GENERAL INFORMATION

Students or prospective students can obtain additional information about The Salon Professional Academy by directly contacting the school. Contact can be made in a variety of ways, and depending on the information needed may vary by department.

The Salon Professional Academy, established April 2009, offers training in Cosmetology – students are trained to provide services in hair, skin care and nail technology. We train students for entry-level positions in a

professional salon environment. Part of their education is providing quality, affordable services to the public. General information on the school or student information required to be made available under the Family Educational Rights and Privacy Act of 1974 (FERPA) can be obtained on the website at:

www.huntsvilletspa.com, by calling (256)721-6772 or via email at admissions@tspahuntsville.com. Paper copies of this information can also be made available upon request. Administrative office hours are Tuesday-Saturday from 10:00am to 5:00pm.

All programs, documents, and communications are presented in English. The Academy has deep respect for all the diversity of languages within our communities. For the well-being, safety, and effective communication with the general public, students, and faculty, The Salon Professional Academy has designated English as it primary language

PURPOSE

This Student Consumer Handbook is published by the Financial Aid Office of The Salon Professional Academy. Its purpose is to describe the services available to students through this office as well to provide important information about policies and procedures. In the case of conflicting information between this publication and the student catalog, the student catalog supersedes.

This handbook will answer most of your questions relating to financial aid, however, it is important that students are aware that federal regulations are subject to change and may impact policies and procedures stated in this publication. Students should visit the Financial Aid Office for the most current information. General questions can usually be answered on a walk-in basis. Specific questions may require an appointment.

MISSION STATEMENT

The Salon Professional Academy is dedicated to student success by enriching lives and cultivating professionals through innovative, personalized education in a creative and inspiring environment that prepares graduates in the business skills and fiscal responsibility necessary for success in their career.

STATEMENT OF PRINCIPAL

The Salon Professional Academy strives to provide the resources and support that our students need in order to attend school, graduate, and find employment in a professional salon environment. We are committed to assisting our students and providing resources and support that are in the best interest of our students.

RESPONSIBILITY OF HANDBOOK/CATALOG INFORMATION

Each student is responsible for knowing the information provided in this handbook and catalog. The Academy reserves the right to change policies and/or to revise curricula at its discretion without notice.

The Academy is a beautiful 9,000 square foot educational facility with state-of-the-art equipment and spacious hair styling, manicurist, skin care, student lounge, classroom and office areas located in the at 4925 University Drive Suite 134, Huntsville. AL 35816.

Information relating to financial aid can be obtained by contacting our Financial Aid Office at (256)721-6772. The financial aid office can also be contacted by email at: tspafinanceoffice@gmail.com. Financial aid office hours are Tuesday – Saturday from 9:30am till 4:30pm.

Admissions Information

ALA SPA, LLC, a limited liability company in Alabama owned by William Lee Schultz, Paula Lecher, Phyllis B. Schultz, Michelle McGough and Renee Elliott.

The most current pre-enrollment information is given upon enrollment. This will include the graduation, placement, and licensure rates for the most recent award year. This information may also be found on the website www.huntsvilletspa.com under program disclosure.

IMPORTANT INFORMATION FOR ALL COURSES

HOW TO APPLY

1) Send completed application form to The Salon Professional Academy. 2) Have high school and post-high school transcripts sent to The Academy. 3) Schedule a visit and tour.

4) Meet staff and students.

5) Learn about curriculum, books, kits, apparel code and payment plans. 6) Sign enrollment agreement and pay enrollment fee.

EDUCATIONAL REQUIREMENTS

A student must have a high school diploma, G.E.D. certificate, or official transcript showing proof of high school graduation. Students who are home schooled must have a diploma or transcript approved by the Alabama State Board of Cosmetology. Students are admitted on the basis of educational background. The Academy does not admit ability-to-benefit students.

NON-DISCRIMINATION

The Academy does not discriminate on the basis of sex, age, race, color, religion, or ethnic origin in admitting students.

ADMISSIONS REQUIREMENTS FOR U.S.CITIZENS

The following are required for admission to all programs at The Academy: 1) Enrollment application with required enrollment fee ($200).

2) Copy of student’s official high school diploma, G.E.D. certificate, Certificate of Home Schooling Program Completion

3) Copy of the student’s high school and/or college transcripts with graduation date.

4) Copy of the student’s driver’s license, two 2”x2” passport photos, and social security card. 5) Signed complete enrollment agreement

ADMISSIONS REQUIREMENTS FOR NON-U.S.CITIZENS

Applicants who are not citizens of the United States have the same requirements as U.S. citizens, except that a valid visa or green card must be provided in place of the documentation that only U.S. citizens can provide, such as a birth certificate and social security card.

ORIENTATION

All courses will have a complete orientation on the Saturday before the first day of class.

CLASS SIZE

EARLY ENROLLMENT IS ENCOURAGED.

The Academy limits the class size for all programs. For each class, we accept: Cosmetology: 15 Students

CLASS START DATES

Cosmetology classes start in January, March, May, July, September, and November. See insert #1 for class start dates. Esthetics and Manicurist classes have not been scheduled at this time.

BOOKS AND KIT

Appropriate books and kits are provided by the Academy and issued to the student during training at the appropriate time. Students provide their own paper supplies.

APPAREL

Students wear academy-approved professional apparel and shoes.

HOLIDAYS

The Academy is closed for the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas.

TRANSFER POLICY

A transfer student may be accepted after careful evaluation of the student's academic records. No more than 20% of the program can be credited from another institution unless student is transferring from another Salon Professional Academy. Each transfer must be evaluated on an individual basis. Transfer students pay full tuition. Transfer students must begin at the beginning of the program. All transfer hours accepted are applied at the end of training. We do not recruit students already attending or admitted to other schools offering similar programs. We do not guarantee the transferability of our credits to any other institution.

