We appreciate your interest in the General Surgery Residency program at Swedish Medical Center in Englewood, Colorado. We are a community-based Level I Trauma Center with specialized resources and expertise. The physicians who practice here address the most complex of traumas.

General surgery residents will learn from and train with board-certified critical care/trauma surgeons and general surgeons who practice in many diferent specialties and who provide the very best care in the region.

Our goal is to mold competent, well-trained general surgeons who can go into the community and practice immediately or who can pursue a fellowship for additional training.

Our hospital is well respected with exceptional patient outcomes. We have enjoyed tremendous growth and an increase in volume in recent years. We have over 22 ORs, interactive technology in 4 ORs, and a newly remodeled surgery waiting room.

Our residency program has its own dedicated residency workspace/didactics suite equipped with computers and technology. We have our own FLS and FES training equipment. We have a new residency simulation lab where our hands-on training equipment is located and used. We also have a large hospital conference center, a hospital library, and multiple call rooms.

We are an ACGME accredited residency program. We originally began our residency in August 2014 with American Osteopathic Association accreditation and then received our initial ACGME accreditation in fall 2017. We are now under ACGME continued accreditation without outcomes.

Our program and hospital are part of the HCA Healthcare. As part of a large healthcare system, we have an abundance of research resources and a specialized trauma research department in-house. We require each resident to produce one published research publication during our five-year residency program.

Through HCA Healthcare, we can ofer residents opportunities for careers and fellowship programs. We strongly recommend you consider our residency program for your surgical training.

Our Faculty

Kaysie Banton, MD

Kaysie Banton, MD, is the Trauma Services Medical Director at Swedish Medical Center. She began her role as both a trauma surgeon and medical director at Swedish in March 2018. Dr. Banton completed her medical degree at the University of New Mexico. She went on to complete her residency in general surgery at the University of Minnesota. She furthered her training with a fellowship in surgical infectious disease and a fellowship in surgical critical care at the University of Minnesota.

Daine Bennett, MD

Dr. Daine Bennett is board certified in general surgery and colon and rectal surgery. Dr. Bennett is an Arkansas native and attended the University of Arkansas--Fayetteville where he graduated Cum Laude with a bachelor of science. Dr. Bennett then attended University of Arkansas for Medical Sciences for medical school and then University of Colorado for general surgery residency where he also obtained a MS in Clinical Science. He then followed with a colon and rectal surgical fellowship at Baylor University Medical Center in Dallas, TX.

Douglas Benson, MD

Dr. Douglas Benson is board certified in general surgery and colon and rectal surgery. Dr. Benson attended the University of Minnesota- Morris and Macalester College where he graduated Magna Cum Laude with degrees in both chemistry and biology. Dr. Benson attended the University of Minnesota for medical school; he then moved to Aurora, CO, where he completed surgical residency and an oncology research fellowship at the University of Colorado Health Sciences Center. Dr.

Benson completed his colon and rectal surgery residency in Cleveland, Ohio at the Cleveland Clinic Foundation.

Kenneth Blake, MD

Dr. Ken Blake is a graduate of The Johns Hopkins University School of Medicine in Baltimore Maryland. He is a board certified Fellow of both the American College of Surgeons and the American Society of Colon

& Rectal Surgeons.

Andrea Boedecker, DO

Andrea Boedecker, DO, is a board certified general surgeon at North Denver Surgical Associates with a decade of experience in general surgery. Dr. Boedecker earned her medical degree from Kansas City University of Medicine & Biosciences College of Osteopathic Medicine in 2009. She then completed a residency program at St James Hospital

& Health Centers in Chicago, Illinois and an Advanced Gastrointestinal Surgery fellowship at LifeBridge Health in Baltimore, Maryland

Winston Chan, MD

Dr. Chan graduated from the University of Alberta, Faculty of

Medicine, in Edmonton, Alberta, Canada in 1988. He works in Spokane, Washington and specializes in pediatric surgery. Dr. Chan is afliated with Providence Holy Family Hospital, Providence Mount Carmel Hospital and Providence Sacred Heart Medical Center & Children’s Hospital. He holds an MBA. Dr Chan has served as the section chief of pediatric surgery at Sacred Heart Children’s Hospital since 2010.

Lily Daniali, MD

Dr Lily Daniali graduated medical school from the University of Washington School of Medicine. She completed her residency in plastic surgery at Rutgers Biomedical and Health Sciences School in Newark, New Jersey. She went on to complete a fellowship in hand and microsurgery at the University of Texas Southwestern and a craniofacial and pediatric plastic surgery fellowship at the Pediatric Plastic Surgery Institute.

