LARRY R. DUST

Larry Dust is the Chief Executive Officer of the Key Family of Companies. Larry has substantial background in the insurance

industry. He was a brokerage supervisor for Aetna Life and Casualty until he co-founded Life Insurance Brokerage Agency, Inc., an independent life insurance agency, in 1973. In 1976 he co-founded Key Life Insurance, and then established Key Benefit Administrators, Inc. in 1979. Key Benefit Administrators was acquired by Blue Cross/Blue Shield of Indiana in 1985, and Larry became one of the executive management staff of the Associated Group, the parent of Blue Cross/Blue Shield. He was involved with the founding of Anthem during his tenure with the Associated Group.

In 1992 he repurchased KBA and has built that business into a leading benefits administrator in the United States. In 1998 he founded the American Health Data Institute (AHDI) and has subsequently been granted two U.S. Patents concerning population management. Since 2000 AHDI has statistically-proven results of improved participant health, lower healthcare costs, and higher quality of care using these patented processes. A native of Indiana, Larry is a graduate of Indiana University.

WALLACE T. GRAY

Wally is Vice President and General Counsel for the Key Family of Companies. A graduate of Indiana University with a

degree in political science, Wally holds a JD from the University of Louisville School of Law and an MBA from the University of Indianapolis. His specialty areas are ERISA and employment law. Wally oversees all aspects of compliance related to group benefits including Department of Labor regulations. Wally joined the Key Family of Companies in 1988.

BRADLEY P. RAY

Brad is Vice President and Chief Financial Officer for the Key Family of Companies. A Certified Public Accountant, Brad

holds a BS in Accounting from Indiana University and has over 32 years experience in life insurance and benefit accounting. He currently serves as CFO for many of the Key Companies. Brad joined the Key Family in June 1979.

TOM REMLINGER

Tom Remlinger is Chief Operating Officer for the Key Family of Companies. He began his insurance industry career in 1985

as a CPA for Allstate Insurance Company, performing financial and operational audits for life, property casualty, and health insurance operations throughout the U.S. In 1990 he became Audit Manager at Washington National Insurance Company. While at Washington National between 1990 and 1996, he acted in various corporate and field capacities. In 1996 he helped found Millennium Underwriting, Inc., a Managing General Underwriter that underwrites, issues, and administers employer stop-loss policies. In April 2002 he became Vice President of Claims for the Key Family of Companies, and in 2011 he took on the role of Chief Operating Officer for Key Family.

DAVID B. COOK, M.D., FACP, FCCP

Dr. Cook is Medical Director for Key Family and the President of the American Health Data Institute (AHDI), a subsidiary

of the Key Family of Companies. A seasoned physician executive with experience managing a physician group practice, a

major clinical department in one of the nation’s largest private hospitals, and the quality management system of a developing integrated delivery and financing system, David is a critical resource for dealing with issues requiring physician consultation or in-depth medical evaluation. A graduate of Wabash College and Indiana University, Dr. Cook is board Certified in Medical

Management, Pulmonary Disease and Internal Medicine; he is a Fellow of the American College of Chest Physicians and the American College of Physicians; and he is a Certified Quality Manager by the American Society of Quality Control. Dr. Cook joined the Key Family in April 1998.

NANCY C. BLOUGH, JD, MBA

Nancy is the Executive Vice President of the American Health Data Institute, a company owned by the Key Family of

Companies. Nancy has been with the Key Family of Companies since 1998 in the roles of Chief Operating Officer and President

of the Southeast Office TPA. Prior to joining the Key Family of Companies, Nancy was the Director of Strategy and Design, Healthcare Management for Ford Motor Company in Dearborn, Michigan. Her responsibilities included group medical, workers compensation, and disability benefits for active and retired employees of Ford Motor Company. Nancy was also on the Board of Directors of Blue Cross and Blue Shield of Michigan. Prior to joining Ford Motor Company, Nancy was the Deputy State Health Commissioner for the Indiana State Department of Health. Ms. Blough received her MBA, Doctorate of Jurisprudence, and undergraduate degree from Indiana University. Nancy is an adjunct professor in the Belk School of Business at the University of North Carolina Charlotte as well as a member of the Kelly School of Business MBA Alumni Advisory Board. She is licensed to practice law, is a certified mediator and certified coach.

JOHN O.H. TOLEDANO, JR

John is Senior Vice President of the American Health Data Institute, a subsidiary of the Key Family of Companies. John

joined Key Family in 2007 and has held various positions including Vice President of Acme-McCrary Corporation in Asheboro, North Carolina. He was Director of Human Resources for both Acme-McCrary and Sapona Manufacturing Company. John was also an Officer and is a Board Member of Acme-McCrary Corporation, Sapona Manufacturing Company, and Randolph Oil Company. John was a Board Member of Randolph Hospital, is an active member of his community, and is a past President of the Piedmont Chapter of the National Association of Hosiery Manufacturers.