HOURS

The Academy is open Monday through Saturday plus evenings. Hours of attendance depend on the schedule assigned.

 Cosmetology students in the full-time program begin with a schedule of Monday through Saturday 9:00 a.m. to 5:00 p.m. for foundation training and then attend a schedule including two evenings from 12:00 pm to 8:00 pm and 3 days from 9:00 am to 5:00 pm including Saturdays for instructional training.

 Esthetics students in the full-time program begin with a schedule of Monday through Saturday 9:00 a.m. to 5:00 p.m. for foundation training and then attend a schedule including two evenings from 12:00 p.m. to 8:00 p.m. including Saturdays for instructional training.

 Nail students in the full-time program being with a schedule of Monday through Saturday 9:00 a.m. to 5:00 p.m. for foundation training and attend a schedule in 3 hours a day with alternating Saturdays.

ACADEMIC INFORMATION

RADUATION REQUIREMENTS

Graduates earn a diploma.

A student may graduate from the Academy when the student has completed the required Alabama Board of Cosmetology hours, has 85% attendance, and 85% grade average. Students are evaluated on a level system that prepares them for salon performance levels. All work must be completed to graduate.

The following are required for graduation from all programs:

1) completion of Alabama Board of Cosmetology required number of hours 2) completion of all assignments and tests including projects

3) an 85% attendance and 85% grade average

4) payment of all tuition, fees and over-contract charges per the enrollment agreement or other arrangements that have been made

5) a completed exit interview

HOUSING

Although The Academy does not offer housing, housing may be obtained through www.apartmentguide.com.

EXTRA EDUCATION

Extra educational events are available for students at the Academy.

GRADING PROCEDURES

The qualitative element used to determine academic progress is a reasonable system of grades as determined by assigned academic learning. Students are assigned academic learning and a number of practical

experiences. Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion. Practical skills are evaluated daily according to

procedures. Students must maintain a cumulative written grade average of 85% and pass a FINAL written and practical exam prior to graduation. Students must make up missed tests and incomplete assignments. The Academy utilizes a 100-point grading scale: 85% - 100% is passing, 0-84% is not passing. Grades are given in all instructional areas. Work habits, appearance, conduct, initiative, cooperation, and attendance are also considered.

Numerical grades are considered according to the following scale: 95 - 100 EXCELLENT

90 - 94 VERY GOOD 85 - 89 SATISFACTORY

80 - 84 BELOW STANDARDS– UNSATISFACTORY

Incompletes

Incompletes are at the discretion of the educator. When the student is making every attempt to learn a skill or subject but requires additional time to complete the work successfully, student may be given an incomplete. Student will receive deadline and description of work that must be completed from the educator.

Inadequate Grades

The student will be informed immediately after a Satisfactory Academic Progress point if they are weak in an area of study or skill. Educator will determine deadline student will have to complete the work satisfactorily. The

CONDUCT

Students may be suspended for poor performance, absence, tardiness, disrespect, or inappropriate behavior included but not limited to bad attitudes. If a student is suspended, the student will be advised about what the student must do to correct the problem. The Academy’s intention is to establish professional behavior for the likelihood of success in the industry. If a student is not showing a positive attitude and demonstrates the possibility of limited growth for success in their career, it is the responsibility of the Academy to inform the student how the deficiency can be corrected. Suspension or terminated from the program is at the discretion of The Academy and is a possible remedy for any behavior depending on the nature and severity of the conduct in question.

*Also note that any student who has been suspended may not make up those hours. Suspended hours will automatically put students over contract.

TERMINATION

A student may be terminated for failure to comply with Academy attendance policies, inadequate grades, or inappropriate behavioral issues as outlined in the policies (a part of the contract.) The student may also be terminated for failure to fulfill tuition requirements as agreed upon per the enrollment agreement.

COMPLAINT PROCEDURE

If the student has a complaint that needs to be addressed, the student needs to pursue the following procedure:

1. The student should submit in writing a signed complaint to an educator, the director, or owner outlining the allegation or nature of the complaint.

2. Upon receipt of the complaint, the educator, director, or owner will a. Review the complaint

b. Will respond to the student within 10 days of receipt of the written complaint with a possible resolution

c. Schedule a meeting to discuss the complaint and resolution. The student will be provided a copy of this written record at the time of the meeting.

If the problem cannot be resolved through discussion, the student will be referred to The Academy’s complaint committee consisting of an owner, an educator and an administrative office staff member. The complaint committee will meet within 21 calendar days of the receipt of the complaint and review the allegations. If more information from the student is needed, a letter will be written outlining the additional information. If no further information is needed, the committee will send a letter to the student within 15 calendar days stating the steps taken to correct the problem or information to show that the allegations were not warranted or based on fact.

If the student has tried to resolve the issues through the Academy’s complaint process and is unsuccessful, the student may contact the Alabama Board of Cosmetology, 100 N. Union St., #320, Montgomery, AL 36130; phone 334-242-1918.

FINANCIAL AID INFORMATION

FINANCIAL AID CODE OF CONDUCT

(Based on Federal Regulations and the Higher Education Opportunity Act)

Personnel Affected

This code of conduct applies to all financial aid employees and all other employees and agents who have responsibilities with respect to educational loans or who have contact with guaranty agencies or lenders (either federal or private loan lenders). All decisions and actions must be consistent with the following principle:

1. Student Choice -All students must be given an equal and fair opportunity to choose from all lenders and loans, federal and private. These choices will not be limited to lenders or loans that are available through or recommended by The Salon Professional Academy. The Academy does not have a preferred lenders list.

2. Student Interests -The Salon Professional Academy will seek relationships with lenders which can provide the best benefits for students such as interest rates, fees, payment terms, and services.

3. Avoidance of Conflict of Interest -A conflict of interest is created when an employee’s personal situation interferes with his/her judgment which can make him/her unable to act in the best interest of The Salon Professional Academy or the students attending the institution. All employees must avoid conflicts of interest. No employee shall have any relationship with any lender guaranty agency nor accept any fees or anything of value, other than token marketing items and nominal conference refreshments, from any lender.

