1) Previous Academic Degrees from Regionally Accredited program

 BA/BS or AA/AS

5 BS/BA or higher = 5 points AS/AA degree = 3 points

Two official transcripts from regionally accredited U.S. colleges or universities with degree posted. One unofficial transcript will be accepted for degrees posted from College of Sequoias. All transcripts from

outside the United States must be evaluated by an ECC-approved foreign evaluation service.

Licensed or certificated healthcare worker requires a copy of the current California certificate including name, certificate number, date of issue and date of expiration.

*If applying to the LVN-RN Bridge Program, 2 official transcripts from LVN program attended, in addition to LVN certificate will be required.

3) Recent work or volunteer

experience with direct patient care in relevant jobs (as listed above).

Original letter from current/former employer verifying employment.

The letter must be on organization letterhead with an original signature and must include:

Applicant’s name, Start date and end date (if applicable), Employee status (full-time/part-time), Number of hours worked per week (or total hours from/to date), Job title and Department ,

Examples of duties including patient interaction. 4) Grade Point Average in Relevant

Course Work: Minimum 2.5 GPA in Anatomy, Physiology, and

Student is allowed one repeat in each science prerequisite.

Two official transcripts (in original sealed/unopened envelope) for all lower and upper division courses completed at any and all regionally accredited U.S. institutions, regardless of applicability to nursing requirements will be required. One unofficial transcript will be accepted for courses completed at College of Sequoias.

All transcripts from outside the United States must be evaluated by a CC approved foreign evaluation service.

Courses completed at other regionally accredited United States colleges or universities must be equivalent to College of Sequoias courses. For determination of course equivalency, official

transcripts AND course descriptions for prerequisite courses, if necessary for COS evaluator, will be

All prerequisite courses must be completed with a grade of “C” or better and the final grade must be posted on the transcript. Only one repeat is allowed for each science prerequisite.

Failure to submit official transcripts with all grades posted will result in disqualification from the application process.

Only the first passing score will be accepted, only one repeat of TEAS will be allowed.

If TEAS taken at College of Sequoias, a printed copy of the TEAS report is required. If TEAS not taken at College of Sequoias, a printed copy of the report and an electronic transcript transfer receipt must be submitted. These can be purchased at
