MODULE AIM(S)
By undertaking this module you will be able to critically analyse some of the key features of chronic disease management in the context of policy, frameworks and the underpinning research base. You will use a range of information to support and disseminate evidence-based approaches to decision-making and care co-ordination. You will explore and evaluate factors that influence case management to enable you to provide evidence informed choice for clients and contribute to the development of self and others. You will critically evaluate the range of factors that influence case management such as; the physical, psychological and social impact of chronic illness; interagency and
interdisciplinary collaboration; your role in case management and the inherent ethical and legal implications.
MODULE LEARNING OUTCOMES
By engaging successfully with this module a student will be able to
1. select and critically examine and apply relevant social, psychological and biological theory to inform practice which enhances health and to develop a reasoned and informed approach to the planning, management and
evaluation of care (2.1.7)
2. collate and critically appraise reliable and valid information from a variety of sources to respond creatively to the wide ranging needs of patients who require district nursing care, case management or complex care co-ordination (2.1.1; 2.1.5 and 2.3.1)
3. apply critical thinking, complex problem-solving and an evidence based approach to professional practice, particularly in relation to complex, chronic or long-term disease management formulating new insights(2.2.4; 2.3.2; 2.4.4)
4. critically examine and analyse research literature, policy and guidance to disseminate ‘best practice’ and contribute to the development, knowledge and practice of others (2.1.4; 2.3.7 and 2.4.2)
5. critically examine and analyse the concepts of self management of health and illness, particularly in clients with limited self management potential and the implications for your own and others practice (2.3.2; 2.3.4; 2.3.8)

• Overview of common chronic or Long Term Conditions; Pathophysiology, intervention, prevention, health promotion and care management

• Risk management, ethical, legal and professional issues for case management • Critical consideration of evidence based interventions and approaches to care
management for individuals with long-term conditions.
• The principles of, policies related to and evidence base for, patient centred approaches to care and promoting self management
• The concepts of self-care, promoting independence, empowerment; success factors, vulnerability and limitations for patients.
• Inter-professional, interagency, multi-agency and multi-disciplinary working • Critical analysis of literature and policy to inform clinical decision-making and

You will be supported in your learning, to achieve the above outcomes, in the following ways;
The variety of teaching and learning strategies will include:
Lectures, to introduce the module, key concepts and topics of the module, including case management; self care; inter-professional and interagency working; shared decision making, and introduction to the assessment task.
Student directed study will allow you to work on your own personal exploration of issues relevant to the module and to undertake extra work in areas that you find challenging. It will be used to prepare for and follow-up from lectures and for the preparation of your summative assessment.
Tutorials by personal and electronic contact to facilitate critical assessment and reflection of your skills in relation to the learning outcomes of the module, and, to provide support for the completion of the assessed work
Directed learning opportunities through use of electronically delivered learning materials and reading lists to guide you to appropriate research and evidence and to facilitate a critical exploration of the key areas of the module’s curriculum.
Use of mentors to facilitate learning from practice
The module Blackboard site will provide online support of your learning including supplementary information, links to internet based resources and Blackboard message boards which will run across the duration of the course, allowing you to give and gain peer support and prepare for face-to-face sessions.
ASSESSMENT STRATEGY AND METHODS
You will undertake a literature review (2000 words) related to an identified long-term, chronic or palliative care condition to establish and disseminate an evidence base for practice. The literature review should be used to underpin the development of a teaching or learning resource, suitable for client / carer or staff education. The
resource pack may be designed as a teaching aid, a self-directed resource, or a tool to aid patient-informed choice or a combination of these. The teaching / learning resource needs to include a rationale (1000 words) based on the critical analysis of current practice, the policy context and range of information.

ASSESSMENT CRITERIA
To achieve a pass you must demonstrate that you have met all the module-learning outcomes. This includes:
1. Evidence of learning in relation to each learning outcome; this needs to be contained within the written work that demonstrates:
• an analysis of some of the key features of case management and care coordination (LO1)
• the appraisal and collation of a range of information to support evidence-based approaches to decision-making and care co-ordination in a format suitable for dissemination (LO2)
• the ability to justify decision making for complex, chronic or long-term disease management (LO3)
• the development of a teaching / learning resource that is informed by a critical
approach to the evaluation of the research literature, policy and guidance and that is presented in an appropriate format for the intended audience (LO4)
• the analysis of the concept of self-management of health and illness and its
application to chronic disease and case management and its application to chronic disease and case management within ethical, legal and professional frameworks.. (LO5)
2. The written part of the assignment should be understandable with generally accurate spelling, grammar, punctuation and references.

You will receive feedback on your performance in the following ways
You will receive continuous assessment for learning in various formats from tutors throughout the duration of the module. The feedback that you receive on the results of your work provides a further learning opportunity. You will receive comments on assessment Task to encourage self- reflection and feed forward to your next
academic work. The comments you receive may be verbal, via audio feedback files, in an audio-visual format or in writing. The feedback you receive will be constructive, timely and aimed at further developing your learning. The assessment task is an essay enabling you to discuss and evaluate theory and practice. The feedback will normally be provided within 4 weeks of the submission date. In addition to the initial feedback, further opportunity to discuss the assessment and the feedback is
provided as requested in one to one tutorials.
The feedback will:
provide information about your performance in relation to each assessment criterion
indicate what you may need to do to address particular issues encourage dialogue and collaboration to enhance your learning
assist in the development of self-assessment and reflection in learning
provide information that can be used to help shape your further academic work aim to be encouraging and motivating and provide opportunities to close the gap
