Niteesh K. Choudhry, MD, PhD, is an Associate Professor of Medicine at Harvard Medical School and an Associate Physician in the Division of Pharmacoepide-miology and Pharmacoeconomics and the Hospitalist Program at Brigham and Women’s Hospital. His research focuses on the impact of medication costs and drug benefi t design on the use of and adherence to medications for common chronic conditions, such as coronary artery disease, hyperlipidemia and diabe-tes.

He is the principal investigator of a nationwide trial which enrolled more than 5,800 patients who were recently discharged from hospital after myocardial infarction and randomized them to receive full drug coverage or their usual levels of drug insurance for secondary prevention medications. Related to this, he has conducted policy evaluations of cost-sharing reductions recently introduced by large insurers and employers.

Dr. Choudhry attended McGill University and then received his doctorate of medicine and did his residency training in Internal Medicine at the University of Toronto. He received his doctorate of philosophy in health policy at Harvard University, with a concentration in statistics and the evaluative sciences, where he was a Frank Knox Memorial and Canadian Institutes of Health Research Post-Doctoral Fellow. He was also a Fel-low in Pharmaceutical Policy Research at Harvard Medical School. Valerie Fleishman provides the strategic and operational direction for NEHI. As Executive Director, she oversees the institute’s policy research, programs, opera-tions, fundraising and communications. Under her leadership, NEHI’s programs seek to improve chronic illness care, promote disease prevention and wellness, and speed the adoption of valuable innovations across the health care system. Ms. Fleishman joined NEHI at its founding in 2002. She has over fi fteen years’ ex-perience in health care, health policy, technology and innovation. Prior to joining NEHI, Ms. Fleishman was a director in Health and Wellness at Scient Corporation, where she led strategic consulting engagements with leading biotechnology, medical device and pharmaceutical clients in areas such as telemedicine, disease management, health information technology, clinical trials, and sales and marketing strategies.

Previously, Ms. Fleishman was a manager in Arthur D. Little’s Global Strategy Practice. She also worked in product management and brand marketing at Johnson & Johnson’s McNeil Consumer Products.

Ms. Fleishman received her bachelor’s degree, cum laude, from Harvard University and her masters of business administration from the Harvard Business School. In 2008, she was honored as one of Boston’s “40 under 40” by the Boston Business Journal.

Tom Hubbard leads NEHI’s projects on comparative eff ectiveness research, patient medication adherence, and prevention and wellness initiatives. Mr. Hubbard leverages his policy and technology experience to examine ways the country’s health care system can be transformed, focusing on the promotion of medical innovation and the improvement of quality and effi ciency in clinical care. Mr. Hubbard came to NEHI after seven years at the Massachusetts Technology Collaborative (MTC), where he led industry-focused projects and supervised the annual publication of the MTC Index of the Massachusetts Innovation Economy. His work has included supervising the I-495 Technology Corridor Partnership, an industry-municipal collaboration on growth, and leading projects on broadband deployment, federal research funding advocacy, and nanotechnology.

Previously, Mr. Hubbard served as Executive Assistant for Economic Aff airs to U.S. Senator John Kerry, as Deputy Director of Development for Massachusetts Governor Michael Dukakis, and as Director of Commu-nity Development and Planning for the City of Gardner, Massachusetts.

He is a graduate of Harvard College and holds a masters in public policy from Harvard’s Kennedy School of Government.

Anita Allemand is Vice President of Product Innovation and Management for CVS Caremark. In this role, Dr. Allemand leads the organization responsible for de-veloping PBM product strategies that help clients and their members maximize the value of their pharmacy benefi t through clinical interventions, benefi t plan designs and a variety of member outreach programs.

Previously, Dr. Allemand served as Vice President of Analytics and Outcomes where she had responsibility for all customer facing analytics, reporting and forecasting as well as providing outcomes related support to health care decision makers.

A pharmacist by background, her professional experience includes various clinical roles in managed care pharmacy and tenures as a hospital and retail staff pharmacist. Prior to her pharmacy career she held research positions in the pharmaceutical industry in the areas of drug develop

ment and testing.

Dr. Allemand earned two bachelor of science degrees, in biology and chemistry, from the University of Il-linois. She went on to obtain a doctor of pharmacy degree from Midwestern University, Chicago College of Pharmacy.

Joanne Doyle Petrongolo, PharmD, is ambulatory pharmacist in the Outpatient Pharmacy at Massachusetts General Hospital.

Dr. Doyle Petrongolo began her pharmacy career at CVS, then later went on to Tufts Medical Center where she worked in the outpatient pharmacy. Since 2005, she has worked at Massachusetts General Hospital outpatient pharmacy. From 2007 to 2008, she was part of the ambulatory pharmacy pilot study working in a primary care offi ce assisting in patient counseling, disease state management, and helping patients with medication access issues including Medicare Part D selection.

Dr. Doyle Petrongolo is also the pharmacist liaison for the Medicare care management program where she works with case managers and social workers to resolve medication problems in a high risk population. In 2010, the program partnered with a statewide initiative to prevent re-avoidable admissions within this Medicare demographic.

Dr. Doyle Petrongolo received her bachelors degree in pharmacy from Northeastern University and doctor-ate of pharmacy from the University of Rhode Island.

Marie Brown, MD, is an Assistant Professor of Medicine, Department of Internal Medicine, Rush University Medical Center Chicago, Illinois. She is a practicing internist and geriatrician actively engaged in patient care.