Laura Hafertepen, DO

Laura Hafertepen, DO, is a breast surgeon practicing at Swedish and the Centrum Surgery Center in Colorado. She completed her degree is osteopathic medicine at the Western University of Health Sciences/

College of Osteopathic Medicine of the Pacific in Calfornia. Her general surgery residency was completed at the University of California San Francisco Fresno.

Thomas Hefron, MD

Dr. Thomas Hefron is board certified in general surgery with certification in solid organ transplant and hepatobiliary surgery. Dr.

Hefron was raised in Rapid City, South Dakota. Dr. Hefron attended Creighton University and graduated Magna cum laude with a degree in biology. Dr. Hefron then attended the University of Nebraska Medical Center for medical school and completed his general surgery residency at Creighton University Afliated Hospitals. Dr. Hefron then completed a fellowship in hepatobiliary surgery, renal. Pancreas and liver transplantation at the University Of Chicago Pritzker School Of Medicine.

Sarah Hill, MD

Dr. Sarah Hill is a general surgeon in Spokane, Washington and is afliated with Providence Sacred Heart Medical Center and Children’s Hospital. She received her medical degree from George Washington University School of Medicine and has been in practice between 6 and 10 years. She is one of 31 doctors at Providence Sacred Heart Medical Center and Children’s Hospital who specializes in general surgery.

She was a pediatric surgery clinical fellow at the Division of Pediatric Surgery, Emory University and Children’s Healthcare of Atlanta, in Georgia. She is board certified in general surgery and pediatric surgery.

Burt Katubig, MD

Burt B. Katubig, MD, is a trauma/critical care/acute care general surgeon based out of Swedish Medical Center since 1997. He

completed his general surgery and trauma service residency in Cook County Hospital/University in Illinois in 1996. He then completed a fellowship in trauma/critical care at St. Louis Hospital in 1997. He moved to Colorado afer gradating from St. Louis.

Bryan Kramer, MD

Dr. Kramer completed his undergraduate studies at Rice University and his medical degree at Albany College in 1995. He completed residency in general surgery at the University of Cincinnati in 2000 and a fellowship in Vascular Surgery at Albany Medical Center Hospital in 2002. Dr. Kramer is board-certified in both general and vascular surgery and has been practicing exclusively vascular surgery since 2000.

Ashwin Kurian, MD

Dr. Kurian completed his medical schooling in Bangalore Medical College, India in 2001. Afer this he completed a general surgery residency in the United Kingdom, where he is a Member of the Royal College of Surgeons of England. He moved to Philadelphia, USA to continue his general surgery training in Abington Memorial Hospital.

He graduated from his training program in 2011. He has completed a fellowship in minimally invasive surgery of the esophagus and stomach and advanced flexible endoscopy.

Luciano Lemos, MD

Dr. Luciano Lemos Filho is an internist in Denver, Colorado. He received his medical degree from Albert Einstein College of Medicine of Yeshiva University. He specializes in internal medicine, infectious disease and critical care medicine. He is afliated with National Jewish Health and Swedish Medical Center. He has been in practice between 11-20 years.

Thaddeus Liniewicz, DO

Thaddeus Liniewicz, DO is a graduate of Lake Erie College of

Osteopathic Medicine. He completed his undergraduate education in biology at the Master’s College in Santa Clara, California, graduating Summa Cum Laude. He is a native of Lake Forest, Illinois. In medical school he received an academic excellence scholarship from 2012-2015.

Dr. Liniewicz graduated from the Swedish Medical Center General Surgery Residency in June 2020. He is now a trauma/general surgery attending physician at Swedish Medical Center.

Kian Modanlou, MD

Dr. Modanlou is a native of Southern California. He completed his undergraduate studies at Duke University where he graduated with distinction with a degree in biology. He returned to his home state for medical school and graduated from the University of California, Davis, School of Medicine. Dr. Modanlou then moved to Denver where he completed his residency in general surgery at the University of Colorado Health Sciences Center. He then went on to Chicago where he completed a fellowship in Abdominal Transplant and Hepatobiliary Surgery at Northwestern University.

Andrew Nemechek, MD

Dr. Nemecheck is board-certified in Otolaryngology-Head and Neck Surgery and serves as the Medical Director of Head and Neck Oncology at Porter Adventist Hospital. He is fellowship-trained in advanced head and neck oncologic surgery. His other clinical interests include advanced cutaneous malignancies and reconstruction, HPV- related head and neck cancers, and tumors of the skull base and paranasal sinuses. He also commonly treats tumors of the thyroid and parathyroid glands as well as salivary neoplasms.

Benson Pulikkottil, MD

Benson Pulikkottil, MD, has been in practice at the Swedish Medical Center Burn and Reconstructive Centers of Colorado since September 2016. He attained his medical degree at Albany Medical College in 2001.