TOM SATARINO, SPHR

Tom, President of the American Health Data Institute, joined KBA in 2001 as Chief Human Resources Officer for Key

Family, and in 2011 he became COO of Pinpoint Wellness Systems. He began his insurance career in 1981 and has held

various human resources and operations positions with national property and casualty and health companies based in Texas, New Jersey, Ohio, and Indiana. He is a member of the Board of Operations for Key Family. Tom graduated from the University of North Texas with majors in Organizational Behavior and Operations Management, with continuing education through the IBM Executive School and Stanford University.

Joe Harvey

Joe is the President of Key Partners, Inc. (KPI). He is a graduate of The Ohio State University with a Bachelor of Science (BS) in Administrative Sciences and holds a Master of Business Administration (MBA) from Capital University. Joe is also a Health Insurance Associate – a designation obtained through the Health Insurance Association of America. Although Joe has been a member of the Key Family in the past as Director, Network Development with KPI from 1998 – 2000, he has recently rejoined the Key Family of Companies in his present capacity in May 2014. Throughout his career Joe has held key management roles with leading companies in the managed care industry including Anthem Health Systems, UnitedHealthcare and Unicare – a Wellpoint affiliate. Joe brings significant experience in the areas of managed care contracting, strategic planning, new business

development, market expansion and network administration and has over 30 years of experience in various aspects of health care management.

ANITA DUST, CLU, FLMI

Anita is Vice President of Recovery for the Key Family. Anita started her career with Jefferson National Life Insurance Company, working there in various departments for ten years before joining Key Benefit Administrators in 1982. During her tenure at Key Family of Companies, Anita has been involved in the operations of nearly every department including Claims, Customer Service, Billing and Eligibility, and Client Relations. She has served as the Vice-President of Claims, President of Cost-Pro, and President of Key Benefit Administrators, our Indianapolis, Indiana, TPA. Anita attended Indiana University in Indianapolis and also holds designations of CLU and FLMI.

TIFFANY ALSABROOK

Tiffany is the Vice President of Customer Care and Flexible Benefits for the Key Family of Companies. In this capacity,

Tiffany oversees both customer service and claims processing for both fully-insured and self-funded health plans and she oversees all operations of the Flexible Benefit department (i.e., FSA, HRA, and Section 125). Tiffany joined Key Family in 2000 after spending seven years in the banking sector. During her tenure at Key Family, Tiffany has had the responsibility of re-structuring and managing the Operational Support, Statistical Analysis, and Data Entry Departments. Tiffany was responsible for the project plan and implementation of EDI, OCR, and claim automation functions. Tiffany attended Ball State University.

PATRICK COHEN

A graduate of the Indiana University Kelly School of Business, Patrick joined the Key Family of Companies in 2004 and now serves as our Vice President of Operational Accounting. He has oversight responsibility for the day-to-day operations of billing and eligibility, cash receipts, cash application, commission payment, carrier/vendor reporting, and COBRA administration. Upon joining the Key Family, Patrick held various positions within the organization including Vice President of Product Relationships, where he was responsible for the management of several Key Family of Companies partnerships. Prior to joining the company, Patrick held key project management roles with a leading technology company based in Indianapolis.

JESSICA DUST

Jessica is a graduate of Indiana University, Bloomington. Having grown up in Indianapolis, she moved to the southeast after graduation and calls Charlotte home. Jessica joined Key Benefit Administrators as an Account Executive and has held multiple positions in many departments within the Key Family of Companies. In 2012 Jessica began serving as the Vice President of

Marketing and Reinsurance Services. She oversees the departments of Marketing, Self-funded New and Renewal Business,

and Reinsurance Recovery. Jessica has received her Competent Communicator and Competent Leader designations from the International Toastmasters Club.

MAUREEN HUNTON

Maureen, Vice President over social media and corporate communications, graduated from Indiana University with BS

and MS degrees and holds a Life and Health License in the State of Indiana. Before joining the Key Family, Maureen taught English at the secondary level and writing at Indiana University. She joined the Key Family in 1987 and has held positions as a Client Vice President consulting with clients on their benefit plans; as Vice President of Operational Support Services over training, auditing, and plan building; and as Vice President of Administration and Service for a block of KBA’s small self-funded groups. Also during her tenure with the Key Family, she ran one of our satellite offices, overseeing the claims, customer service, and billing and eligibility personnel and maintaining that office’s budget and facility.

MIKE GOODMAN

Mike joined KBA IN 1997 after a fifteen-year career with Duke Energy in Charlotte, NC. While at Duke, Mike was responsible for the welfare benefit plans and the benefits budget for the corporation. Mike began his KBA career sixteen years ago as a Client Services Vice President. He also spent several years as the Director of Human Resources in KBA’s southeast office, which was in Charlotte, NC, at that time. Because of Mike’s benefits experience, analytical skills, and customer service strengths, Mike has been awarded management of our largest, most prestigious accounts. Mike graduated from Wake Forest University and holds life and health licenses in several states.