The Salon Professional Academy will not accept any service or anything of value from any lender or guaranty agency with exception of the following services: loan processing materials and financial literacy materials, training or financial aid staff related to loan processing, entrance and exit counseling services conducted under the supervision of a financial aid officer from The Salon Professional Academy, emergency staffing services, and/or reimbursement of reasonable costs for domestic travel and training conferences hosted by lenders and/or guaranty agencies.

Student Privacy

It is the policy of The Academy to abide by the Family Educational Rights and Privacy Act of 1974. This act guarantees a student's right of access to the student's personal file and the student's rights to the privacy of that file. Information from a student's file will only be released upon written permission from the student. Written

permission is required for each and every request prior to the release of information. Parents/guardians of a minor student, accrediting agencies, and government officials may gain access to a student’s files without the

expressed permission of that student. This policy will be given at orientation, and is available at any time through the Admissions, Financial Aid, or Education offices at The Salon Professional Academy.

21 Code of Conduct

All financial aid employees and all other employees and agents are required to comply with all of the following rules (prohibited relationships) in this Code of Conduct regarding lender relationships and loans. In addition, they are required to notify the director/owner if they become aware of facts indicating that there may have been a violation of the Code of Conduct regarding lender relationships and loans.

 No financial aid employee or any other employee or agent, shall act as an employee, consultant or sales representative or officer for any lender or guaranty agency.

 No financial aid employee or other employee shall serve on the Board of Directors (compensated or uncompensated) for any lender or guarantee agency.

 No financial aid employee or any other employee shall serve on any lender or guaranty agency’s advisory board.

 No agent of The Salon Professional Academy will be allowed to serve on a lender or guaranty agency’s advisory board without written approval from the director/owner to ensure the agent will comply with this policy.

HOW TO APPLY FOR FINANCIAL AID

Filling out the Free Application for Federal Student Aid (FAFSA) is the first step in securing financial aid.

All students must complete this in order to obtain any federal aid, regardless of the financial status of the family. This will determine your eligibility for a wide range of financial aid. There is a year-end deadline of June 30th to complete the FAFSA. We encourage all students to apply early in order to ensure you receive your

disbursements within a reasonable time.

To ensure we are able to properly process your FAFSA, make sure your completed FAFSA includes The

Salon Professional Academy’s School Code: 042110. This code will be used by the federal processor to ensure we receive your FAFSA results, which will determine your federal aid eligibility. To ensure accuracy, whenever possible, use income and other information from your completed tax return, rather than estimated figures. Once your FAFSA results are received by The Salon Professional Academy, you will be awarded financial aid, according to your eligibility. You can find more information, or apply for financial aid by visiting the FAFSA website at: http://www.fafsa.ed.gov/.

WHO RECEIVES FEDERAL STUDENT AID?

Our most basic eligibility requirements are that you must:

 Demonstrate financial need

 Be a US citizen or an eligible noncitizen

 Have a valid Social Security number

 Register with Selective Service (if you haven’t already and are male between the ages of 18 and 25)

 Maintain satisfactory academic progress in a postsecondary school, and show you are qualified to obtain a postsecondary education by

-having a high school diploma or General Educational Development (GED) certificate -completing a high school education in a home school setting approved under state law

TYPES OF FINANCIAL AID AVAILABLE

FEDERAL PELL GRANT

Pell Grants provide a foundation of financial assistance that may be supplemented by other resources and doesn't have to be repaid after graduation. Pell Grants are determined after the financial status of a student is fully reviewed. Based on a student's financial circumstances, a specific amount of money is disbursed annually toward the student's education through the Pell Grant. How much you can expect to receive from a Pell Grant is solely based on your financial situation and other criteria.

WILLIAM D. FORD DIRECT STAFFORD LOANS

Loans made through this program are referred to as Direct Loans, because eligible student and parents borrow directly from the US Department of Education. You must be enrolled as at least a half-time student to be eligible for a loan. Direct loans include the following:

Direct Stafford Loans

 Direct Subsidized Loans – you must have financial need to receive a subsidized loan. The US

Department of Education will pay (subsidize) the interest that accrues on your Direct Subsidized Loan during certain periods.

 Direct Unsubsidized Loans – financial need is not a requirement to obtain an unsubsidized loan. You are responsible for paying the interest that accrues on your Direct Unsubsidized Loan.

 Direct PLUS Loans (PLUS Loans) are loans parents can obtain to help pay the cost of education for their dependent undergraduate children.

G.I. BILL

If you've spent time in the military, you are eligible to use your G.I. Bill toward your cosmetology education. The G.I. Bill was specifically designed for college or vocational education. Certified by State Approving Agency for Veterans’ Educational Benefits. *These approvals are state and school specific and may not apply to all schools. For updates and more information on federal financing, go to studentaid.ed.gov to learn more.

LOAN DISCLOSURES

 Student Loan Information Published by the US Department of Education (The Guide to Federal Student Aid) is available in the Financial Office.

 NSLDS (National Student Loan Data System – student loans will be submitted to the NSLDS and will be accessible by guaranty agencies, lenders and schools determined to be authorized users of the data system.

HOW FINANCIAL NEED IS CALCULATED

1. Apply for federal student aid – Complete the Free Application for Federal Student Aid (FAFSA). For FAFSA on the web go to www.fafsa.gov. Using FAFSA on the web is faster and easier than using paper. (Hint: be sure to have your tax forms from the previous year available to use in filling out the FAFSA). Our school code is 042110.

2. The school will receive your Institutional Student Information Record (ISIR). The Financial Aid Officer will review your ISIR and your Expected Family Contribution (EFC) calculated on your FAFSA application. The Financial Aid Officer will then figure out how much aid you can expect to receive through grants and loans. The Financial Aid Officer will discuss with each student. What aid they will receive because each student has different personal information and financial circumstances.

