or out-of-country, or a direct cremation with no formal services , we will customize our

PROFESSIONAL SERVICES

These prices may already be included in the service options listed on the previous page.

BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF………...…...………...$1,250 Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service (24 hours per day); arrangement conference with family or responsible party; arrangement of services selected; preparation and filing of necessary authorizations and permits; recording vital statistics; staff assistance prior to, during, and following arrange-ments including coordination with those providing other portions of the arrangearrange-ments, e.g. cemetery, crematory and others. Also included in this charge are overhead expenses which include, but are not limited to the following: legal, accounting, maintenance of facilities, licensing, secretarial and administrative costs, and equipment and inventory costs. The fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing or entombment in a mausoleum. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or direct burial. Payment Policy

Thank you for selecting Lewis Funeral Chapel and The Stone Chapel Poulsbo Mortuary to handle the final care of your loved one. We realize that death can come at a time when the family is not prepared financially. Please do not base your service selection solely on your ability to pay cash at the time arrangements are made. We accept Visa, MasterCard or Discover credit cards for payment of services. If preferred and approved by our credit department, contract terms up to 24 months at 1% per month interest may be arranged. Insurance assignments are also acceptable as payment, providing the insurance policy is assignable to the funeral home. Under certain limited instances, there may be funds available from the Kitsap County Indigents Program.

The goods and services shown enclosed are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

Infants & Children

Selections for our services, facilities and vehicles listed on our General Price List are offered at a discounted rate for infants and young children up to the age of 15. Please ask one of our funeral home counselors for a copy of our in-fant and children General Price List for reference.

Benefits Assistance

Our staff will assist in the application for and notification of Social Security, Veterans Administration, Veterans Af-fairs, personnel Departments and Fraternal organizations for available benefits that you may be entitled. When pos-sible, our professional staff will also process life insurance claims on your behalf.

Terms of payment are to be finalized at the time of the arrangement conference. We accept 10 methods of payment, which are listed below:
