Our School Mission 

​The School of the Epiphany is a Catholic, Parochial school serving the 

youth of Epiphany Parish and its surrounding areas. We challenge  students to achieve educational excellence within a Catholic tradition. 

The School of the Epiphany operates in service to ​Epiphany Parish​ and  in the spirit of the Presentation Sisters. 

 

In collaboration with the San Francisco Department of Public  Health and the Archdiocesan Department of Catholic Schools,  School of the Epiphany has developed and created the following  framework for the reopening of our school building and campus. 

 

This plan will support families through in person learning or  distance learning if they chose not to return to school. Our school 

mission and School of the Epiphany does not unlawfully  discriminate on the basis of race, color and national and/or ethnic 

origin, age, sex or disability in administration of educational  policies, admissions policies, scholarship and loan programs, and 

athletic and other school-administered programs.

This Operations Plan has been updated to reflect the State of  California’s ​COVID-19 Reopening In-Person Instruction Framework  and Public Health Guidance for K-12 Schools in California, 2020-2021

As required, the school has posted a COVID-19 Safety Plan (CSP) on 

the homepage of the school’s website. The CSP includes the  school’s Cal/OSHA COVID-19 Prevention Program (CPP) and the  COVID-19 School Guidance Checklist. The detailed plans describing 

how our school meets the requirements outlined in the CSP  elements are found in this Operations Plan. 

     

All contents are subject to change in deference to local, state, and federal 

mandates.

General Protocols  

 

Social Distancing 

Social distancing is an effective way to prevent potential infection. Employees,  students, parents, and visitors should practice staying at least six feet apart from  others and eliminating contact with others whenever possible. 

 

Personal Protective Equipment 

Face masks will be required by all students and staff. They must be worn at all  times.  

 

Handwashing 

In addition to using PPE, please remember to: 

●  Wash your hands often with soap and water for at least 30 

seconds. Use hand sanitizer with at least 70% alcohol if soap and water  are not available 

●  Avoid touching your eyes, nose, and mouth 

●  Cover your mouth and nose or use the inside of your elbow when  you cough or sneeze 

 

Student Protocol and Gatherings 

●  All gatherings of large groups, including whole school masses and  assemblies, are prohibited. Our distance learning practices will serve as a  substitute.  

●  Students will remain with their classmates at all times, including  structured recess. 

●  Any student showing signs of illness will have their temperature  taken in the office, and students with a temperature above 100.4 degrees  or showing any respiratory symptoms will be isolated and sent home. 

●  Students will be mindful of the social distancing and health  practices in which they are instructed or they will not be allowed on  campus. 

●  Students will have a designated space solely for their use in their  classrooms and the extended care space (if applicable). 

●  Students will not share spaces, supplies, or work in groups  (distancing will remain throughout the day). Students will have  individual supplies at all times.

●  Students will use restrooms at designated times, and no more  students will be admitted into a restroom at a time than there are sinks  in the restrooms. 

 

   

 

Staff Protocols 

To help prevent the spread of COVID-19 and reduce the potential risk of  exposure to our employees, employees are required to conduct a 

self-screening of COVID-19 symptoms and pass a temperature screening on  campus entry (a temperature no greater than 100.4 degrees Fahrenheit will be  allowable). Staff with notable symptoms while at home will be expected to  report them to their supervisor and remain home. Staff experiencing 

symptoms may be asked to be tested for COVID-19 before returning to school.   

 

All teachers and students are not to visit another classroom unless absolutely  necessary. A login sheet will be used at the entrance of every classroom. Any  staff member that enters a classroom must write down their name on the  classroom log sheet.   

   

Campus Visitors  

The safety of our students and staff is our top priority   

● No visitors will be allowed on campus including parents until it is  deemed appropriate by The Archdiocese of San Francisco, SFDPH, and  School of the Epiphany. The office will be available via phone and email. 

● All campus visitors will undergo a health screening. Refusal of health  check would result in refusal of entry to the campus.   

● Parents may enter campus upon the expressed consent of the school  administration, for example picking up a sick child. 

● If a student forgets their lunch or other school sites, parents may drop  the items at the front door of the school after ringing the security bell. 

Office staff or administration will sanitize the items and then be dropped  off at the door of the student’s classroom.

At Home Health Screening   

If a child has a fever, or any COVID symptom, the requirement is to get tested, or stay  out of school for at least 10 days since symptoms began.   

 

Before leaving your home to arrive at school parents and guardians should take  their child’s temperature. If the child has a fever, over 100.4*, then the child  should not come to school. Also, children showing any of the following 

symptoms should not come to school: ​fever, cough, shortness of breath, fatigue,  body aches, headache, loss of taste or smell, sore throat, congested or runny nose,  nausea/vomiting, and diarrhea.   

   

 

On Campus Protocols  

The School of the Epiphany will be responsible for maintaining all health and  safety protocols in all areas of the school, including classrooms and 

instructional spaces. 

  

   

School Entrance Protocol   

The School of the Epiphany has created a new system of entering the school  building for students during morning drop off.   

     

Students who arrive by car: 

All students that arrive by car will pull up to the curb in the white zone on Italy  Ave. Students must remain in the car until they are approached by a staff 

with EPA approved  solution to kill COVID-19

member and undergo a health and temperature check. Students will then be  instructed to enter the building through the grade specific entrance.   

   

Students who walk to school: 

Students that walk to school will line up in grade specific areas on Italy St. 

Social distance at 10ft will be applied in these lines. The sidewalk will be  marked with these 10ft foot spaces for students to wait.   

     

Temperature Checks  

Before entering the school building, all students must have their temperature  checked. If a child has a fever of 100.4* or above, he/she will not be allowed to  enter the school building.  

   

Health Screening 

Before entering the school building or exiting their car in the white zone, all  students (or guardians on behalf of students) must respond to the prescribed  health screening. If a student (or guardian on behalf of a student) responds  with an affirmative to any of the screening questions, he/she must not enter  the school building. Health Screening Questions: 

1. Do you or your child live with anyone or have you or your child had close contact  with anyone with a prolonged cough, fever, flu-like symptoms or been diagnosed 

with COVID-19 within the last 14 days?  

2. Do you or your child live with anyone or do you or your child have a fever, cough  and/or shortness of breath? For children and adults, fever is 100.4 degrees or above 

using a forehead thermometer. 

3. Do you or your child live with anyone or do you or your child have any other signs  of communicable illness such as a cold, flu, rash or inflammation?  

4. Do you or your child live with anyone or have you or your child experienced  diarrhea or vomiting (within the past 24 hours)?

Refusal of Health Screening 

Refusal of a health check will result in refusal of entry to the campus and  school building.   

    Contacting Tracing  

Each staff member and visitor will fill out a form asking for their name, phone  number, when they entered the school building, and their estimated departure  time. This information will be used if needed for contract tracing but will also  serve as a log for those who have been on campus.

   

   

Student Cohorts 

Student cohort lists will be ready for review by the visiting group from the  SFDPH. The list will be kept in the main office. Individual classroom lists will  be posted outside each individual classroom for review by the SFDPH.   

   

Stable Student Cohorts 

Students will remain in stable cohort groups by grade levels. Students who are  participating in school learning are expected to be in school on a daily basis.   

Under advice of the SFDPH in order to maintain a stable and healthy cohort  environment within our school the following requirement will be followed at  School of the Epiphany: 

 

If your child is absent or sick from school they may return to school under any  of the following circumstances: 

● Medical Note: A medical note from your child’s doctor stating that they  have a specific illness that is not COVID (example: strep throat). When  the school receives this doctor's note they will be allowed to return to  school the following day.   

● COVID-19 Test: If the student takes a CODVID19 test that shows they are  negative they are allowed to return to school the following day.  

● 10 Day Waiting: If the student does not take a COVID 19 test or receive a  doctor’s note they will be asked to stay home for a 10 day quarantine  period. During this time they will participate in online learning.

Drop Off Maps    

 

Students in grades Kindergarten, 1st, and 2nd Grade will enter through the  Italy St. door. They will then proceed to the 1st floor of the school via the Italy  St. Stairs.   

