SPONSORING AGENCY QUOTA(SAQ AND DQ) ADMISSION SESSION 2021-22
CLASSES II TO IX
Parents of the children whose names have been recommended by Chairman,VMC, KV No.4,Bhubaneswar for admission of their ward in different classes (other than class-I )are hereby required to follow the given points below for admission of their ward for the session 2021-22.
1. Parents to complete all admission formats given below and send the scan copy to email address [email protected] by 13.06.2021 for documents verification.
a. KV No4, BBSR Admission form filled and signed by parent(Page No1 and 2 of Annexure-I) b. KV No4, BBSR Admission Registration form completed in all respect with sign and seal from
competent authority and parent in the proper places.(Page No. 3-6 of Annexure-I )
c. Self-attested copy of address proof in the parents name only (Like Aadhar card /Voter ID card/ Gas connection/Passbook/Office quarter allotment letter/ Rent Agreement etc)
d. Salary slip of May 2021 paid in June.
e. CBSE School TC in original and Non-CBSE Original TC countersigned by DEO/BEO
f. If no TC is there, up to class- VIII - Original affidavit regarding home study till date from First class executive magistrate along with original birth certificate of the child.
g. Self-attested copy of Caste certificate if any. h. Self-attested copy of Disability certificate if any.
i. Self-attested copy of Single girl child certificate in the given format if any.(Page no.7 of Annexure-I)
j. Self-attested copy of Aadhar card of the child.
k. Self-attested copy of Blood Group Certificate of the child.

Self-attested copy of Birth certificate of the child.m.
Two passport size photographs with name written on the back side.2.
At the time of document verification if any information found to be false or the parent is unable to submit the document in support if their claims their admission will be rejected.3.
After the documents are verified, parents shall be intimated on 14.06.2021, if any lacuna is there in documents.4.

Parents to collect the fees payment challan from the security room near school gate on 18th June 2021 at 9 a.m. for payment of school fees online/ offline in any of the Union
Bank of India branch on the same day.
