1.

Register with Cogent ID to capture your

fingerprints. You may go online to

www.machs.mo.gov

or call 1-877-862-2425. To register, you must use code

“2301”. Once registered, you will receive a “TBN”

number. You must take the “TBN” number with you to a

selected location. A list of locations is available on the

registration website.

2.

Apply for your Substitute Certificate online at

http://dese.mo.gov

located under “Web Applications.”

3.

Mail original transcripts to DESE (write in educator

ID or SS number and “Sub Cert”” mail to DESE Attn:

Certification, PO Box 480, Jefferson City MO 65102

(fully certified people do not have to send transcripts)

Dear Applicant,

Please complete the enclosed paperwork, as all forms must be complete before your name can be placed on the substitute call list. Also, please include two forms of ID. Most commonly used is a copy of your driver’s license and social security card; however, there are other form(s) of ID from the enclosed list of acceptable documents.

In order to qualify for substitute teaching, you must have a minimum of 60 hours of college credit. Please provide the following items located on the DESE website: 1) a copy of your substitute or current teaching certificate AND 2) your profile page showing that your fingerprints have cleared.

Applicants who do not currently hold a valid teaching certificate or a substitute teaching certificate will need to apply for a substitute teaching certificate through the Missouri Department of Elementary and Secondary Education website at: http://dese.mo.gov. You can also proceed with getting your fingerprint capture set-up with Cogent ID at:

www.machs.mo.gov. The fingerprint fee is not covered by our district.

We use a system to help us fill our vacant positions. The system is called Aesop and is very similar to the systems you have already used when dealing with your bank, insurance company, or any airline. Aesop will call you each day with job opportunities. Using your phone you can agree to work jobs that are presented to you when it calls, as well as call in to Aesop to hear jobs you may want to express interest in, change your telephone number, enter dates when you are unable to work, and much more. In addition to the telephone system, you can review available jobs, agree to work jobs, and much more using the Internet link to Aesop. Using Aesop on the Internet allows you to perform the exact same functions available over the phone system at your convenience. On your profile you should indicate the subjects and job area in which you can substitute, the work locations you will or will not work.

Please return your application and other forms in this packet once you have secured your Substitute Teaching Certificate from the State of Missouri to my office. Your name will not be approved by the Board of Education until we have received the completed forms.

Daily Rate of pay for Substitute Teachers: • 0-25 Days - $70 per day • 26-50 Days - $80 per day • 51 Days or more - $85 per day

Long-term substitute pay is $105 per day, which begins on the 6th day when a substitute is assigned to the same classroom for 6 consecutive days. Pay day is the 20th of each month. Pay is based on the days worked the previous month.

I have read the Fulton School District No. 58 Technology Usage policy and procedure and agree

to abide by their provisions. I understand that violation of these provisions may result in

disciplinary action taken against me including, but not limited to, suspension or revocation of my

access to district technology and termination of my employment with the district.

I understand that my use of the district's technology resources is not private and that the school

district may monitor my electronic communications and all other use of district technology

resources. I consent to district interception of or access to all of my electronic communications

using district technology resources as well as downloaded material and all data I store on the

district’s technology resources, including deleted files, pursuant to state and federal law, even if

the district’s technology resources are accessed remotely.

I understand I am responsible for any unauthorized costs arising from my use of the district’s

technology resources. I understand that I am responsible for any damages to district technology

due to my negligent or intentional misuse of the district’s technology resources. I understand

that this form will be effective for the duration of my employment with the district unless

changed or revoked by the district or me.

The purpose of this form is to provide information available to child care agencies including volunteer agencies. The records you receive will be based on the search options you select. The Missouri State Highway Patrol will respond when you choose option 1 or 2. The Missouri Childrenʼs Division will respond when you choose option 1, 2, or 3. Direct questions regarding criminal records to the Missouri State Highway Patrol (573-526-6153); direct questions regarding child abuse or neglect to the Childrenʼs Division (573-526-1438, TT: 1-800-735-2466).

The information on this form, and responses generated as a result of this form, are confidential. Any person disclosing the information in violation of 43.540, 589.400, RSMo. and/or 210.150 RSMo. is guilty of a class A misdemeanor.

1. Name Search - $10.00 Provides open records obtained from the Missouri Criminal Record Repository and informationfrom Missouri Childrenʼs Division Central Registry. a) Complete the request form.

b) Make a check or money order for $10.00 payable to “State of Missouri Criminal Records System.”

2. Fingerprint Search - $14.00/$20.00 Provides open and closed records with positive identification obtained from the Missouri Criminal Records Repository and information from Missouri Childrenʼs Division Central Registry. a) Complete the request form.

b) Obtain fingerprints on: Applicant card FD-258 or Patrol card SHP-152. Official taking fingerprints must verify identity of person fingerprinted with an official id such as a driverʼs license and sign the card as the person taking the fingerprints. Complete the rest of the card as applicable.

c) Make a check or money order for $14.00/$20.00 payable to “State of Missouri Criminal Records System.”

d) Mail completed forms and check or money order to: Missouri State Highway Patrol, Criminal Justice

Information Services Division, P.O. Box 9500, Jefferson City, MO 65102.

3. CD Central Registry Child Abuse Search Only - No Charge Provides information obtained from the Childrenʼs Division Central Registry only. The Childrenʼs Division (CD) Central Registry screening will reflect information contained in the CD database. Any questions about the accuracy of that information should be directed to the CD office in the residential county of the applicant or the county of employment if the applicant is not a Missouri resident. a) Complete the request form.

b) Mail completed form to: Missouri Childrenʼs Division, Background Screening / Investigations Unit, P.O. Box

88, Jefferson City, MO 65103.

OPEN RECORDS - convictions, charges pending, arrests less than thirty days old, and suspended imposition of sentence

during probation.

CLOSED RECORDS - charges not filed, not prosecuted, dismissed, or subject found not guilty or suspended imposition

of sentence after probation.
