alphabetically for each state, county, and city. The listing includes the dental provider’s

name, group practice, office location, phone number and provider number.

This directory is for Burlington, Camden, Cumberland, Gloucester, and Salem counties.

NOTE: If you are a member of an Employer Group or Union, our plan is available in the

state of New Jersey.

How to use this Provider Directory

You should use this provider directory to select a primary dental provider. Before

selecting your primary dental provider, check the provider’s office status. ‘Current

patients only’ means the provider’s office is accepting only existing patients. When no

status is listed, the provider is open to accepting both current and new patients. When you

have selected your primary dental provider, include the provider’s name, group practice

and provider number on your enrollment form.

The directory lists network dental providers as of the date that it is published. A

provider’s listing in this directory does not guarantee that the provider is still in the

network or accepting new members. For updated information regarding plan providers,

covered services from non-plan providers. Some covered services require “prior

authorization” by the plan in order to be covered. Some of the covered services listed in

the Evidence of Coverage book are covered only if your doctor or other plan provider

gets “prior authorization” (approval in advance) from our Plan. Covered services that

need prior authorization (approval in advance) are marked in the Benefits Chart in your

Evidence of Coverage booklet.