In 2012, Dr. Brown was elected Governor of the Illinois Chapter-Northern Re-gion of the American College of Physicians. Previously she was the Chair of the Scientifi c Program Subcommittee for the American College of Physicians Annual Session in 2007, and currently she serves on the national ACP medical education committee.

With a grant funded by the American College of Physicians, she has produced instructional videos on medication adherence highlighting interviews with real patients’ perspectives on the challenge of medica-tion taking. She lectures to physicians and primary care providers namedica-tionally on medicamedica-tion adherence, patient centered medical home, diabetes and updates in general internal medicine.

Dr. Brown received her bachelor of science from the University of Dayton in Ohio and her medical degree from Rush Medical college. She is board certifi ed in internal medicine and a member of the Board of Direc-tors of the American Board of Internal Medicine.

Mary Neagle has worked in health systems planning and program development for the past 25 years with a focus on health promotion, disease prevention and quality improvement.

Ms. Neagle began working with Partners HealthCare and Massachusetts General Hospital in the late 1990s to develop a women’s cancers disease prevention pro-gram with 10 community health centers in Metro Boston. In 2006, as Propro-gram Director for the Care Management Program at Mass General, Ms. Neagle sup-ported the development of an integrated care management system for high risk, medically complex patients. Originally a CMS Case Management for High Cost Benefi ciaries Demonstration Program, this program improved health outcomes, had high patient and physician satisfaction and reduced the cost of care. She is also co-director of the Partners Integrative Care Management Program (iCMP), a Partners HealthCare risk-based initiative to expand this model through-out its primary care network.

Ms. Neagle received her bachelors degree in sociology from Emmanuel College and her masters in social work, community organization and social planning from Boston College.

Patty Kumbera is a registered pharmacist and co-founder and Chief Operat-ing Offi cer for Des Moines, Iowa-based OutcomesMTM. OutcomesMTM helps payors such as consumers, employers, Medicare/Medicaid programs, pharma-ceutical manufacturers and health plans get more value from their medication purchases and reduce medication waste.

At a time when the importance of medication therapy management was just beginning to come to the attention of the health care industry, Ms. Kumbera and her OutcomesMTM partners were the fi rst to develop a practical business model for advanced pharmacist services and compensation. Today, over 33,000 pharmacists, trained by OutcomesMTM, have the opportunity to care for more than 2.5 million covered patients nationwide.

A graduate of the Drake University College of Pharmacy, Ms. Kumbera has been active in the evolution of Medicare Part D medical therapy management (MTM) services since they were introduced in 2006. She in-cited progress in MTM by urging CMS and Medicare Part D plan sponsors to regard MTM beyond its basic necessity—to comprehend its potential for enhancing quality health care and reducing costs for all parties involved in its delivery.

Bruce Stuart, PhD, is Professor and Executive Director of the Peter Lamy Center on Drug Therapy and Aging at the University of Maryland Baltimore.

An economist and health services researcher, Dr. Stuart began his career in health services research as an economic analyst and later Director of the Health Research Division in the Michigan Medicaid program in the early 1970s. Leaving state government for academia, Dr. Stuart taught health economics, fi nance and research methods at the University of Massachusetts and The Pennsylvania State University. In 1997 he joined the faculty of the University of Maryland School of Pharmacy as the Parke-Davis endowed Chair in Geriatric Pharmacotherapy and was selected as a Maryland Eminent Scholar for his work in geriatric drug use. He is a member of the AHRQ study section on Health Systems Research and also serves as a grant reviewer for various founda-tions.

Dr. Stuart received his bachelor’s in economics and fi ne art from Whitman College and received his masters degree in economics and doctorate of philosphy in economics from Washington State University.

Edith Roasto is the Chief Executive Offi cer for the Academy of Managed Care Pharmacy. As CEO, Edith is responsible for all programmatic, fi nancial and opera-tional aspects of the Academy, including a $10 million budget and staff of nearly forty.

For the past eleven years she has held leadership positions at the National As-sociation of Chain Drug Stores (NACDS). Most recently, she served as President of the NACDS Foundation and as Senior Vice President, Pharmacy Aff airs. During her tenure at NACDS she also served as Senior Vice President of Strategic Alli-ances and Development, Executive Director, Leadership Council and Vice Presi-dent, Pharmacy Aff airs. Prior to her time at the NACDS, she was Director of Customer Development at Healthcare Systems, National Accounts Manager for Wyeth-Ayerst Laboratories, and Corporate Pharma-ceutical Buyer for CVS Caremark.

Ms. Rosato holds a bachelor’s degree from Temple University School of Pharmacy and is a registered phar-macist in the states of Pennsylvania and Massachusetts.

Winston Wong, PharmD, is the Associate Vice President, Pharmacy Management at CareFirst BlueCross BlueShield. Since joining CareFirst in 1997, he has been responsible for the development, implementation and administration of the prescription benefi t, including the Medicare Part D prescription benefi t, where he was instrumental in the development and implementation of a pharmacist driven MTM program that has been recognized by CMS and many national organiza-tions.

Dr. Wong is also recognized for the implementation of a cutting edge program, showcasing the collaboration between Oncologists and the health plan, to de-velop a Pay-for-Quality program focused on the dede-velopment and compliance to treatment pathways for specifi c cancer types. This program has recently been enhanced to go beyond just treatment and incorpo-rate clinical and quality measures.

Dr. Wong earned his doctorate of pharmacy from the University of the Pacifi c in Stockton, CA, and com-pleted a general residency at the University of Nebraska Hospital and Clinics in Omaha, NE.