He went to residency at Rutgers New Jersey Medical School in general surgery. He completed a research fellowship in the Division of Plastic and Reconstructive Surgery at the University of Pittsburgh Medical Center. He also completed two additional fellowships: one in plastic surgery at the University of Texas Southwestern and one in orthopedic hand surgery at the University of Pittsburgh Medical Center in 2016.

Glenda Quan, MD

Dr. Glenda Quan was raised in California. She attended the University of California at Berkeley, majoring in biology and English; she attended the University of California at Davis for medical school. She moved to Portland, Oregon, for surgical residency. Dr. Quan is board certified in general surgery and neurocritical care. She lives in Greenwood Village with her husband and two daughters. She is the Associate Program Director for the Swedish General Surgery Residency.

Cassandra Reynolds, MD

Cassandra Reynolds, MD, FACS is a Trauma/ Critical Care Surgeon at Swedish Medical Center in Denver, Colorado. Dr. Reynolds completed her Trauma Critical Care/ Fellowship at University of California Davis in Sacramento. She joined the team at Swedish afer having worked at East Carolina University in Greenville, NC a busy level 1 academic trauma center for the last three years.

Paul Thorne, MD

Dr. Paul Thorne is a general surgeon in Spokane, Washington and is afliated with Providence Sacred Heart Medical Center and Children’s Hospital as an attending pediatric surgeon. He received his medical degree from Dalhousie University Faculty of Medicine and has been in practice for more than 20 years. He is one of 31 doctors at Providence Sacred Heart Medical Center and Children’s Hospital who specializes in general surgery.

John Tillett, MD

John W. Tillett, MD FACS, is a board-certified urologist with the Urology Center of Colorado on staf at Swedish Medical Center and Craig Hospital. He graduated magna cum laude from Vanderbilt University. He received his medical degree from Emory University School of Medicine, where he also completed his post-graduate urology training. He is a Clinical Assistant Professor at the University of Colorado School of Medicine, and he was also

previously on the medical school faculty at Florida State University and the Uniformed Services University of the Health Sciences

Richard Tillquist, MD

Dr. Richard Tillquist is a general surgeon in Englewood, Colorado and is afliated with multiple hospitals in the metro area, including Swedish Medical Center. He received his medical degree from Baylor College of Medicine and he completed his residency at Baylor University Medical Center. He has been in practice for more than 33 years. He specializes in bariatric surgery and general surgery at SurgOne, PC.

Bruce Weber, MD

Bruce Weber, DO, is an internal medicine specialist who practices at North Suburban Medical Center. He has been in practice for 42 years.

He is an endoscopist.

Adam Weingart, MD

Adam Weingart, MD, is an anesthesiologist with USAP South Denver Anesthesiologists. He holds many committee positions on various boards and committees in the community. He received his medical degree from the University of Colorado in Denver. He completed his internship year of residency at Exempla St. Joseph’s Hospital in Denver. Then he completed his residency in anesthesiology at Northwestern Memorial Hospital. He furthered his training with an adult and pediatric regional and acute pain fellowship at Northwestern Memorial Hosptal

Ahmed Zihni, MD

Ahmed M Zihni, MD, MPH is a board-certified general and laparoscopic surgeon and works as a foregut surgeon for Denver Esophageal and Stomach Center in Denver, CO. He received his Bachelor of Science (Summa Cum Laude) from SUNY Stony Brook University in Stony Brook, NY and his Masters of Public Health (Distinction) and Medical Degree from SUNY Downstate College of Medicine in Brooklyn, NY.

Dr. Zihni completed residency at Washington University in St. Louis, MO, followed by a fellowship in Foregut Surgery (Gastrointestinal and Minimally Invasive Surgery) at the Oregon Clinic in Portland, Oregon.

About the Program

Swedish Medical Center, part of HCA Healthcare’s HealthONE, is located in the south metro Denver area where it has been a proud member of the community for more than 110 years.

An acute care hospital with 408 licensed beds, Swedish Medical Center (SMC) cares for more than 200,000 patients annually with a team of approximately 2,000 dedicated employees, 300 volunteers and 1,400 physicians.

The Swedish Medical Center General Surgery Residency Program’s mission is to develop the next generation of physicians and physician leaders. As a part of HCA Healthcare, we are driven by a single mission: Above all else, we are committed to the care and improvement of human life.

This program is designed to ofer residents a rewarding, educational environment where residents are provided individualized learning opportunities by faculty and staf who support and sustain one another throughout quality care delivery. Our faculty are committed to ensuring the residents’ clinical experience and educational needs are fulfilled while expanding residents’ perspectives to be inclusive of cultures, values, and ideals.

