NOTIFICATION OF ADDENDUM ADDENDUM NO. 2

DATED 2/08/2010

Ladies/Gentlemen:

Attached please find an addendum on the above captioned project. Included in the attachment is an adendum notification which details the changes and the respective proposal pages which were added and/

or changed.

Except for new bid insert pages, it is unnecessary to return any of the pages attached.

Bid insert pages must be returned with the bid proposal submitted to the Department, unless your firm is submitting a bid using a computer print out. The computer print out must be changed to reflect the new bid item information.

Contractors and material suppliers, etc. who have previously been furnished informational proposals are not being furnished a copy of the addendum. If you have a subcontractor on the above project, please advise them of this addendum. Acknowledgment of this addendum is not requested if your company has been issued a proposal stamped “This Proposal Issued for Informational Purposes.”

You are required to acknowledge receipt of this addendum on the Addendum Acknowledgement form contained in your bid proposal by placing a mark in the box next to the respective addendum.

Failure to Acknowledge receipt of this addendum in your bid proposal will result in your bid not being read.

2004 Specifications

SPECIAL PROVISION 429---008

Concrete Structure Repair

For this project, Item 429, “Concrete Structure Repair,” of the Standard Specifications, is hereby amended with respect to the clauses cited below, and no other clauses or requirements of this Item are waived or changed hereby.

Article 429.2. Materials is voided and replaced by the following:

Provide materials in accordance with the requirements of the following Items:

 Item 420, “Concrete Structures”

 Item 421, “Hydraulic Cement Concrete”

 Item 431, “Pneumatically Placed Concrete”

 Item 440, “Reinforcing Steel”

 DMS-6100, “Epoxies and Adhesives”

 DMS-4655, “Concrete Repair Materials”

A. Pre-approved Materials. Submit all proposed repair materials for approval. The Engineer may disallow any product based on its structural compatibility or appearance. Provide repair materials suitable for the appropriate horizontal, vertical, or overhead application meeting the requirements in DMS-4655 for Type A or D materials unless noted otherwise.

Prepackaged repair materials not meeting the requirements of DMS-4655 or not on the Department’s list of pre-approved materials may be used only with approval. In addition to the material requirements in DMS-4655, use repair materials with a Modulus of Elasticity (E) greater than 2500 ksi for horizontal repairs greater than 4 in. thick and vertical or overhead repairs greater than 1 in. thick. Use ultra-rapid hardening materials (Type B per DMS-4655) only if indicated on the plans. Use only Type A materials per DMS-4655 for repair of prestressed or post-tensioned concrete. When needed, use a coarse aggregate whose maximum size is less than 1/3 the thickness of the thinnest portion of the repair.

Follow the manufacturer’s recommendations for type and quantity of aggregate used. Do not use materials containing corrosion inhibitors unless indicated on the plans or approved.

B. Pneumatically Applied Materials. Pneumatically applied concrete or mortar may be used, if approved, for any repair thickness. Provide and place pneumatically applied concrete in accordance with Item 431, “Pneumatically Placed Concrete”. Prepare trial batches of any proposed repair material and application method as directed.

C. Epoxy Mortars. Epoxy mortar, using a Type VIII epoxy per DMS-6100, may be used for vertical/overhead repairs less than 1 in. thick unless noted otherwise.

D. Concrete. Unless noted otherwise provide Class “C” concrete for substructures, Class “S”

 as an option for vertical/overhead repairs greater than 6 in. thick;

 full or partial depth slab repairs;

 for replacement of entire members or elements;

 and as an option for horizontal repairs greater than 4 in. thick

Submit a mix design for approval in accordance with Item 421, “Hydraulic Cement Concrete,” with a water-cement ratio less than 0.45, unless otherwise shown on the plans.

Include all pertinent information on admixtures. Do not use corrosion-inhibiting admixtures unless indicated on the plans or approved.

E. Steel. Provide steel pins, studs, or expansion bolts with a minimum diameter of 1/8 in. and a minimum length of 2 in. to attach reinforcement at the locations shown on the plans or as directed. Provide reinforcing steel, either welded wire fabric or reinforcing bars, as required by this Item or as shown on the plans.

Article 429.4. Construction, Section B. Surface Cleaning. The first sentence is voided and replaced by the following:

Clean the area to be repaired by abrasive blasting, water blasting or other approved methods.

Article 429.4. Construction, Section F. Substrate Preparation. The second sentence of the second paragraph is voided and replaced by the following:

If ponding is not possible, achieve SSD conditions by water blasting 15 to 30 min. before placing the repair material.

Article 429.5. Measurement is voided and replaced by the following:

This Item will be measured by the square foot, in place, as measured on the specified horizontal,

vertical or overhead surfaces of the completed repair as shown below or by the cubic yard for

full element or member replacement. When a repair involves multiple surfaces, such as a corner,

measurement will be made of all surfaces repaired. Bridge deck repairs will be measured by the

square foot in place of the completed repair.

Article 429.6. Payment is voided and replaced by the following:

The work performed and materials furnished in accordance with this Item and measured as provided under “Measurement” will be paid for at the unit price bid for “Concrete Structure Repair (Horizontal)”, “Concrete Structure Repair (Vertical or Overhead)”, “Concrete Structure Repair (Bridge Deck)(Partial Depth)”, “Concrete Structure Repair (Bridge Deck)(Full Depth)”, and Concrete Structure Repair (Remove and Replace)”. This price is full compensation for furnishing,placing, and curing all repair materials; removing all loose and defective concrete;

saw-cutting; cleaning reinforcing steel; supplying and installing replacement or supplemental

reinforcing steel, drive pins, studs, or expansion bolts; and equipment, labor, and incidentals.