STUDENT (BORROWER) RESPONSIBILITIES

It is expected that students are actively involved in their education. This begins with understanding the

Admissions process, determining the best option to pay for school, and striving to always maintain satisfactory progress. We are here to assist students in all aspects of this process, but in the end each student is responsible for his/her own education.

Students are expected to pay for their tuition by one of the methods outlined in the enrollment agreement, signed prior to beginning classes. Failure to make these payments could result in termination from the Academy. The Salon Professional Academy will not penalize students for delays in aid due to regulations, timing of financial aid awards, or other circumstances which are out of the student’s control. However, if a student fails to take action on items required for aid to be processed, the student will be responsible and it may jeopardize that student’s ability to continue in the program. It is essential that students watch for

correspondence from the school or other agencies which may send notices about financial aid.

Verification is a common item which may lead to delays or cancellation in financial aid. If a student is selected for verification, it is essential that the student provide the information requested or the student's financial aid package cannot be processed. For more information on verification, see the Verification Policy located in this handbook.

It is the responsibility of the student to:

 Think about how much you are borrowing: how the amount of the loan will affect your future finances, and what your repayment obligation means before you take out a student loan.

 Sign a promissory note: you are agreeing to repay the loan according to the terms of the note even if you do not complete your education, can’t get a job after you complete the program, or you didn’t like the education that you received.

 Make payments on time: you are required to make payments on time even if you don’t receive a bill, repayment notice, or a reminder. You also must make monthly payments in the full amount required by your repayment plan. Partial payments do not fulfill your obligation to repay your student loan on time.

 Continue to pay your loans while waiting for deferment or forbearance approval.

 Keep in touch with your loan servicer: Notify your servicer when you graduate; withdraw from school, drop below half time status, change your name, address, or Social Security number; or transfer to another school.

ENTRANCE COUNSELING

In addition to meeting with the Financial Aid Administrator, the student must also complete the required entrance loan counseling prior to receiving any disbursements. This is a onetime requirement and must be completed in advance by using the Federal Direct Loan Entrance Counseling link on the Direct Loan website at: www.studentloans.gov. The student is required to complete the quiz using their name, social security number and date of birth. Students are also required to complete an Entrance Loan Counseling form, which the student and Financial Aid Administrator must sign to verify that this counseling has been completed. Once the student has completed the quiz, the results will be electronically available to the Financial Aid

Administrator, who will then sign the form and place it in the student’s file. It is required that all Direct Loan Borrowers complete entrance loan counseling, even if the borrower has had loan counseling at another institution.

MASTER PROMISSARY NOTE

In addition to meeting with the Financial Aid Administrator and completing Entrance Loan Counseling, the student must also complete a Master Promissory Note prior to receiving any disbursements. This must be completed in advance and can be completed through the Master Promissory Note link on the Direct Loan website at: www.studentloans.gov. It is recommended to turn off all pop-up blockers to enable successful completion of the Master Promissory Note.

In order to complete the Master Promissory Note, the student will need their Federal PIN, driver’s license (or state ID) number, and the names and addresses of two personal references from two different households.

EXIT LOAN COUNSELING

If a student who was awarded a Direct Loan withdraws, drops below half-time enrollment, or graduates, the student must complete exit loan counseling. The exit counseling can be completed online through the Direct Loan website at: www.nslds.ed.gov. The exit counseling will provide the student important information

regarding repaying their loan, average monthly repayment amount, deferment, loan cancellation and consequences that may occur due to student loan default.

After completing this counseling, the student and the Financial Aid Administrator must sign the Exit Loan Counseling form to verify that it has been completed. This form will then be placed in the student’s file. Please contact the Financial Aid Office if you have questions in regards to exit loan counseling.

REPAYMENT OF LOANS

There is a set time period after a student graduates, leaves the school, or drops below half-time status before the student must begin repayment of loan(s). This period of time is called a grace period and gives the student the time to get financially settled and select a repayment plan. The grace period for a Direct Stafford loan is six months. PLUS Loans do not have a grace period. Depending on the type of loan a student receives and the repayment plan chosen, the student may have from 10 – 25 years to repay the loans. Monthly repayment amount will depend on the type of loan, size of debt, length of repayment period, and repayment plan chosen. For more information, go online to www.studentaid.ed.gov/repaying.

LOAN REPORTING REQUIREMENTS

Subsidized and Unsubsidized Federal Loan information will be submitted to the National Student Loan Database System. This system is accessible by guaranty agencies, lenders, borrowers and institutions determined to be authorized users of the data system.

VERIFICATION

Verification is the confirmation through documentation that the information provided on a student's Free Application for Federal Student Aid (FAFSA) is correct. The federal government requires all Title IV approved institutions to verify or confirm the data reported by students and their parent(s) on the FAFSA. The verification process ensures that eligible students receive all the financial aid to which they are entitled and prevents ineligible students from receiving financial aid to which they are not entitled.

A student may be selected for verification either by the U.S Department of Education Central Processing System (CPS) or by The Salon Professional Academy’s Financial Aid Office. If you are selected for verification by CPS, there will be an asterisk next to your Expected Family Contribution (EFC) on your Student Aid Report (SAR). You will also be informed of having been selected for verification in the SAR acknowledgement letter, which will be included with your SAR documents.

If you are selected for verification, the school Financial Aid Office will notify you in writing via email to the email account you provided to the school upon enrolment, via phone or in person (if you are an active student). If you are notified by The Salon Professional Academy’s staff that you have been selected for verification you have 30 days from the date you received notification or up until the first day of class (for new students) whichever comes first, to submit the required documentation. Documentation required to complete verification may include a copy of the most recent tax return transcripts for the student and parents (if applicable), a verification

25 worksheet and any additional documents and forms required depending upon the information the student and/or parent provided on the FAFSA.