 

Students in grades 3rd, 4th, and 5th will enter through the Italy St. Convent  Doors. They will then proceed to the 2nd floor of the school via the Convent  stairwell.   

 

For students that walk to school, a line with 10ft distancing between students  will form down Italy St. and then down Naples St for their temperature and  health screening.

Students in grades 6th, 7th, and 8th grades will enter the campus through the  schoolyard gate on Italy Ave. 6th, 7th, and 8th grade students that are walking  to school, will line up along Italy St. and onto Vienna (if needed) for their 

temperature and health screening.

Arrival/Drop of Schedule: 

 

 

After students have been health screened and temperature checked they will be  allowed to enter the school building. Prior to entering the building students  will sanitize their hands using hand sanitizer. Students will then proceed  directly to their classroom where they will be admitted by their teacher. 

Students will enter the classroom and go directly to their assigned seat.

End of the Day Pick Up Schedule   

Traditionally, School of the Epiphany has used the white zone area for the car  line pickup at the end of the school day. Under these circumstances we will be  moving the car line pickup into the schoolyard. No parents or guardians may  enter the school yard to pick up their child, only cars may enter the yard. The  map below shows the flow of traffic to pick up your child in the yard.   

Parents will drive their cars into the yard. At that time they will tell a staff  member what student(s) and grade of the student they are there to pick up. 

Students will then be directed to the correct car. There will be a staggered  dismissal time for car pick up and the dismissal of students who are walking  home from school or are being picked up by an adult who is walking.  

 

Students that are walking home independently will leave onto Naples St. via  the side door between the church and school   

 

Students that are being picked up by a parent who walks to school will go to  their assigned area in the schoolyard. A staff member will then direct the  student to their parents. Parents will pick up the student at the gate closest to  the church on Vienna St.

Clean and Healthy Classrooms and Campus   

● Hand sanitizer will be used by students and staff when entering and  exiting classrooms.   

 

● Hand sanitizers will also be available in common areas of the school  building, including school entrances, hallways, the main office, and all  other rooms in the building.   

 

● Face masks will be required by all students and staff. They must be worn  at all times.  

 

● Commonly touched surfaces, such as door handles, light switches,  student chairs, desks, etc. will be part of routine cleaning. These will be  cleaned a minimally four (4) times per day. This includes after morning  recess, after lunch recess, before dismissal, and after eating in class.  

 

● Classrooms will be cleaned by a janitorial service company on a nightly  basis.  

 

● Students may assist with cleaning only when there are child-safe 

cleaning products to use. Students would only clean their desk and chair. 

They would not clean commonly touched areas.   

 

● Water fountains will be shut off. Students will be encouraged to bring  reusable water bottles to school. Students will be allowed to bring 

multiple bottles, they will not be allowed to refill water bottles at school.   

 

● Windows will remain open to keep rooms ventilated with fresh air at all  times.   

 

● HEPA Air Filter/ Purifiers will be present in each classroom to help clean  the air on a continuous basis.

Inclement Weather Plan/ Fresh Air  

In the event of rain the following procedures will be followed to ensure the  classrooms have fresh air at all times. If it is raining outside the windows of  the school will remain open. Students will be allowed to wear coats in the  classroom. The heater (boiler) will also be turned on to ensure warmth for the  student. The boiler is on a timer and the classrooms can be warmed in the  morning before students come to school.   

     

Social Distancing in the Classroom   

Student desks in all classrooms will be arranged in a manner that meets the  SFDPH mandate for safe social distancing of desks within the classroom.   

All students in grades K through 5 will be able to accommodate the full  enrollment of each class based upon the mandate of the SFDPH. 

Grades 6 through 8 have the possibility of coming to school on a hybrid  schedule or full time depending on classroom size, enrollment, and SFDPH  mandate for spacing of student desks.   

   

Classroom Supplies   

● Students will never share any type of supplies with each other. All  students in every grade are required to have their own classroom  supplies that are on their grade level specific supply list.   

