Making the Connections - A Transition Resource Guide

This guide is designed to help parents, family members and students locate

resources for future planning. The resources listed will assist students to make

connections to services that may be needed during and after the high school

years.

It’s never too early to start the transition planning process. By age 14, and

every year thereafter, every student and his/her transition team should be

discussing activities that promote movement from high school to post high

school activities.