Students should submit the requested information in a timely manner and by the stated deadline to prevent any delays in the processing of their awards. Failure to submit the requested documents will result in cancellation of all federal financial aid awards. Students are advised to retain copies of any document submitted to the Financial Aid Office.

If corrections or updates are required as a result of the verification process, the school will submit the corrections to the U.S. Department of Education and the student will receive a corrected SAR. All documents provided by a student for verification will be retained in the student's file for auditing purposes. The student's Expected Family Contribution (EFC) may change based on the corrections made. If the award changes due to verification, the students will be notified in writing by The Salon Professional Academy.

HOW FUNDS ARE DISBURSED

Student Aid Report (SAR)/Institutional Student Information Report (ISIR)

Once the FAFSA is complete, the student will receive a Student Aid Report (SAR) via email or regular mail, depending on the method of communication provided by the student. If the student provided the School Code on the FAFSA, the school will receive an Institutional Student Information Report (ISIR). These reports will detail the information provided by the student on the FAFSA.

Verification

Additional information may be needed from the student in order to complete the financial aid process. The Financial Aid Administrator will contact the student and inform him/her of any additional information that has been requested. More information on verification can be found in the Student Consumer Handbook under the General Verification Policies and Procedures section, or by contacting the Financial Aid Office.

Award Letter

Once all the necessary information is processed and deemed to be accurate by the Financial Aid Office can generate the student’s Award Letter. The Financial Aid Administrator will contact the student by phone or mail a copy to the student. An appointment to discuss the financial aid award with the student will then be

scheduled.

At that meeting, the Financial Aid Administrator will explain to the student what aid (s)he is eligible for and any additional steps that must be taken prior to the student being able to receive a disbursement. The student will be also be given a chance to accept a portion or all of the aid provided.

Counseling and Master Promissory Note

If the student is eligible, and accepts Direct Loan assistance, the student must also complete Entrance

Counseling and a Master Promissory Note, prior to being able to receive any disbursements. If the parent of a dependent child is eligible, and accepts Parent PLUS Loan assistance, the parent must also complete a Master Promissory Note prior to being able to receive any disbursements. More information on these processes can be requested through the Financial Aid Office.

Financial Aid Disbursements

Cosmetology students abiding by the Satisfactory Academic Progress (SAP) Policy are eligible for financial aid disbursements (up to their maximum eligibility) at the following checkpoints 0-450, 451-900, 901-1200, and 1201-1500 hours.

Each time a disbursement is requested, the student will be required to meet with the Financial Aid Administrator to sign a Disbursement Acknowledgement Form. The student’s transcripts will be attached to the form, which will be signed by the student, the Financial Aid Administrator and the Director of Education before being filed in the student’s permanent file.

26 When the disbursement is received by the school, the percentage of tuition and fees owed to the school for that disbursement period will be credited toward the cost of the student’s tuition. If there are additional funds available after this credit, a disbursement check will be prepared for and given to the student within fourteen days.

 Pell Grant: If the student is eligible for a Pell Grant, the Financial Aid Administrator will request the first disbursement during the first week the student begins school.

 Direct Loans: If the student is eligible for and chooses to take a Direct Loan, the Financial Aid Administrator will request the first disbursement thirty (30) days after the start of their program.

 Parent PLUS Loans: If the parent is eligible for and chooses to take a Parent PLUS Loan, the Financial Aid Administrator will request the first disbursement thirty (30) days after the start of their program. Unlike the other types of financial assistance, if there is an overage after tuition and fees have been credited, the overage check will be sent directly to the parent or student within seven days.

PROCEDURES FOR EARLY WITHDRAWAL Official and Unofficial Withdrawal Process

If a student wishes to withdraw from school, they must notify the Academy’s Admissions/Financial Aid office. The notification may be in writing or orally. The Admissions/Financial Aid office must begin the withdrawal process. For official withdrawals a student’s withdrawal date is the date the school received notice from the student that they are withdrawing. For unofficial withdrawals a student’s withdrawal date is their last day of physical attendance. The school’s determination that a student is no longer in school for unofficial withdrawals is determined after 14 days of non attendance.

An unofficial withdrawal with regard to a student that has taken an approved leave of absence, and does not return will have a withdrawal date of the last day that the student attended before beginning the Leave of Absence.

The student must then set up an exit interview with the admissions/financial aid office to complete the withdrawal. At the exit interview the student’s file will be checked for correct documentation. Exit loan

counseling, concerning loans and financial aid, must be read through by any student taking financial aid loans. A tuition refund will be calculated using the formula listed on the student’s contract, and any required refund would be paid within 45 days.

Dependent upon the student’s hours, a withdrawal may result in the student owing The Academy tuition funds. After a student has dropped or terminated, any awarded financial aid disbursements can only be

requested/applied to a student’s balance if the student has satisfactory progress at the time of the exit and if the Return of Title IV Funds Calculation worksheet indicates the school can be paid. If the calculation shows that the student owes The Academy, all funds must be paid before the student’s transcripts can be released. If the calculation shows a refund to the student, a refund check will be issued to the student/guarantee agency within 45 days of the date of the determination of the withdrawal from school.

All students are required to attend the exit interview. However, if for some extenuating circumstances a student fails to attend the exit interview, all exit documents will be mailed to the last known address of the student. It will include the responsibilities of repaying the loan and where to get information concerning their loan. A student can contact the Admissions/Financial Aid office at any time if they have questions concerning their loan or need information concerning loan consolidation.