 

● Classroom computers and ipads that are usually shared by students will  be cleaned after every use. Devices will be cleaned with approved wipes  after each student use.   

 

● Students will have their own tissue box that will remain on their desk at  all times   

 

● Any additional supplies, such as cleaning wipes and hand sanitizer will  be managed by the classroom teacher and aide.

● ***​Students will also be required to bring a “Safe at School” Kit. The  SAS Kit will consist of the following items within a quart sized ziploc bag.  

■ Two additional face masks 

■ A small package of antibacterial wipes 

■ Personal sized hand sanitizer   

 

Lockers   

Middle school students will not be allowed to use lockers. All students will  keep their supplies and books in their backpack and desk at all times. Lockers  will be blocked off to prevent student use.

Signage in School   

School of the Epiphany has received posters from the SFDPH that encourage  social distancing and wearing face masks. Signs and posters will be placed  outside the building and in the stairways and hallways of the school.   

 

   

 

Additional signage will be used throughout the school in the hallways,  stairwells, inside and outside classrooms, and bathrooms to: 

● Direct movement of students and staff to move easily throughout the  building. “STAY TO THE RIGHT” will be the term used throughout the  school  

● Remind students about safe hygienic practices and routines 

● Provide reminders and guidance about safe distancing. Stay 6ft apart 

● Remind students of closed areas such as water fountains    

 

Social Distancing with the school   

When school is in session, all adults and children will enter and exit the 

building following marked pathways (see above). A staggered schedule will be  used for all aspects of school including arrival, dismissal, recess, and bathroom  breaks. This is to ensure social distance protocols and avoid crowding of spaces  on campus.

Signage and floor markings have been purchased to indicate the directions of  foot traffic in all the hallways and stairwells. Foot traffic will move in two  directions, students will follow arrows on the floors to maintain social  distancing.

All bathrooms will be marked with occupancy limits throughout the campus  and school building. Floor signs will be used outside of all bathrooms for  students waiting to use the restrooms. Bathroom doors will remain open at all  times.   

   

Teachers and staff will be responsible for instructing their homeroom classes  in social distancing while in the classroom and walking around campus. They  will be giving explicit reinforcement of hallway walking flow, entrances, exits,  and all rules related to safe social distancing.   

   

Outdoor Instruction 

The outside schoolyard space will be used for Physical Education classes once a  week for each of the homeroom classes.   

The yard could possibly accommodate a dedicated outdoor space for a Library  classroom. This would be dependent on weather conditions.   

 

Code of Christian Conduct  

It shall be an express condition of enrollment that the student behave in a  manner, both on and off campus, that is consistent with the Christian 

principles of the school as determined by the school in its discretion. These  principles include, but are not limited to any policies, principles, or procedures  set forth in the student/parent handbook of the school and newly added 

policies, principles, or procedures in this Return to School Plan.  

   

Policy for students that do not follow Health and Safety Protocols  The school reserves the right to determine, in its discretion, which  actions fall short of meeting the Christian principles of the school. 

Failure to follow these principles will normally result in verbal or  written warning to the student and/or parent/guardian and normally  will first result in disciplinary action short of requirement to withdraw  from the school (e.g. suspension of student or suspension of 

parent/guardian’s privilege to come on campus grounds and/ or  participate in parish/ school activities, volunteer work, etc.)

The school reserves the right to determine, in its discretion, when  conduct is of such a severe nature as to warrant immediate action  without a warning and /or without an intermediate step short of  withdrawal.  

 These rules also apply to students following the Health and Safety  Protocols set forth in this plan. If a student does not follow the mask  wearing protocols, spacing of 6ft between themselves and another 

student, and other protocols they will face the above disciplinary actions. 

  

Rules Regarding Misuse of PPE 

All personal items and/or school provided materials must be used 

appropriately as delineated in the school handbook. Misuse of items will  result in a school/home consequence and reparations made for any  materials that are broken or damaged. 

