Section 1 – Introduction

This directory provides a list of Aetna Medicare Plan (HMO) network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage.

If you are enrolled in our Aetna Medicare Prime Plan (HMO), you have access to a network of local providers who we work closely with to coordinate your care. If you have the Prime HMO plan, you will have to pay on your own for out-of-network care. If you need urgent or emergency care, or get out-of-area kidney dialysis, we’ll cover it. Even if the provider isn’t part of the Prime network.

If you are enrolled in a standard Aetna Medicare Plan (HMO), you will have to choose one of our network providers who are listed in this

directory to be your primary care {physician/

provider} (PCP). The term “PCP” will be used

throughout this directory. Generally, you must get your health care coverage from your PCP. Your PCP will issue referrals to participating specialists and facilities for certain services. For some services, your PCP is required to obtain prior authorization from Aetna Medicare. Except in an emergency and for certain direct access services, you will need to obtain a referral from your PCP before seeking covered non-emergency specialty or hospital care. You must use network providers except in emergency or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers neither Medicare nor Aetna Medicare will be responsible for the costs.

The HMO network providers listed in this directory have agreed to provide you with your health care, vision, hearing and dental coverage. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from out-of-network providers.

In cases where non-contracting providers submit a bill directly to you, you should not pay the bill, but should submit it to Aetna Medicare for processing and determination of your cost-sharing amount, if any.

Certain network health care services, such as hospitalization or outpatient surgery, require prior authorization from Aetna Medicare. This means the service must be approved by Aetna Medicare before it will be covered under the plan. Check your Evidence of Coverage for a list of services that require this approval. When you are to obtain services requiring prior authorization from a network provider, the provider is responsible to obtain prior authorization from Aetna Medicare before treatment. If your plan includes an out-of­ network benefit and you decide to receive covered services from an out-of-network provider, contact Aetna Medicare for prior authorization of those services.

Emergency care

If you need emergency care, you are covered 24 hours a day, 7 days a week, anywhere in the world. Whether you are in or out of an Aetna Medicare service area, we simply ask that you follow the guidelines below when you believe you need emergency care.

Call the local emergency hotline (for example, 911) or go to the nearest emergency facility. Notify your PCP as soon as possible after receiving treatment. Or if a delay would not be harmful to your health, call your PCP.

If you are admitted to an inpatient facility, you, a family member or friend on your behalf should notify your PCP or Aetna Medicare as soon as possible.

If you are traveling outside of the Aetna Medicare Plan (HMO) service area, you are covered for emergency and urgently needed care. Emergency care can always be obtained in or out of the service area from the nearest available provider. When in the network service area, HMO plan members must use plan providers for urgent care unless the providers are not available. When out of the service area, urgent care may be obtained from the first available provider. Urgent care may be obtained from a private practice physician, a walk-in clinic, an urgent care center or an

emergency facility. We will review the information submitted to us by the provider who supplied your care. If the nature of the urgent or emergency problem does not qualify for coverage, you may have to provide us with additional information. Follow-up care after emergencies

All follow-up care should be coordinated by your PCP. For HMO plans, follow-up care with out-of­ network providers may only be covered with a referral from your PCP and prior authorization from Aetna. Whether you were treated inside or outside your Aetna Medicare service area, if your plan requires referrals, you must obtain a referral before any follow-up care can be covered. If your plan does not require referrals you should contact our Member Services department at

1-800-282-5366 (TTY: 711) before care is received at out-of-network facilities. Suture removal, cast removal, x–rays and clinic and emergency room revisits are some examples of follow-up care.

Si está inscrito en nuestro Plan Prime (HMO) de Aetna Medicare, usted tiene acceso a una red de proveedores locales con los que trabajamos muy de cerca para coordinar su atención. La atención médica de emergencia siempre se puede obtener dentro o fuera del área de servicio del proveedor disponible más cercano. Cuando en el área de servicio de la red, los

miembros del plan HMO deben utilizar los

proveedores del plan para recibir atención médica de urgencia a menos que los proveedores no están disponibles. Cuando fuera del área de servicio, se puede obtener atención médica de urgencia del primer proveedor disponible. La atención médica de urgencia puede obtenerse de un médico de práctica privada, una clínica sin citas, un centro médico de urgencias o un centro médico de emergencia. Revisaremos la información que nos presenta el proveedor que suministró su atención. Si la naturaleza del problema urgente o de

emergencia no cumple los requisitos para recibir cobertura, probablemente sea necesario

proporcionarnos información adicional.

Seguimiento médico después de emergencias Todo seguimiento médico debe ser coordinado por su PCP. Para los planes HMO, el seguimiento médico con proveedores fuera de la red sólo puede estar cubierto si cuenta con una referencia médica de su PCP y autorización previa de Aetna. Sin importar si recibió tratamiento dentro o fuera del área de servicio de Aetna Medicare, si su plan requiere referencias médicas, usted debe obtener la referencia médica antes de que se pueda cubrir ningún seguimiento médico.