RETURN TO TITLE IV POLICY (R2T4)

The Federal Return of Title IV funds formula (R2T4) dictates the amount of Federal Title IV aid that must be returned to the federal government or the lending institution by the school and/or the student. The federal formula is applicable to an eligible student receiving federal aid when that student withdraws on or before the 60% point in time in the payment period. If a student does not begin training, the R2T4 formula does not apply. The federal formula requires a Return of Title IV calculation if the student received or could have received (based on eligibility criteria) federal financial assistance in the form of Pell Grants, PLUS loans, Stafford Loans, and withdraws on or before completing 60% of the payment period, The percentage of Title IV aid earned is equal to the percentage of the payment period that was completed as of the withdrawal date if this occurs on or before the 60% point of time. The percentage that has not been earned is calculated by subtracting the percentage of Title IV aid earned from 100%.

Return to Title IV (R2T4) Paperwork

This form is used to determine the amount of federal student funds that must be returned to the Title IV program on the school’s behalf. The Financial Aid Administrator will complete this form, and send it to the third party processor. The third party processor will review the form and inform the school of the results.

Returning funds to the Department of Education

If the R2T4 form determines that the school must return federal money, the business office transfers the money back into the school’s federal account. The third party processor is informed of the amount and date that the money was transferred back into the federal account.

Collection Policy

Tuition accounts are submitted to a privately contracted collection agency 30 days after a student has dropped/terminated from the program. If the student re-enrolls, the account will be removed from the collections process.

Returning funds to the student

If it is determined that the student has overpaid for the portion of the program completed, the student will be issued a refund check. The refund will be mailed to the student’s current address, unless other arrangements have been made with the Financial Aid Office.

The Academy’s Withdrawal, Cancellation and Settlement Policy

For applicants who cancel enrollment or students who withdraw from enrollment a fair and equitable settlement will apply. Applicants not accepted by the Academy shall be refunded all monies paid to the Academy except a non-refundable enrollment fee.

If student (or in the case of student under legal age, his/her parent or guardian) cancels the enrollment in writing within three business days of signing the enrollment agreement, all monies collected by the Academy will be refunded less the enrollment fee in the amount of $200 even if the student has begun classes. The "formal cancellation date" will be determined by the postmark on written notification; the date said notification is delivered to the Academy in person, the date of expulsion by the Academy, or30 days after the last day of attendance which is reviewed monthly by submission of hours sent to The Alabama Board of Cosmetology or the expiration date of an approved Leave of Absence.

If a student cancels the enrollment more than three business days after signing the contract but prior to starting classes, a refund of all monies paid to the Academy less the enrollment fee in the amount of $200 will be made.

28 For a student who enrolls and begins classes but withdraw prior to course completion (after three business days of signing the contract), the following schedule of tuition earned by the Academy applies:

If the student has completed 50% of the scheduled hours, no refund is due and the remaining unpaid tuition is due to the Academy. All monies due The Academy must be paid before transcripts are released.

Any monies due the applicant or student shall be refunded within 45 days of formal cancellation date as defined above. In the case of disabling illness or injury, death in the student's immediate family or other documented mitigating circumstances, a reasonable and fair refund settlement will be made.

If permanently closed or no longer offering instruction after a student has enrolled, the Academy will provide a pro rata refund of tuition to the student. A list of all students enrolled at the time of Academy closure including the amount of each pro rata refund is submitted to the appropriate agencies.

If the course is canceled subsequent to a student's enrollment, the Academy will either provide a full refund of all monies paid or completion of the course at a later time. The Academy does not participate in any teach-out plans with other institutions.

Students who withdraw or terminate prior to course completion are charged a withdrawal fee of $150.00. This policy applies to tuition and fees charged in the enrollment agreement. Other miscellaneous charges the student may have incurred at the institution (EG: extra kit materials, books, products, unreturned Academy property, etc.) will be calculated separately at the time of withdrawal.

All fees are identified in the catalog and in the enrollment agreement.

The Academy expects tuition and expenses to be paid in a timely manner. The Academy provides the courtesy of a collection phone call or written notice to a student when a payment becomes a few days past due. Once a student’s account exceeds 30 days past due and no alternative payment arrangements have been made, the account is subject to being turned over to a collection agency, lawyer and reported to the appropriate credit bureaus who abides by this policy. All fees incurred due to collection efforts will be added to the student’s total balance due.

The Academy provides necessary documentation to collection agencies and credit bureaus to assist in collecting payment for past due accounts. Signature from third party agency is required on documentation provided.

If promissory note or contract for tuition is sold or discounted to a third party, the third party agrees to comply with the withdrawal, cancellation and settlement policy of the Academy.

29 If the course is cancelled after the Student’s enrollment, The Academy shall provide a full refund of all monies less the enrollment fee or provide for completion of the course.

 Students using Title IV funds will follow the above refund policy AFTER the return of unearned Title IV funds calculation has been made. This calculation often results in the Student owing tuition and fees to the Academy.

The Federal return of Title IV funds calculation will be used for Students who have received financial assistance under the Higher Education Act, i.e. Federal Pell Grants or Stafford Student Loans, or Federal PLUS Loans awarded under the Federal Family Education Loan Program.

 If the enrollment is terminated during the first 60% of any payment period (450 hours), the Federal return of Title IV funds calculation will apply. If the Student has completed 60% of the payment period, no refund is due. However, if the Student must terminate due to physical incapacity or the transfer of a spouse’s employment, the Student will receive a refund based on the formula(s) above.

THE ACADEMY

P

AYMENT PLANS

We accept cash, check or credit card for any payment. The Academy has loans and payment plans for those who qualify. Loan applications are available in the TSPA Admissions or Finance Office.

1. Pay in Full

 Tuition is paid in full when the Enrollment Agreement is signed.

 Cash, check or credit card (VISA, MasterCard, or Discover) are accepted. A five percent (5%) discount is awarded if the contract is paid in full at signing.

2. The Academy’s Payment Plans

Option One: Ten monthly payments with no interest and no balloon payment at contract end.

 Student makes ten consecutive monthly payments of 1/10th of the total loan amount.

 At contract end date the Student's tuition will have been paid in full and the Student's school transcript will be immediately available for his/her State Board License application.