   

Distance Learning 

In the event of a school closure due to SFDPH, State of CA, or Archdiocese  mandates the school will revert to its Distance Learning program. This plan  can be used during a day when the air quality is too poor for students to attend  school. This plan/program was mailed to all family members, it is also 

available on the schools website at sfepiphany.org

Communication with Parents 

School of the Epiphany has posted these Return to School Plans on its website. 

We also have the ability without alert system ​OneCall ​to email and text all  parents at any time. This system can immediately notify parents of any 

important information or updates that they school needs to provide to them.   

   

Parent Orientation for Return to School 

Parents who are choosing to send their children back to the school building  will be required to attend a ZOOM training session on the return to school  protocols in this plan. This training would take place on ZOOM and conducted  by the principal, Brian Joost.  

 

Staff Training 

Staff members will also be trained on the procedures and protocols written in  this plan. The training will be conducted by the Principal, Brian Joost, and will  take place prior to the reopening of school.   

   

STAFF TESTING FOR COVID 19  

The Cluster II/IV Schools for the Archdiocese of San Francisco are working  together to have the company, Agile Force COVID-19 Testing, test our staff 1-2  weeks before the possible return date. The teachers would then be tested on a  rotational basis with 50% (approximately 10 teachers/staff members) being  tested every month. All staff returning to the building for November 9, 2020 in  person instruction will be tested on October 26, 2020 by Agile Force COVID-19  Testing. The next testing date will take place on November 23, 2020 with Agile  Force COVID-19 Testing. Teachers and staff may also obtain testing through  their own health care provider given that results of the test are made available  to them within at most 72 hours of their test. All results of teacher and staff  testing will be kept on file in the school office and shared with the San 

Francisco Department of Public Health Staff may not return to campus until  they show a negative test result.

Updates to COVID-19 School Operations Plan 

January 20, 2021 

​Face Coverings: 

Students in all grade levels K-12 are required to wear face  coverings at all times, while at school, unless exempted. 

All staff must use face coverings in accordance with CDPH  guidelines 

● CPDH recommends disposable 3-ply surgical masks, which  are more effective than cloth face coverings 

● For staff, bandanas, gaiters and similar face coverings are no  longer acceptable. 

● In limited situations where a face covering cannot be used for  pedagogical or developmental reasons, a face shield with a  drape can be used as long as the wearer maintains physical  distance from others 

Classroom Spaces 

● Distance teacher and other staff desks 6 feet away from  student and other staff desks 

● Distance student chairs at least 6 feet away from one another,  except where 6 feet of distance is not possible after a 

good-faith effort has been made 

● Under no circumstances should distance between student 

chairs be less than 4 feet

● If 6 feet of distance is not possible, optimize ventilation, use  desk partitions, or arrange desks to minimize face-to-face  contact 

Positive COVID-19 Case 

In the event of a positive case, the school will: 

1. Schools must adhere to required reporting requirements and  notify, as described below, the LHD of any newly reported case  of COVID-19 in a student or staff member if the LHD has not  yet contacted them about the case, 

2. If the case is present at school at the time the school is 

notified, the case must go home and be excluded from school  for at least 10 days from symptom onset date or, if 

asymptomatic, 10 days from the date the specimen was  collected for the positive test. 

3. Assist LHD in identifying close contacts of the case. Send a  notice, developed in collaboration with the LHD, to parents  and staff to inform them that a case of COVID-19 in a student  or staff member has been reported and that the school will  work with the LHD to notify exposed people.  

4. Arrange for cleaning and disinfection of the classroom and  primary spaces where case spent significant time. This does  not need to be done until students and staff in the area have  left for the day. 

5. Implement online/distance learning for student cases if they  are well enough to participate. 

Required COVID-19 Reporting 

● The school will notify the local health officer of any known 

case of COVID-19 among any student or employee who was 

present on a campus within the 10 days preceding a positive 

test for COVID-19. Specifically, the school shall report the 

following information:

○ The full name, address, telephone number, and date of  birth of the individual who tested positive; 

○ The date the individual tested positive, the school(s) at  which the individual was present on-site within the 10  days preceding the positive test, and the date the 

individual was last on-site at any relevant school(s); and 

○ The full name, address, and telephone number of the  person making the report. 