JAMIE LIGHT

Jamie is a Client Vice President on KBA’s Client Relations team. She attended the University of South Florida where she

majored in finance and has been a licensed insurance agent since 2004. Previously, Jamie worked for five years at First Benefits Group as an office manager, benefit specialist, and agent. She has been part of the Key Family since 2010 where she began as a benefit consultant. She has been in her executive role since 2012.

KRIS CARDELL

Kris is Vice President of Professional Cost Management for the Key Family of Companies. Kris began her career in the

insurance industry in 1991 and has extensive background in both marketing and operations. Prior to joining the Key Family, Kris worked with a national managed care company where she was responsible for sales, service, and various operational areas. Currently Kris is responsible for sales, service, and overall operations of the fully-insured portfolio of products underwritten by Fidelity Security Life Insurance Company.

DALE KALISER

Dale is Vice President of Special Insurance products with primary emphasis on the overall operations of the Fidelity Security Life Insurance Company fully-insured portfolio of products. Prior to this role, Dale was Vice President of Client Services managing all of KBA’s consulting staff. Dale has been in the insurance industry for over 35 years. Dale began his career with John Hancock Insurance Company holding several regional management positions in Atlanta, Georgia, and Dallas, Texas, responsible for the southern and southwest Claim Operations. Prior to joining KBA, Dale was the Director of Operations for United Healthcare’s southeast office. Dale is a graduate of Georgia State University where he majored in Political Science. Dale has been with the Key Family since June 1999.

CRAIG FRAZIER

Craig, Director of Human Resources since 2013, joined Key Family in 2003 as an IT Business Analyst and has held various

operational management positions within the company prior to his current position. He began his career in 1995 and has specific experience in personnel management and operations with companies in Colorado, Tennessee, and North

Carolina. Craig is a member of the Board of Management and the Board of Operations for Key Family. Craig graduated from Lee University with a major in Business Administration which included completion of an abroad program in International Business.

DON JAMISON

Don, Corporate Controller of Finance, joined the Key Family in 1995. He has had several positions within the Finance department before becoming the Corporate Controller. He graduated from Ball State University with a BS and received an MBA from Anderson University. Don oversees the monthly and annual budget process, financial statement reporting, accounts payable, accounts receivable, payroll, bank reconciliations as well as several monthly analysis reports.

EUGENIA MITCHELL

Eugenia is KBA’s Claims and Mailroom Manager. See started at KBA in 2001 as a claims administrator. Before

undertaking her current management role, she was an operational trainer of claims and customer service. Eugenia graduated from York Comprehensive High school and also attended York Technical College. She has been in the insurance industry for 26 years, beginning her career with Mutual of Omaha. She has also worked for Cigna and United Healthcare.

LISA ABERNATHY

Lisa is KBA’s manager Customer Care on all lines of business. She began her career at KBA in 2007 filling the role of Customer Service Manager, overseeing the customer service team for most products administered by Key Benefit Administrators. Lisa also managed the claims and customer service functions for the FlexPro department from 2009 to 2013. Prior to joining the Key Family of Companies, Lisa held the positions of claims manager, customer service manager, and performance improvement manager at CIGNA Healthcare.

MICAH VELASQUEZ

Micah is the Billing, Eligibility and COBRA manager over KBA’s Self Funded business. He began his career in the

Healthcare industry with Cigna Healthcare in 2002 as a customer service representative. He quickly advance to handling dedicated accounts after one year in the role and was added to the Training and Technical support staff a year after that. This allowed him to earn his Six Sigma Yellow belt and become a Certified Technical Trainer (CTT+) which added to his Associates Degree in Business. He joined KBA in 2007 as a customer service manager which allowed him to transition into his current role with KBA in 2009.

GRETCHEN WILKENING

Gretchen, Operational Accounting Manager since 2010, joined Key Family in 2008 as a Senior Project Manager and has

held various operational and client management positions within the company prior to her current position. She began her career in 1999 and has specific experience in training, project management, relationship management, business analysis, and operations with digital media and e-commerce companies in Indiana, New York, and South Carolina. Gretchen graduated from Pensacola Christian College with a double major in History and English Education.

DEB GOWEN

Deb joined the Key Family of Companies in 2012 as a Benefit Consultant. With 30 years of experience as a professional

in the benefits industry, prior to joining KBA, she owned a consulting firm working exclusively with self-funded employers. As a member of the client relations team, Deb provides clients with an analysis of their current and future healthcare risks, providing programs designed to improve the health of their members, and reduce plan costs. Deb has been a licensed agent since 1982. She studied at Western Michigan University where she majored in Marketing.