Option Two: Ten monthly payments of $250.00 at 12% interest with a balloon payment at the contract end date. Promissory Note and a qualified Co-signer are required for consideration.

 Student makes ten monthly payments of $250.00. Twelve percent (12%) of each monthly payment is deducted for interest charges.

 At contract end date, the balances of the tuition fees are due and must be paid in full within thirty days (balloon payment). The Student's school transcript is released when the tuition balance is paid in full. Option Three: Two-tiered loan at 12% interest. Promissory Notes and a qualified Co-signer are required for consideration. On-time payments on the First Tier Loan are required for Second Tier Loan eligibility.

 Two-Tiered Loan: Fourteen (14) monthly payments of $150.00 at 12% interest with the balance financed for 48 months at 12% interest. Second Tier Loan payment amounts will be considerably higher, on average between $350 and $500 per month, depending on the amount financed.

First Tier: Requires a Promissory Note and a qualified Co-signer. Student makes monthly minimum payments of $150.00 for fourteen months at 12% interest. Twelve percent (12%) of each monthly payment will be deducted for interest charges. A missed payment or late payment (beyond the 7-day grace period) will disqualify the Student from Second Tier Loan eligibility.

30 payment will increase significantly. The tuition balance will be financed at 12% interest for a maximum of 48 months. Shorter repayment periods are optional.

Enrollment Fee: $200.00 due when Enrollment Agreement is signed.

Cosmetology/1500 hours: Cost of tuition is $16,900.00 which includes textbooks and kit Esthetician/1500 hours: Cost of tuition is $16,900.00 which includes textbooks and kit Manicurist/750 hours: Cost of tuition is $7,170.00 which includes textbooks and kit

Cosmetology Instructor/650 hours: Cost of tuition is $6,770.00 which includes textbooks and kit Students provide their own stationery-type schools supplies

Students purchase professional apparel and shoes that meet Academy requirements.

DEFAULT MANAGEMENT

Default Management is required by the Department of Education for institutions who participate in the Direct Loan program. This is used as a tool to promote student success, and in an effort to reduce student loan defaults in this program. There are nine activities which make this successful, which are: Entrance

Counseling, Financial Literacy for Borrowers, Communication Across Campus, Exit Counseling, Timely and Accurate Enrollment Reporting, NSLDS Date Entered Repayment (DER) Report, Late State Delinquency Assistance (LSDA), Loan Record Detail Report (LRDR) Data Review and Analyzing Defaulted Loan Data to Identify Defaulter Characteristics.

The Salon Professional Academy has adopted the Department of Education’s Default Management Plan. We work with Wright International Student Services (WISS) in order to successfully follow this plan in an effort to reduce student loan default rates in the Direct Loan program.

PLACEMENT INFORMATION

The Salon Professional Academy provides each student with graduation and placement information prior to enrollment. The graduation rates are based on the requirements set forth by the Department of Education, whereas the placement rates are based on the information that we provide to our accrediting body annually. These placement statistics will include details regarding the source of the information, the timeframe used in calculating the rates, and the methodology used to compile the rates. Additional information on placement or types of employment obtained by graduates of The Salon Professional Academy can be obtained at any time by contacting the Admissions Office.

PRE-ENROLLMENT INFORMATION:

Accreditation auditing requirements: The following information is based on the auditing requirements of our accreditation agency, which are calculated and reported annually, November 30th of each year.

The 2011 Rates for all programs are as follows: Completion Rate: 86.11%

Licensure Rate: 100% Placement Rate: 83.87%

Compensation Expected - Cosmetologists

Cosmetologists can expect to make a national median wage of $23,300. In the state of Alabama,

cosmetologists can expect to make a median wage of $ $24,370. A commission scale is commonly used to pay cosmetologists resulting in much higher pay after an introductory period of several months. Retail commissions are also common. Many salons now offer benefits resulting in higher earnings. The compensation information is based on the information from O*NET, the Occupational Information Network.

Which is a comprehensive database of job information included wage information, job characteristics, and worker attributes. O*NET was developed by the U.S. Department of Labor and the Employment and Training

31 Administration (USDOL/ETA). The information provided is based on the SOC code 39-5012.00, Hairdressers, Hairstylists and Cosmetologists.

Physical Demands of Cosmetology

There are physical demands placed on the body in any career. In cosmetology, care must be taken to protect your back, legs and feet.

• One way to do this is strength training to enhance your back, abdomen and leg muscles. Regular weight lifting exercises will benefit individuals in this profession. Regular exercise will help to promote all over body conditioning, and will improve circulation in your legs and feet. We recommend consulting your doctor before beginning any exercise program.

•This job requires that you stand for long periods of time, so it is suggested that you wear proper fitting and supportive shoes and support hose. These are not a requirement, but will help increase your chances of longevity in the profession.

Safety Requirements of Cosmetology

Safety suggestions for this profession include:

• Wearing shoes that would not be slippery when walking on a damp floor. You will be working with water which means there is always a risk of water spills. All water spilled should be wiped up as quickly as possible after a spill. Damp hair lying on the floor can also pose a chance of slipping. All hair needs to be swept up immediately following each haircut to minimize accidents. It is the responsibility of each stylist to promote a safe working environment.

• Gloves are to be worn during chemical services to reduce the risk of any allergic reaction that an individual may have to certain chemicals.

• Anytime a product accidentally gets in your eyes, flush your eyes thoroughly with water. (It is recommended that this is done for at least 15 minutes.) Seek medical attention if irritation continues

The safety of our campus community is very important to The Salon Professional Academy. Involvement of our campus and community officials help ensure that we are able to provide a safe environment learning and growth on campus. The following information is provided regarding our campus security, and we ask that you read it carefully and ask questions about anything you may not understand. This information is provided to encourage a safe campus, and as a member of our campus, we ask that you share in this responsibility to prevent crime from occurring on our campus.