● This information shall be reported to the local health officer  by telephone within ​twenty-four hours​ from the time an  individual within the local educational agency or private  school is first made aware of a new case.

When either a school or LHD is aware that an outbreak may be  underway, the LHD should investigate, in collaboration with the  school, to determine whether these cases had a common exposure  at school (e.g., a common class or staff member, bus ride, or other  common exposures outside of school). CDPH defines a school  outbreak as 3 or more confirmed or probable cases of staff or  students occurring within a 14-day period who are 

epidemiologically linked in the school, are from different  households and are not contacts of each other in any other 

investigation cases (e.g., transmission likely occurred in the school  setting). 

During the investigation, the school will: 

1. Notify parents/guardians and school staff of a 

cluster/outbreak investigation related to the school and  encourage them to follow public health recommendations. 

2. Identify, as part of the CSP, one or more school staff member 

who can liaise with the LHD regarding the cluster/outbreak

investigation by confirming which classes and stable groups  included confirmed cases or symptomatic students and staff  members, and if recent events or gatherings involved any  cases or symptomatic persons. 

3. Identify absenteeism among those in affected classes or stable  groups, and coordinate with the LHD to contact these 

absentees to screen for symptoms of COVID-19 if they were  exposed to a case during the case's infectious period. 

4. Coordinate with the LHD to share a line list of cases and  contacts with dates present at or absent from school. 

5. Arrange for cleaning and disinfection of classrooms or other  areas where cases or symptomatic students or staff members  spend significant time. 

6. Coordinate with the LHD on notifications to the school 

community, including specific notifications of stable groups  or classrooms regarding their exclusion status and 

instructions. 

7. Coordinate with the LHD on whether and when the school  should be closed and reopened. 

8. Notify the school community if the school is to be closed for 14  days due to widespread and/or ongoing transmission of 

SARS-CoV2 at the school or in the general community, and  repeat recommendations for prevention and control measures  (see sample notification #3 in Appendix 2). 

9. Implement online/distance teaching and learning during  school closure. 

10. Arrange for cleaning and disinfection of entire school  before reopening in the case of closure. 

​School Closure Determinations 

Individual school closure, in which all students and staff are not on 

campus, is recommended based on the number of cases and stable 

groups impacted, which suggest that active in-school transmission

is occurring. Closure should be done in consultation with the LHO. 

Situations that may indicate the need for school closure: 

● Within a 14-day period, an outbreak has occurred in 25% or  more stable groups in the school. 

● Within a 14-day period, at least three outbreaks have occurred  in the school AND more than 5% of the school population is  infected. 

● The LHO may also determine school closure is warranted for  other reasons, including results from public health 

investigation or other local epidemiological data. 

Length of closure: 14 days, or according to a decision made in  consultation with the LHO. 

​Testing 

Symptomatic testing: If a student or employee exhibits COVID-19  symptoms, the individual is sent home (or stays home). Testing for  COVID-19 is encouraged. The student or employee can use the 

testing sites available on the County’s website or make an  appointment to see their medical provider. While waiting for 

results, the student or employee will not be able to return to school.  

Response testing: If a student or employee is exposed to a  confirmed case of COVID-19, the student or staff member is  required to quarantine for 10 days. Testing for COVID-19 is  encouraged. The student or employee can use the testing sites  available on the County’s website or make an appointment to see  their medical provider.  

Asymptomatic testing: Employees participate in asymptomatic 

testing. The school has established a schedule for testing so that 

employees are tested once every two months. The employees use 

the testing sites available on the County’s website.

​Travel Requirements 

The State of California has updated its travel policy with the  following provisions: 

● Non-essential travel (for recreational purposes) should be  limited to 120 miles from the person’s home. 

● Californians who travel outside the state or the country must  quarantine for 10 days upon their return to California. 

● Visitors to California from other states and countries must  also quarantine for 10 days upon their entry into California. 

With this in mind, please limit recreational travel to no more than  120 miles from your home. If you must travel outside of California  or outside of the United States, you will need to quarantine for 10  days upon your return to California.
