Progressive Employer Management Company (PEMCO) offers Allstate Workplace

Division (AWD) group and individual employee benefit products to eligible employees

through payroll deduction.

Health insurance does not cover all expenses and it’s not surprising that in today’s

media much attention is being drawn to examples of people who have health insurance,

yet severe injury and illness drove them to lose their home and assets. In fact, 40% of

bankruptcies are due to sickness and injury, and in 75% of those cases, the families

had health insurance. However, it did not cover all the additional expenses associated

with the disease or injury.

Voluntary insurance products are among the most sought after employee benefits since

they provide the opportunity to choose benefits employees actually need at affordable

premiums.

Group Accident Coverage

Accidents happen and Allstate’s Group Accident coverage pays the following benefits for covered on and off the job accidental injuries that result within 90 days (180 days for Accidental Death or Dismemberment) from the date of the accident. A physician must diagnose covered losses. Any loss not stated is not covered. Treatment must be received in the United States or its territories.

• $40,000 Accidental Death for primary insured; $20,000 spouse; $10,000 each child, if covered,

• $200,000 Common Carrier Accidental Death for primary insured; $100,000 spouse; $50,000 each child,

• Up To $40,000 Dismemberment for primary insured; Up to $20,000 spouse; Up to $10,000 each child,

• Up to $4,000 Dislocation or Fracture for primary insured; Up to $2,000 spouse; Up to $1,000 each child,

• $1,000 Initial Hospitalization Confinement for each covered person, the policy will pay a one-time benefit of $1,000 when the covered person is admitted for an overnight stay in the hospital as a result of an injury.

• $200 per day Hospital Confinement: Maximum of 90 days per injury.

• $400 per day Intensive Care Benefit: the policy will pay a benefit of $400, per day of confinement. The maximum number of days that this benefit is payable as the result of a single injury is 90 days.

• Ambulance: (needed as a result of accidental injury). $200 Regular Ambulance; $600 Air Ambulance

• Up to $500 Medical Expenses: Medical fees up to $500 for each covered person. Expenses incurred for this benefit are limited to: physician fees, X-rays, emergency room services, and repair to natural teeth if diagnosed by a dentist to have resulted from an injury.

• $50 per visit Outpatient Physicians Treatment Benefit: This benefit pays $50 for each visit by a covered person to a doctor, outside of a hospital, for any reason. This benefit is payable a maximum of two times per calendar year per covered person, not to exceed 4 times per year for family coverage.

• 12/12 PRE-EXISTING CONDITION LIMITATION and POLICY LIMITATIONS AND EXCLUSIONS apply.

Cancer/Specified Illness Coverage

It’s probably crossed your mind that you could get cancer and you may have thought about the ways it would affect your life and that of your loved ones. Have you considered how cancer would impact your financial security though? An average of 62% of cancer related expenses are considered non-medical, which means your health insurance may not pay for all of the costs associated with having cancer. This is where we can help.

The cost of cancer treatment today can create a severe financial hardship for many families. Despite cancer’s grim statistics, there is hope. Advances in detection and treatment of cancer are lowering the death rate, but the costs associated with cancer treatment continue to increase. Allstate Workplace Division’s Group Voluntary Cancer/Specified Disease Protection is

supplemental insurance designed to help offset non-medical expenses associated with the treatment of cancer and 29 other specified diseases.

The Heritage Paycheck Protector from Allstate Workplace Division gives you a source of income when you can’t work because of a covered sickness or off-the-job injury.

Hospital Indemnity (SHOP)

The sample rates below provide lump sum $180 per day benefit for covered hospital confinements plus $180 for intensive care, ambulatory surgical benefit of $360 and initial hospitalization benefit of $900 to help offset the expenses you would incur in the event that you are hospitalized. Even if you currently have major medical insurance, the supplemental health options policy (SHOP) benefit would help cover the gaps due to high deductibles / high co-pays for hospital confinement. The SHOP plan can be customized to meet your individual needs. This is only an example.

Critical Illness

With the advancement in medical technology and treatment, people are living longer and once deadly diseases are being controlled and cured. One way to protect yourself, your family and your finances is to purchase a critical illness policy, which pays a lump sum benefit ($5,000 to $50,000) when you are diagnosed with a covered critical illness

.

Category I illnesses include Heart Attack, Stroke, Heart Transplant, By-pass surgery, Angioplasty and others specified. Category 2 illnesses include End Stage Renal Failure, Multiple Sclerosis, Alzheimer’s Disease and Paralysis. Benefits are paid on a schedule within each category. See the Critical illness brochure for details. A Cancer Rider and other riders are also available. The Critical Illness plan can be customized to meet your individual needs.

Rates for $50,000 of Critical Illness benefit start as low as $2.17 per week (48 paychecks per year). Premiums vary by age.

20 Year Term Life Insurance

Term Life Insurance offers an economical method of owning life insurance to cover a specific need for a specific amount of time. Horizon 20 year insurance is available through payroll deduction with a $10,000 minimum, and up $150,000 maximum issue limit.

Rates for Term Life Insurance vary by age.