In compliance with the Campus Crime Statistics Act, we are required to provide you with our annual security report. Included in this report are statistics concerning reported crimes which have occurred on campus or on public property surrounding campus for the period of the previous three years. In addition, prior to October 1st of each year, The Salon Professional Academy updates this report and presents it to the students and staff. At this time a local police officer also comes on campus to discuss the topics covered in this report with the campus population. The Director of Education also utilizes this time to identify any hazards or other safety concerns that need to be addressed. To request more information on these requirements, clarifications on the procedures within this report or any information contained in this report, contact the Director of Education. Preventing campus crime is a responsibility that should be taken very seriously. Never assume that someone else has already reported a crime or suspicious activity. All criminal actions, suspicions or emergencies must be reported to an educator, administrator or owner immediately. The staff member will then assist you in reporting to the local police or other appropriate security force. A staff member made aware of a crime will notify the

32 rest of the staff as soon as possible, perhaps even prior to notifying police, depending on the situation. It is critical that all staff be aware of any report of crime, and that the local police be notified immediately. Timely Warning

The Salon Professional Academy will issue a warning when an incident that represents a serious or ongoing threat to the safety of the campus is brought to the attention of the administration or law enforcement or other offices. The administration will make every effort to inform the campus that a warning has been put in place within 24 hours of the determination being made. The decision to issue this alert will be based on the specific incident and the factors surrounding it. The decision to issue a warning will be made by the Director or the Director of Education and will contain as much of the following information as possible and if available: _ Date and time the warning was issued

_ Connections to previous incidents _ Composite drawing of the suspect _ Description of the suspect

_ Any other information that may be relevant _ Any information relating to tips to remain safe

Depending on the circumstances surrounding the warning, The Salon Professional Academy will inform the campus through means such as Facebook, signage in or around the building, or possibly notifying local television and radio stations.

Building Information

The facilities are open Monday through Saturday according to assigned class/styling area schedules. The building may also be open for educational classes for licensed professionals in cosmetology or to groups

securing the use of the facilities through the owner. Only educators, administrators and owners have keys to the building thus preventing internal crimes to as great an extent as possible.

The Salon Professional Academy also is protected by an alarm system and full sprinkler system as required by the city of Huntsville.

The Salon Professional Academy has no off-campus locations of student organizations which are officially recognized (examples include: sororities, fraternities, etc.).

Campus Safety

All reported incidents are reviewed and applicable information is collected and presented to the proper authority. In order to ensure that our campus remains safe, it is important for members of the campus to report this information in a timely manner.

Any suspicious activity should be considered a reasonable suspicion and reported. (Examples of suspicious activities include seeing an unescorted guest in an unapproved area, doors propped open, or unauthorized individuals using campus equipment or offices.) If it seems a situation appears abnormal to you in any way, report it.

The Salon Professional Academy does not have specific policies or procedures for confidential crime reporting, but crimes can be reported confidentially to the local authorities by simply requesting that the complaint be made anonymously.

33 Emergency Response and Evacuation Procedures

In the event and confirmation of a dangerous or emergency situation which could pose an immediate threat to the health or safety of students, employees or guests on campus, The Salon Professional Academy

administration and staff will immediately notify everyone on campus. Individuals will be notified of what the danger is and what actions should (or should not) be taken to best protect themselves from the danger. In addition to notifying individuals, the local Police Department and/or Sherriff’s Department will be contacted and will consult with the administration in an effort to maintain order and isolate the situation until the danger is contained. Additionally, The Salon Professional Academy will post timely written notices and warnings

explaining the danger and instructions on how to ensure the safety and health of individuals by avoiding the affected area.

On Campus Notification

Upon confirmation of an emergency or dangerous situation, The Salon Professional Academy will:

• Immediately notify all staff in the building. Staff members will then be responsible for notifying all students and guests, and ensuring the proper procedure for the specific danger (example: Fire Exit Procedure) is followed. • Contact the local authorities (if necessary) to assist in the containment of the emergency or dangerous situation.

• Contact the Director and Owner (if they are not presently in the building) to inform them of the situation and consult with them in an effort to maintain order and isolate the situation until the danger is contained.

• If the danger is contained to a small area of the building, the administration will post written notices and warnings on the inside and outside of all entry/exit doors and in other locations as necessary. The notices will include a description of the danger, what actions should be taken to minimize any possible harm, and an estimated duration of the danger. (If the danger consumes the entire building, the administration will post these notices, as possible, avoiding areas that could pose a threat of possible harm to staff members.)

On Campus Notification of a Potential Danger (Example: heavy storms or tornados)

If there is anticipation of a danger which causes the campus to close for a period of time, the administration at The Salon Professional Academy will:

• Notify local television and radio stations and post on the Academy’s Facebook page of the closure. • Post written notices and warnings on the inside and outside of all entry/exit doors and in other locations as necessary. The notices will include a description of the danger, what actions should be taken to minimize any possible harm, and an estimated duration of the danger.

• If the campus is open prior to the danger, the administration will notify all staff of the possible danger and what actions can be taken to avoid such danger. The staff will then notify all students and guests on campus who may be affected by the danger. If time permits, all guests who have scheduled appointments will be notified of the closure and possible rescheduling options.

Notifying the Local Authorities

Depending on the nature of the problem the proper authorities will be notified by the staff (or administration, if possible) of The Salon Professional Academy. Chosen authorities will be dependent on the nature of the danger, and could include any or all of the following: the Huntsville Police Department and the Huntsville Fire Department, Ambulance services or Emergency Services (911). At least once annually, the administration at The Salon Professional Academy will provide the Huntsville Fire Department and the Huntsville Police

Department with a floor plan of the building. The administration will also notify these agencies of any new construction, long-term functions or any other events which may affect routing or access to the campus. In addition to floor plans, the Huntsville Police Department, Huntsville Fire Department, and other local emergency officials are encouraged to tour and walk through the campus regularly.