It’s important to note that 57% of our graduates have gone on to successful fellowships post- graduation from our surgical residency program to the following institutions for further sub- specialty experience: Duke, Swedish Medical Center, Jacobi/Montefiore Surgical Critical Care Fellowship, and East Carolina University. The other 43% are gainfully employed in private practice settings in general surgery.

Do you accept international medical school graduates?

International medical school graduates must be certified by the Educational Commission for Foreign Medical Graduates (ECFMG) at the time of application.

Do you require clinical experience for international medical school graduates?

No, US clinical experience is not required but it is strongly preferred. We do not consider observerships, externships and/or preceptorships to be clinical experience, nor do we ofer those opportunities.

Do you sponsor VISAs?

No, ECFMG is the sole sponsor of visas for physicians in clinical training programs.

How long can I be out of medical school and still apply for your program?

Applicants must be recent (2 years or less) graduates from medical school to be considered competitive.

Where can I find support in applying to the Electronic Residency Application Service (ERAS)?

The ECFMG ofers a service called ERAS Support Services to anyone who is ECFGME certified. The ERAS Support Services will issue the ERAS token, receive the applicant’s supporting documents, and transmit them to the ERAS Post Ofce, and provide client support between the various organizations. Eligibility and Selection of Residents and Fellows

Residents and fellows in ACGME-accredited programs at the hospital must be selected based on qualifications that meet or exceed the standards below.

1. Graduates of medical schools in the U.S. and Canada accredited by the Liaison Committee for Medical Education (LCME); or,

2.Graduates of osteopathic medicine in the U.S. accredited by the Commission on Osteopathic College Accreditation (COCA); or,

3. Graduates of medical schools outside the U.S. or Canada who have received a currently valid certificate from the Educational Commission for Foreign Medical

4.Graduates (ECFMG); or, Graduates of medical schools outside the U.S. who have completed a Fifh Pathway program provided by an LCME-accredited medical school.

5.Applicants must be recent (two or fewer years) graduates from medical school to be considered. Special exceptions may be considered for:

Candidates with an M.D. or D.O. with a M.P.H./M.S./Ph.D. and extensive prior research experience afer completion of an LCME or a COCAaccredited medical or osteopathic medical school in the U.S. or Canada

Candidates who have served a prolonged period as a general medical ofcer in the U.S.

military

Candidates who have successfully completed an ACGME or AOA-accredited residency/

fellowship program in the U.S. or Canada.

6.All requisite prior training must be successfully completed prior to beginning any residency or fellowship program.

7. Applicants must have passed USMLE (United States Medical Licensing Examination) Step 1 and have taken both components of Step 2 or the COMLEX (Comprehensive Osteopathic Medical Licensing Examination) Level 1 and have taken both components of Level 2.

Programs will select candidates to interview only from among the pool of eligible

applicants,evaluating each applicant on the basis of their preparedness, ability, aptitude, academic background (to include clerkship grades, standardized test scores, communication skills, and humanistic qualities such as motivation, honesty, and integrity).

All GME training programs are required to use the Electronic Residency Application Service (ERAS) to receive and accept applications to the program. All programs at the hospital will participate in the National Resident Matching Program (NRMP) as the primary method of recruiting trainees. All applicants who are granted interviews must interview prior to the initial match process. Exceptions may be made for fellowship or other programs not going through ERAS.

For residents or fellows attempting to transfer, the residency/fellowship program must first seek permission from the HCA Healthcare GME division vice president to consider the transfer. If permission is granted, then the receiving program must receive verification of each applicant’s level of competency in the required clinical field using ACGME or CanMEDS Milestones assessments from the prior training program.

A physician who has completed a residency/fellowship program that was not accredited by ACGME, RCPSC, or CFPC may enter an ACGME-accredited residency/fellowship program in the same specialty at the PGY-1 level and, at the discretion of the program director at the ACGME-accredited program, may be advanced to the PGY-2 level based on ACGME Milestones assessments at the ACGME-accredited program. This provision applies only to entry into residency/fellowship in those specialties for which an initial clinical year is not required for entry.

Applicants invited to interview for a resident/fellow position must be informed, in writing or byelectronic means, of the terms, conditions, and benefits of their appointment to the ACGMEaccredited program, as well as all institutional and program policies regarding eligibility and selection for appointment, either in efect at the time of the interview or that will be in efect at the time of their eventual appointment. This includes financial support; vacations; parental, sick, and other leaves of absence; and professional liability, hospitalization, health, disability and other insurance accessible to residents/fellows and their eligible dependents.

All terms, conditions, and benefits of the potential appointment are described in the GME resident/fellow contract.
