Request for Qualifications Proposal for

Architectural Services

Glen Ellyn School District 41 is seeking Request for Qualifications (RFQ) from qualified

architectural firms to provide architectural services. The RFQ document will be available upon request and questions may be addressed by contacting:

Any Statement of Qualifications received after said time will not be accepted nor considered by the district. The Board of Education of Glen Ellyn School District 41 reserves the right to reject any or all RFQs, to accept RFQs in whole or in part, and to waive any irregularities or defects in any RFQ. The contract will be awarded, if at all, to one or more qualified firms, considering conformity with the RFQ specification, terms of delivery, quality and serviceability and such other factors as determined by the Board of Education. All decisions of the Board of Education shall be considered final.

INTRODUCTION:

Glen Ellyn School District 41 is conducting a search for a firm to provide architectural services for the facilities of the district. Qualification proposals will be accepted from firms interested in providing such services to the district as outlined below:

ARCHITECTURAL RESPONSIBILITIES:

1. Architect of Record for Glen Ellyn School District 41.

2. Repair and/or renovation of existing school district facilities and responsibilities associated with such projects, including but not limited to, procedural, permit and zoning requirements from local, county and state agencies.

3. Architect for potential future building projects including school building design and construction.

4. Knowledge of the Illinois Life/Safety Code and ADA compliance.

5. Services and products provided by the firm shall conform to the Illinois State School Code and be in accordance with all federal, state and local laws, codes, ordinances, and regulations.

6. The firm will be required to work with the district’s designated representatives to service the repair, renovation, design and construction needs of the district. STATEMENT OF QUALIFICATIONS INCLUSIONS:

The qualifications submitted for review should include, but not be limited to: 1. Brief history of firm.

2. Type of firm (e.g. corporation, partnership, sole proprietor, etc.) 3. Number of employees, responsibilities, etc.

4. A simple organizational chart identifying key members of the firm. 5. Services performed in-house (e.g. architect, engineer, etc.)

6. Provide proof and values of professional liability and general liability insurance carried by the firm.

7. School facility projects completed.

8. Information relative to scheduled and actual project completion history of all school facility projects in the last five years.

9. The firm shall detail its experiences managing a school project involving renovation of a facility.

10. Information summary relative to the number of “change orders” concerning school facility projects.

11. Information relative to the actual cost for school facility projects completed as compared to budgeted costs.

12. Information relative to experience in master planning and strategic planning for school facilities.

13. List of any school districts that the firm was dismissed from before a contract was completed and the circumstances surrounding that situation.

14. List of school district references including client name, address, telephone number and a person to contact for the above-named projects.

15. Appropriate visual representations of related project experience.

16. Litigation or court proceedings involving the firm/staff member(s) regarding past or present project performance.

17. Experience, concerns, and recommendations of the firm regarding the use of a construction management firm.

18. Firm’s general fee structure and billing requirements. 19. Completion of Errors and Omissions Request.

20. Completion of Certifications of Compliance with Illinois Drug Free Workplace Act. 21. Completion of Certificate Regarding Sexual Harassment Policy.

22. Completion of Certifications by Bidder.

PROCESS FOR SELECTING ARCHITECTURAL SERVICES:

The board or board committee will review each firm’s past record and experience,

qualifications, ability of professional personnel, performance data on file, location, workload, willingness to meet time and budget requirements, experience with projects similar in scope, previous experiences in the district, types of in-house services (e.g. disciplines), reliance on consultants, and such other factors as the board deems appropriate.

The board of board committee will rank the firms on the basis of their qualifications and competence. The board’s committee will then interview the top three firms. It is anticipated that interviews will be conducted with the top ranked firms during the week of October 19, 2015.

ERRORS AND OMMISSIONS INSURANCE:

As noted above, each firm submitting a qualifications statement must include an errors and omissions insurance overview. The overview shall include a comprehensive loss history, amount of errors and omission coverage, amount of deductible, and the name of the underwriter. PROPOSED TIME TABLE:

CERTIFICATION OF COMPLIANCE WITH ILLINOIS DRUG-FREE WORKPLACE ACT

The undersigned, having 25 or more employees, does hereby certify pursuant to section 3 of the Illinois Drug-Free Workplace Act (30 ILCS 580/3) that it shall provide a drug-free workplace for all employees engaged in the performance of work under the contract by complying with the requirement of the Illinois Drug-Free Workplace Act and, further certifies, that it is not ineligible for award of this contract by reason of debarment for a violation of the Illinois Drug-Free Workplace Act.

CERTIFICATE REGARDING SEXUAL HARASSMENT POLICY

The undersigned does hereby certify pursuant to Section 2-105 of the Illinois Human Rights Act (775 ILCS 5/2-105) that it has a written sexual harassment policy that includes, at a minimum, the following information: (i) the illegality of sexual harassment; (ii) the definition of sexual harassment under state law; (iii) a description of sexual harassment, utilizing examples; (iv) an internal complaint process including penalties; (v) the legal recourse, investigative and complaint process available through the Department of Human Rights and Human Rights Commission; (vi) directions on how to contact the Department of Human Rights and Human Rights Commission; and (vii) protection against retaliation.
