USER MANUAL

for

Data Entry & Verification

About the Application:

Tracking of Pregnant mothers and children has been recognized as a priority area for providing effective healthcare services to this group. As a major initiative in this regard, the Mother and Child Tracking system (MCH) is name based pregnant mother and child tracking system. It is a management tool to reduce MMR/IMR/TFR and track the health service delivery at the individual level.

MCH is a generic system which aims to provide information of different health services received at the individual level, by monitoring all the encounters that an individual undergoes in his/her health program. This system aims to help the service provider (health worker or Doctor) by categorizing various health services the individual person has to get (with due date) and missed services. It also provides for effective monitoring of different health services drilling down to the individual patient information.

By clicking on “Login” you get the login screen:-

Choose the respective State and enter Username and Password and press login.

Note : The user ID and password will be provided to each state respectively. User id for district, blocks etc will be provided by the state administrator.

Main Menu has a Menu Bar with various options. Button Search by ID can be used if mother ID is available, to go directly to data entry screen without entering location details:-

The verify option on the main page is being used by the Verifiers at New Delhi. The states can send their List of verifiers (districtwise) which can then be added to the database and the verification can then be done at their end.

This option is used to verify the records for pregnant

women. Get record for verification shows the list of records that are yet to be verified. The records for pregnant women who have already delivered or their EDD has crossed appear in blue and yellow records signify that the call made was not answered. Show verified records show the list of records that have been already verified.

The record can be selected and then the call can be made. The status with remarks can be entered at the bottom of the page.

DATA ENTRY OF ANM AND ASHA

On clicking on the Data Entry option of the Main Menu screen, the following screen appears...

On this screen, enter the following details:- 1. Select District.

2. Choose either Health Block or Taluka/Tehsil/Sub District as per the state requirement.

3. Select Health Facility type.

4. Select Facility Name according to Facility type chosen in step 3.

5. Choose data entry option – Facility wise or Subcentre wise or Village wise as per the availability of data.

6. Select Sub Centre if data entry option is Subcentrewise or Village wise.

7. Select Village if data entry is Village wise.

8. Select option C. ANM entry of Data entry as shown in the screen below:-

Enter Name, Designation, Sex, Mobile No., Active, Joining Date of each ANM. Fields Name, Designation and Sex are mandatory. Click Save.

After data entry of ANMs, click on the Data Entry option of Main Menu displayed on the top of the screen.

For data entry of ASHAs choose option D. ASHA Entry as shown below:-

Enter Name, Sex, Mobile No., Active, Joining Date of each ASHA. Fields Name and Sex are mandatory. Click Save.

After data entry of ASHAs, click on the Data Entry option of Main Menu displayed on the top of the screen.

DATA ENTRY OF PREGNANT WOMAN

For data entry of Pregnant Woman choose option A. Pregnant Mother Entry as per NRHM format as shown below:-

Click Submit. The following screen

Enter data in the respective fields. On clicking button Save Details, computer generated 18 digit Mother ID will be displayed .

There are various validation checks on the data entry page like

Mobile numbers must be 10 or 11 digits.

LMP date must be in dd/mm/yyyy format.

LMP date should be greater than birth date.

LMP date must not be entered later than the financial year selected. E.g. if

financial year is 2011-2012 (01st April’2011 to 31st March’2012), then year of LMP

date must be between 2010 to 2012 (LMP date must not be later than 31st

March’2012).

If ASHA/ ANM exists, it must be selected.

If Linked facility for Delivery is selected, Name of linked facility must be filled.

First ANC date should not be less than LMP date.

Second ANC date must not be less than first ANC date. Similarly third ANC date

should not be less than second ANC date and fourth ANC date should not be less than third ANC date.

TT1 date should not be less than LMP.

TT2 date should not be less than TT1 date.

TTBooster date should not be less than TT2 date.

IFA date should not be less than LMP.

Delivery date should not be less than LMP date.

Discharge date should not be less than delivery date.

JSY date should not be less than delivery date.

All dates must be entered prior to the current date.

ANC1 should fall in the financial year selected. E.g. If the year selected is

2010-2011 then ANC1 date should be between 01st April’2010 to 31st March’2011

etc.

Age of the beneficiary must between 10 to 50 years.

If type of delivery selected is CS then place of delivery cannot be home.

While entering infant details, Weight of infant must be between 0 to 5 kg.

Note:- Please note down the Mother ID of each mother entered.

For entering a new mother press button New Entry.

For viewing all existing records, press button Search existing records.

The Records searched can be sorted out according to ID, Name or 1st ANC date.

To search a particular mother, press button Search. Search can be done on Mother Id or Mother Name.

For deleting any record, press button Mark for Deletion. (In this, select the checkbox of record to delete and give reason for deletion. Press Update Status button to save the changes.)

For entering other details of the Pregnant woman, click Select other details and choose the following options one by one:-

a. ANC Details

b. Pregnancy Outcome Details c. PNC Details

d. Infant Details

Enter data in the required options and click the corresponding Save button.

After data entry of Pregnant Woman, click on the Data Entry option of Main Menu displayed on the top of the screen.

DATA ENTRY OF CHILD

For data entry of Child, choose option B. Child Entry as per NRHM format as shown below:-

Click Submit. The following screen appears:-

For Sorting the records by Birthdate, ID, Name.

Enter data in the respective field. All mandatory fields are marked with *.On clicking button Save Details, computer generated 18 digit Child ID will be displayed.

Note:- Please note down the Child ID of each child entered.

There are various validation checks on the data entry page like

All dates in Child Immunization e.g. BCG date, DPT1, DPT2, DPT3,OPV0, OPV1, OPV2,

OPV3, Hepatitis-B1, Hepatitis B2, Hepatitis B3, Hepatitis B4 date, Measles date, DPT Booster date, OPV booster, MR vaccines, TT10, TT16, Vitamin A Dose-1 etc. should not be greater than date of birth.

For entering a new child, press button New Entry.

For viewing all existing records, press button Search existing records.

To search a particular child, press button Search. Search can be done on Child Id or Child Name.

For deleting any record, press button Mark for Deletion. (In this, select the checkbox of record to delete and give reason for deletion. Press update Status button to save the change.) For entering other details of the Child, click select other details and choose the option Immunization Details
