WAIVER and RELEASE: I understand that the staff of Accent On Dance-Mukwonago, are not physicians or medical practitioners. With this in mind, I hereby authorize Accent On Dance-Mukwonago staff to render temporary first aid to my child or children in the event of any illness. If necessary, the Accent On Dance-Mukwonago staff may seek medical help. This indudes transportation, by ambulance or an Accent On Dance, Inc. staff member and/or its representatives whether paid or volunteers, to any facility or hospital should Accent on dance deem this necessary.

We, the staff of Accent On Dance-Mukwonago, recognize our obligation to make our students and their parents aware of the risks and hazards associated with the art of dance. In dance, a student could incur injury, possibly minor, serious, or catastrophic in nature.

Parents need to make their children aware of the possibility of injury, and encourage their children to follow all safety rules and teacher's instructions. Accent On Dance and its staff WILL NOT accept responsibility for injuries sustained by any student during the course of dance, competition, recital, or events that he/she may participate in. This includes travel to and from these events, or time before or after scheduled dasses.

While fully aware of the risks of possible injury, I consent to have my child participate in programs offered by Accent On Dance-Mukwonago I will waive and release all rights and claims for damages that my child or I may have against Accent On Dance-Mukwonago and/or its representatives, whether paid or volunteer. I also affirm that I now have, and will continue to provide, proper hospitalization, health, and accident insurance coverage, which I consider adequate for both my child's protection and my own. I also understand that it is the parent's responsibility to educate the child about the risks of dance. Accent On Dance Inc. will teach with appropriate safety measures throughout our classes. I authorize the use of my child's name, or the use of his/her photograph, or likeness of for any promotional use or advertising.

I understand that there are no refunds or transfer of fees to other classes. I understand ALL CLASSES MUST BE PAID IN FULL IN ORDER TO PARTICIPATE.

I hereby agree to all of Accent On Dance-Mukwonago policies.

Our hope is that dance promotes a positive self image for all of our students. With this goal as a priority, we have created three unique programs within Accent On Dance-Mukwonago. Participation in the AOD Summer Program provides enrichment for all of our dancers! Continue training with the intention of accelerating progress and have fun along the way! A great time for NEW students to get acquainted with our studio too!

All Star Summer Intensive Workshop

Our Developmental programfor the recreational dancer is an excellent place to exercise the mind and body while experiencing the joy of artistic expression and accomplishment. There is no audition required for this program. Students progress naturally through each level as they continue to explore the art form. Our Company Program is for those dancers who are ready to train for national competition. Commitment, focus, artistry and passion have lead graduates of this program to choosing careers in dance performance and related fields. All dancers from 2nd- 12th grade audition yearly to be accepted or remain in this nationally ranked program. Students audition in our summer program and enhance their individual progression throughout our summer offerings. It is highly suggested that company students attend the Summer Intensive in Waukesha (see pgs. 4-5). You cannot match the training and growth that takes place in this concentrated and inspirational session. Although not "technically" part of the audition process, it does go a long way in showing your commitment to your goals.

The Company Auditions (2nd-12th grade) take place July 15th-July 19th, 2013 (Mukwonago) or July 22nd - July 26th, 2013 (Week Two-Waukesha)

(only one week is required to audition)

All dancers audition yearly to be accepted or remain in these programs.

Our auditions are dance classes observed by our faculty. Students will be given constant feedback, tips and corrections during class. It is our goal to use the audition process to enhance their overall dance training as well as choose our 2013-2014 dance teams. At the auditions, each family selects a participation choice using the requirements that are laid out to customize their involvement for the year. Your participation choice is a factor in your fall class grouping but is not a reflection of a level of dance in and of itself. Fall placements from these auditions are mailed mid-August. No written evaluations are given of auditions from Accent On Dance-Mukwonago. It is necessary that all dancers and parents accept the outcome of these auditions regardless of placement.

Tuition is calculated per class not per hour and must be paid in full upon registration for class.

their teachers professional opinion. Classes may be

split or combined depending on enrollment or

teachers discretion.

After thoroughly reviewing the summer schedule, complete ONE REGISTRATION

FORM PER STUDENT. Detach and mail with your summer payment to Accent On

Dance-Mukwonago.

Forms and payments will be accepted until classes fill. All classes are subject to

cancellation.

Once payment for summer classes is received, there will be

NO REFUNDS

classes missed or fee transfers to other classes.

Students should use their "grade entering in fall" when choosing the appropriate

class placement.

A teacher's recommendation is required before pointe shoes are purchased and

worn in class.

Solid color leotard and tights must be worn for all ballet and pointe classes.

Jazz pants, crop tops, and dance shorts may be worn for other classes.

Ž

Long hair must be secured up off neck. All hair must be pulled away from the

face during ballet classes, and at any time when asked for by teacher.

Ž

Appropriate footwear must be worn for all classes.

No jewelry, watches, or gum in class.
