received late or incomplete will only be considered for stand-by spots on the tour. After we receive your

application, Paul will look over your essay and call to confirm your place on the tour.

7. The cost of the tour is $125.00. Once Paul has confirmed that you have a spot, the $125 will be due no later

Limited need-based reduced trip fees are available for up to $75 of the trip cost. We ask that if you do have the

funds to cover your trip, that you pay the full amount. However, Project SUCCESS wants to ensure that we include

all students who are serious about their college search, regardless of cost. If you are not able to pay the full $125,

we appreciate the partial contribution you can make toward your trip cost. Your contribution will help us to continue

bringing young people on college tours each year and to serve our growing community of students. Do you want to

8. Hotel rooms are separated by gender. In order to make this tour affordable for everyone, students will be 4 to a

room at the hotels where there are 2 queen sized beds. Each student will be sharing a bed with another student. If

you know another student who is going on the trip and you would like to share a room, please let us know. If you

feel uncomfortable sharing a bed, you may bring a sleeping bag and sleep on top of the bed instead.

North Dakota College Tour: Student Agreement

This trip provides a valuable opportunity to inspire you by showing the many possibilities out there for your college

career and inform you of the details necessary to succeed in that arena. Equally as important on this trip is your

safety. Over the years, we have developed a list of rules that will ensure you and all other participants have a safe

and pleasant trip. Please read the list carefully. In order to come with us to visit colleges on this trip each student

MUST agree to and sign a copy of these rules.

Important Note: We know some students will be 18 at the time of the trip. We follow the same rules as those of

your school. All students, no matter what age, will be asked to follow these same rules.

1. There will be plenty of time for students to “hang out” with friends during the day and in hotel common areas.

a. At no time will students be allowed in another hotel room that is not their assigned room.

b. At no time will any student with the Project SUCCESS group be allowed to enter the bedroom of a

person not with Project SUCCESS.

c. At no time will any person not with Project SUCCESS be allowed to enter the bedroom of someone from

our group.

d. Sexual activity and intimacy between all participants on a Project SUCCESS College Tour is prohibited in

order to create a safer, more respectful, more drama free experience for everyone.

2. The use of alcohol and/or drugs is illegal and strictly forbidden on this trip. Students who choose to use alcohol

and/or drugs will be sent home immediately at the expense of their families and your schools will take

disciplinary action.

3. All activities during the 2 day trip will be chaperoned by a Project SUCCESS Facilitator. Students will not be

allowed to leave the group or the hotel to wander on their own. They must have a chaperone accompany them.

4.

The purpose of this tour is to visit the different colleges and universities to get a feel for college life at the

schools. The 2 day itinerary is very full with activities at the colleges/universities and there will not be time to

visit with friends or relatives that may live in the area. It is important to note

that this trip is not a

“sight-seeing” expedition or a shopping trip. If those are your reasons for attending, this is not the right trip for you.

5. There is to be no smoking at any time on the trip.

Project SUCCESS

WAIVER AND RELEASE OF LIABILITY FOR MINORS

In consideration of being able to participate and travel with Project SUCCESS and _________________School, the undersigned:

1. Agree that the parent(s) and/or legal guardian(s) will instruct the minor participant that during the course of the trip, he or she should inspect all facilities and transports used, and if the participant believes anything is unsafe, he or she should immediately advise his or her supervisor of such condition(s).

2. Agree that the parent(s) and/or legal guardian(s) will instruct the minor participant that during the course of the trip, he or she is to obey all laws and rules of conduct, and to exercise ordinary caution in any and all activities during the course of the trip, and if the participant observes any conduct which he or she believes to be unlawful, unsafe, or in violation of any rules of conduct, he or she should immediately advise his or her supervisor of such conduct.

3. Acknowledge and fully understand that each participant will be traveling via commercial carrier and residing in temporary accommodations. Such travel and accommodations involves the risk of injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence, but also the actions, inactions, or negligence of others, or the condition of the facilities or mode of transport used. Further that there may be other risks not known to us or not reasonably foreseeable at this time.

4. In the event of any of the above events occurring the undersigned assumes all the foregoing risks and accepts personal responsibility for damages and releases, waives, discharges and covenants not to sue Project SUCCESS and ____________ School, their respective administrators, directors, agents and other employees of the organizations, other participants sponsor-ing agencies, sponsors, advertisers, and if applicable, owners and leasers of facilities and modes of transport used dursponsor-ing the course of the trip, all of which are hereinafter referred to as “releases”, form any and all liability to each of the undersigned, his or her heirs and next of kin, for any and all claims, demands, losses or damages on account of injury, including death or dam-age to property, caused or alleged to be caused in whole or in part by the negligence of the released persons or otherwise. 5. In the event of any litigation, the undersigned agrees to resolve all issues under the laws and rules of the State of Minnesota.
