iSOFT Incident

Management (AIMS)

Providing actionable insights

for quality improvement

(2)

iSOFT’s Advanced Incident Management

System (AIMS) solution reports, analyses and

manages adverse healthcare events to improve

delivery of services and ultimately improve the

quality of care.

(3)

Added to the considerable human suffering is the economic impact of adverse effects. Several studies have shown that for some countries, this can cost between AUD$6 billion and AUD$29 billion a year.

Based on 20 years of research and use in over 1,000 facilities across the globe, AIMS is an interactive evidence-based management tool providing detailed information for devising interventions and solutions to healthcare delivery problems.

World-renowned classification

AIMS captures adverse event and near miss information across acute care, community care, disability care, mental health and residential aged care (nursing homes). The solution is recognised by WHO and the US Institute of Medicine for its comprehensive and standardised classification of the ‘how’ and ‘why’ of incidents and its ability to facilitate benchmarking and detailed analysis. Fully web-based for rapid and low cost deployment across large and small

organisations, AIMS is backed by sophisticated security and administrative controls for reliable and effective system management.

AIMS is available as either an on-demand service or for on-premise installation within a customer’s data centre. It can be easily integrated into clients’ patient and staff information systems to avoid double entry and improve data reliability. Clients can either define their own data capture forms or choose standard data capture forms for a simple and standardised approach.

A single point of entry for multiple data sources ensures efficiency and a complete picture of all clinical incidents, occupational health and safety, complaints / feedback, coroner reports and risks. Alert functionality ensures that staff are informed of serious events or simply notified of tasks they are required to complete following an event. Business intelligence and decision support tools also enable a strategic approach to patient safety and quality improvement.

Providing actionable insights for quality improvement

The World Health Organization (WHO) estimates that tens of millions of

patients worldwide suffer disabling injuries or death every year due to

unsafe medical practices and care. Nearly one in ten patients is harmed while

receiving healthcare in well-funded and technologically advanced hospital

settings. Half of all serious incidents are considered preventable.

(4)

Benefits

• Provides actionable, data-driven intelligence about safety and quality

problems and best practices

• Empowers managers to make strategic improvements by providing an

interactive evidence base

• Illuminates problem areas to enable improvement focus and tracking

of results over time

• Adaptable to your unique incident and risk management practices

Incident Capture & Management

Flexibility combined with standardisation

Choose one of our best practice data capture forms for a simple and standardised approach, or work with our professional services to develop a fully customized form to match your business processes. Capture of incident detail is standardised via the world-renowned AIMS classification.

Control the level of detail collected by defining a per incident type minimum dataset for classification. The maximum dataset (standard AIMS classification) is generally reserved for serious or targeted incidents. It is also useful for recording the detailed findings of Root Cause Analyses (RCA).

Classification can be performed by line managers or on a centralised basis according to incident severity.

AIMS supports a variety of data capture methods to suit the needs of different reporters.

Views, Analysis & Reporting

Powerful reporting to suit a variety of users

The analysis and reporting capability in AIMS version 4 has been significantly improved to make it easier and more useful for a wider group of users. Business Intelligence and decision support tools enable a strategic approach to patient safety and quality improvement while simple yet powerful data views help managers to understand what is happening and take action.

• Flexible design options incorporating views, filters and sorting on reports

• Ability to define and save data views at a user level for quick, consistent and repeatable access to key data

• Hyperlink drill down from real time summary data views to underlying detailed information

• Ability to filter and sort information within data views • Easy to design and run standard reports with parameter

selections

• Report scheduling and secure distribution through user subscription

• Integrated Business Intelligence (BI) and data visualisation tool to view, compare and analyse data

• Includes a dynamic data warehouse to enable near-time data access without disrupting system performance

(6)

Three Levels of Analysis & Reporting

• Level 1 - Basic

Registered users such as line managers have access to simple text-based reporting for incident management on a local level, for example by department or by incident type such as warfarin prescribing error. These users can also be provided access to simple, web-based charting to enable exploration of data based on user-defined or distributed data summary views.

• Level 2 - Intermediate

For Patient Safety / Risk Managers, AIMS offers the same charting tools as above plus an additional level of query and analysis for the more sophisticated user with a sound understanding of the data structure, nature and resultant implications.

• Level 3 - Advanced

To meet the demands of analyst users, AIMS provides advanced web-based analysis and reporting for data mining. Using an integrated Business Intelligence (BI) and Data Visualisation tool via the AIMS Console, analysts can interrogate the AIMS dynamic data warehouse in near-time without disruption to system performance. The data warehouse can also be examined using third party BI applications such as Cognos and Business Objects.

This summary view shows incidents by incident type

and filtered for the last calendar year.

The AIMS Analysis module is a fully integrated

business intelligence and data visualisation tool to

view, compare and analyse AIMS data.

Powering healthcare transformation

Easy to create and manage, AIMS automated email-based Alerts inform relevant staff instantly. Email Alert templates are client defined so that recipients receive pertinent information and a link to the issue at hand. In this way, system security is not compromised as only authenticated users with the correct permissions will have access to the issue report.

AIMS On-Demand

Reduced risk & faster returns

AIMS version 4 is fully web-based and is therefore available as a service (SaaS) or for on-premise installation within a customer’s data centre. Either way, users access the application using a standard web browser - there is no need to install any software on local PCs. Benefits of accessing AIMS as a service (SaaS):

Reduced IT staffing cost and expertise as •

iSOFT takes care of all software updates and maintenance

No investment in server hardware or •

software as this is all included in the AIMS SaaS fee

All upgrades are included, even major •

upgrades

Rapid and efficient deployment across large •

and / or geographically dispersed organisations

Easy to control and monitor AIMS use across •

the whole organisation

Staff can notify and manage incidents from •

anywhere they have internet access

Reduced risk – instead of an upfront licence, •

simply pay a rental fee that is a portion of the total cost

Business critical focus – freed from •

maintaining applications and investing in the infrastructure needed to support them, staff and resources can be focused on improving the safety, quality and efficiency of care

AIMS Classification

Validated & comprehensive

The unique AIMS classification transforms free-text incident descriptions into actionable data to inform quality improvement initiatives. The taxonomy includes over 24,000 concepts logically organised into 18 incident types. Classification can be further extended with specialty data sets (taxonomies) such as Anaesthesiology, Obstetric Maternal, Obstetric Foetal, Ambulance / Retrieval, Radiology, and Dental.

To support the high performance reporting capabilities of AIMS version 4, the AIMS classification has undergone a process of refinement. Modifications are in-line with best practice approaches such the World Health Organisation’s International Classification for Patient Safety.

System Administration

Extensive client configuration

Powerful administrator functionality includes distributed administration roles and reduced reliance on iSOFT for configuration and system maintenance. Administrators can define client-specific parameters like date ranges, codes displayed in pick lists and security groups, permissions and individual user accounts. Control over data access at various levels, including module, function and field is now possible; and furthermore comprehensive system and data audit functions to view user activity and monitor data changes are also available.

The Lookup Codes module enables System Administrators to create, define and manage System Values according to client organisation terminology.

Alerts

Issue management

To ensure that staff are informed of serious events or simply told of tasks they are required to complete following an event, AIMS provides sophisticated Alert functionality.
